Our Values The values of EFAD guide how we make decisions and define our approach to partnership and collaboration. Our core values are: fairness, openness, non-discrimination, collaboration and independence. These are expressed more fully in the way EFAD conducts its business, observing our Code of Ethics and Code of Good Practice (p 9) and being:

EFAD, national dietetic associations (NDAs) and dietitians are recognised leaders in the field of dietetics and nutrition. We will continue to pro-actively initiate and grow partnerships in order to improve nutritional health, reduce socioeconomic health inequalities and contribute to economic prosperity. To achieve our vision EFAD supports the highest quality of dietetic education, professional practice, research activity and partnership.

About EFAD EFAD was established in 1978 in Copenhagen, Denmark with 10 member associations. It now has 29 full members and 2 affiliate members, with 4 applicant members (RO, CZ, LT, SK) representing over 30,000 dietitians in 29 countries.

Membership of the Federation is open to the NDAs of all member states of Europe. The General Meeting is made up of delegates from each of the member Associations. The Federation is directed by its Annual General Meeting of Delegates and represented by an Honorary President and Executive Committee. EFAD has three standing Committees:

Education and Lifelong Learning Professional Practice Research and Evidence-Based Practice

In 2010 the Education and Lifelong Practice Committee began work on developing further the EU-funded Thematic Network ‘Dietitians ensuring education, teaching and professional quality’ (DIETS2)

Aims of EFAD

The aims of EFAD are to:

promote the development of the dietetic profession develop dietetics on a scientific and professional level in the common interest of the

member Associations facilitate communication between NDAs and other organisations: professional,

educational, and governmental encourage a better nutrition situation for the population of Europe.

EFAD pursues these aims in co-operation within the member Associations and with international organisations. Our stakeholders include dietetic associations, dietitians, citizens, key national ministries, higher education institutions, non-governmental organisations, food industry, policy-makers and other health professionals. Our engagement with these groups reflects our independence and acts in the best interest of the populations we serve. By embedding dietetic education, professional practice and research activity in dietetics throughout Europe, we enable high quality engagement at all levels, thereby addressing health determinants and creating supportive environments for healthy lifestyles and prevention of disease through nutrition. EFAD communicates its aims and objectives by fostering exchange of knowledge, best practice promotion and action with and between our stakeholders.

Honorary President’s Report I am delighted to be writing this annual report after serving for one year as Honorary President. I must pay special tribute to my predecessor, Karin Hådell, for all the fine achievements of the Federation during her tenure. In these times of heightened awareness about the role of food, nutrition and physical activity in the health and prosperity of Europe, it is essential that dietitians and their contribution to the European agenda is clarified and promoted. Over the past year, two key position papers have been published to reflect our priorities of education, food and health. They are The Role of the Dietitian in the Prevention and Management of Malnutrition in Adults and The Role of the Dietitian in the Management of Obesity (see Appendix 9). Both position papers have been distributed at notable conferences (ECO2011 and ESPEN 2011 respectively) and have raised the profile of dietitians to other health professionals. EFAD has also continued to make commitments to European projects such as the EU Platform on Nutrition, Physical Activity and Health and EURRECA as well as a proactive contribution to EPHA and ENHA. EFAD was invited to be a Member of Scientific Advisory Board of an EU project: “Fibres as support of the Gut and Immune function of Elderly - From polysaccharide compound to health claim” (FibeBiotics). A full list of the meetings where EFAD has been represented is at Appendix 6. EFAD was also invited to be part of the Action Network for Hospital Nutrition, which is a WHO European Region facilitated Network organised by four European countries. Presenting EFAD and the role of dietitians at European level is a high priority but so too is the need to listen and assist where requested at national level. EFAD representation at high level meetings, for example with Members of the European Parliament, Commissioners and Ministers of Health, can be very much more effective if our representatives are aware and know more about national dietetic priorities. Learning more about opportunities and excellence at national level also allows the Federation to support its members through dissemination. I am particularly grateful to the Secretariat for the role they play in achieving this interaction. Over the past year EFAD has accepted invitations to attend the National Association meetings in the Netherlands, Austria (50th Anniversary celebrations), Germany, Italy and Greece. On EFAD's behalf, I was able to write in support of the German Association to the Minister of Health concerning the development of degree level entry to the profession and a change of title. We now know that a new initiative has been started to enable degree level dietitians to be educated in Germany. A key characteristic of any Federation is to have clarity with respect not only to its role but also to its governance and financial security. Through the very able work of its Honorary Treasurer, EFAD is able to undertake its core activities with confidence. The Honorary Treasurer will write more of this in his report later. However, if the Federation is to work more strongly in Europe, then further resources need to be available. Over the past year a list of experts has been drawn up, supplemented by an emphasis on groups of specialist dietitians meeting together at the annual DIETS/EFAD Conference. The identification of this resource has allowed EFAD to develop briefing papers and position statements, and also to be represented at European level by highly knowledgeable dietitians. To fully brief our members, to present the role and function of the Federation, the Executive Committee and the General Meeting of Delegates have identified our mission, vision, values and plans in a new Strategic Plan 2011-16.

The governance of EFAD has also been strengthened over the past year largely due to the work of our Secretariat. The Secretary General will report on these initiatives and successes elsewhere. However, clarity on policy (for example sponsorship, transparency, and criteria for membership) has been tackled efficiently and effectively. The ambitious Strategic Plan has been prepared for 2011- 16 and will require the Federation to find additional resources if it is to be brought to fruition. In May 2011 we submitted an Operating Grant proposal, to the European Commission’s Executive Agency for Health and Consumers (EAHC). Other income streams are already in place such as funding from the EACEA for the DIETS2 project and a collaborative project with Brunel University, England and the European Food Information Council (EUFIC). EFAD was also part of two other European project bids during 2010-11 and we continue to receive offers to collaborate and bring dietetic expertise and knowhow. Part of the role of the Honorary President is to initiate and enhance the profile of the Federation. In May, I was privileged to lead a delegation to Copenhagen to discuss EFAD and its potential contribution to the Strategic Objectives of the WHO and in particular the European Region. The discussion was potentially very fruitful and will be vigorously pursued. Our three new committees will report elsewhere, but their work in the areas of education, research and professional practice have been highly valued by the Executive Committee. They are moving towards fulfilling our strategic goals and have initiated scoping exercises and provided leadership in these three key areas for the profession in Europe. Finally I need to acknowledge the work of the Associations in responding to the calls made on their time and expertise to comment and support their Federation. The review of the governance structure, development of the Strategic Plan and support given to the work of the Committees has been invaluable and enabled the Federation to represent the profession at European level. The Executive Committee meets every month to operationalise our plan and I would like to acknowledge the unstinting giving of their time. The Secretariat, so ably led by the Secretary General, has once again allowed the Federation to show itself as an agent of change and I believe that this is what our members and those concerned with European nutritional health want and deserve.

Honorary Treasurer’s Report EFAD has stabilized its financial situation in the previous and current year. Income remains at round 90.000 € per year, including fees from the National Dietetic Associations, donations and refunds. A moderate increase of expenditure in 2011 is due to the consultants and subcontractors costs. The launch of three new Committees and increase in size of the Executive Committee with increased travel and activity cost will also increase expenditure. In anticipation of these costs, the surplus from 2010 and a small withdrawal from the deposit account will cover these additional costs. For an overview of income and expenditures, please see pages 10 and 11. Ulrich Huehmer

Secretary General’s Report I welcome the opportunity to write my first Secretary General’s report in this, the first “EFAD Annual Report”. I took on the newly created, part-time (18 hours a week) role of Secretary General on 1st January 2010. Since then, a great deal of progress has been made, which I would like to share with you. I have ordered my report in accordance with the “Role of the Secretary General”. Development of EFAD strategies and policies Our primary development has been the drafting of the EFAD 5-year Strategic Plan 2011-2016, which will guide the work of all EFAD members and officers. It will harness our potential to improve the nutritional situation for the people of Europe through the development of the profession of dietetics within all member states. To support the implementation of the Strategic Plan we have applied for an operating grant. The preparation of this application required an enormous commitment from each of the Executive Committee members, but brought several added benefits, not least of which were the development of vision, values and transparency statements. Within the reporting period the issues of sponsorship, membership and representation have each been considered and draft policies developed. There are now clear guidelines to inform decisions on sponsorship and collaboration opportunities, which will ensure that EFAD maintains its autonomy as it increases income. The policy on membership applications will ensure transparency in the process of accepting new members. It will also ensure that EFAD continues to offer support to associations in countries where the profession of dietetics is not yet fully established. The policy on the acceptance of invitations to attend meetings and conferences (or to enter into collaborations) will enable EFAD to be represented when this will contribute towards meeting EFAD’s strategic plan. Declining invitations that do contribute will save money and manpower by. Lists of all the invitations that EFAD has received (57 so far in 2011), both for collaboration and attendance of events, are published on the website. Everyone who attends an event as an EFAD representative is asked to write a report after the event, identifying the benefits to EFAD, which is published on the website. This enables strategic opportunities to be identified and developed. Support the Honorary President, Executive Committee (EC) and other Committees The Education, Research and Professional Practice Committees have each been established and have started to meet regularly to ensure that they are working towards meeting their deliverables. The Chair of each committee is also an EFAD Executive Committee member, which means the EC now has seven members in addition to the Honorary President. The new committee chairs have conscientiously undertaken their additional roles as EC members, bringing new ideas and enthusiasm to the committee. Since January 2010, the EC has met monthly, using internet-based virtual meeting technology to save time and money. Monthly meetings have enabled more efficient running of the organisation, with a shorter time-frame for decision-making. A list of Action Plans captures all the items that the EC is working on – ensuring efficient delivery of outcomes. This is only possible with the support of the Executive Committee members, who each voluntarily contribute many hours of their time to EFAD each year.

Anne de Looy took on the role of Honorary President in September 2010, bringing with her a whirlwind of activity! I still feel it only appropriate to acknowledge her extraordinary vision and her enthusiasm and commitment to making it a reality. In January 2011 Vanessa Teutschländer joined EFAD as an administrative assistant. Vanessa works from a home office, based in Hamburg, Germany to provide valuable support by maintaining the EFAD and DIETS websites, writing meeting minutes and undertaking general administrative duties. Vanessa’s positive attitude and enthusiasm contribute greatly to the smooth running of the secretariat.

To be the first point of contact with members, stakeholders, partners, other organisations and the public EFAD’s primary stakeholders are the member associations. EFAD currently has 27 Full Member Associations and 2 Affiliate Member Associations from 25 Council of Europe states (see Appendix 3). One of the roles of the Secretary General is to ensure that the member associations are actively engaged in planning and executing the work of EFAD. Over the past year we have directly mailed delegates with requests almost 50 times. In addition they receive weekly notifications of all new updates on the EFAD website. To effectively disseminate all this information within their own associations and to give the required feedback to EFAD requires both time and commitment. In addition delegates from 23 Associations (80% of members) plan to attend the 22nd General Meeting in Vienna. I am extremely grateful to the delegates for the wonderful job they do. Without them the work of EFAD could not have progressed as it has. The EFAD website is the main tool of communication. The public pages contain information about EFAD and dietetics that is of interest to the public. The extranet, which was launched in March 2011, contains information of relevance to dietitians throughout Europe and is open to anyone who wishes to register to use it. Already around 500 people have registered. Only delegates can access the EFAD intranet. All documents relating to the running of EFAD are stored there, along with confidential information from our partners. EFAD has also recently launched a Facebook page, where dietitians from around Europe can exchange information. This is still in its infancy but will hopefully prove to be the first of several new innovations to improve dissemination and communication. Work is also underway to engage a communication specialist and to commission a new EFAD logo. To represent EFAD as required by the Honorary President and the Executive Committee Since September 2010, I have represented EFAD in the DIETS2 Thematic Network as Network Manager and Quality Assurance lead. I have also represented EFAD at EU Platform, EPHA and ENHA meetings and at some conferences. Working with our Partners (see Appendix 4) enabled EFAD to identify areas in which we wished to advocate for changes in policy, either alone or in collaboration with our partners. EFAD responded directly to the Public Consultation on the Recognition of Professional Qualifications Directive 2005/36/EC of the European Parliament and of the Council. Through our partner organisations EFAD has also lobbied Members of the European Parliament regarding the resolution on Health Claims made on Foods (EPHA), on Nutritional Labelling (EPHA) and on Routine Nutritional Status and Risk Screening across Europe (ENHA). EFAD also coordinated a dietetic input into a report on Inter-Professional Collaboration in Primary Care (EFPC).

Professional Practice Committee Report The Professional Practice Committee (PPC) is a new committee for EFAD. The committee had two face-to-face meetings (February and September) and six virtual meetings this year. It was established to propose and implement a strategy for the development of professional practice for dietetics in Europe. The objectives for the PPC are:

advise on mechanisms for establishing development of professional practice of dietitians in Europe, using best practice of Member Associations

encourage developmental opportunities that are multidisciplinary, multi-professional or interdisciplinary

liaise as appropriate with other European professional groups and monitor the developments in professional development, accountability and European directives of other health professionals

standardise the practice of dietetics in Europe We have given priority to standardising the practice of dietetics in Europe. The Committee have proposed that the work begun by the American Dietetic Association on the Nutrition Care Process and the International Dietetic and Nutrition Terminology (IDNT) could provide a focus for the development of European work to standardise practice. In doing this, the Committee recognised the developments in process and language through the International Confederation of Dietetic Associations and ongoing work in Europe. This year the committee focussed on three areas:

to describe the current European initiatives to implement IDNT in European settings and prepare a discussion on the pros and cons of adopting one standardised terminology for dietitians Europe-wide

to define essential professional practice-related concepts, in order to be able to describe current strategies and practices of member associations aiming at development

to begin to explore current legislation related to professional qualifications in Europe and prepare a response on the Green paper of Directive 36

Jolein Lestra Plan for the Professional Practice Committee 2011:

Either to support a network of European IDNT initiatives or to prepare a Europe- wide strategy for implementation of the international terminology dietetics.

To develop an EFAD strategy on professional development, by conducting a survey on circumstances (national legislation and regulatory mechanisms and requirements), current activities and needs of Member Associations related to professional development of their members

The Education and Lifelong Learning Committee Report The work of EFAD´s Education and Lifelong Learning (LLL) Committee is undertaken jointly with the European-funded Thematic Network project ‘Dietitians ensuring education, teaching and professional Quality’ (DIETS2). The Committee consists of two work packages from DIETS2: WP2 (Second and third cycle specific dietetic competences) and WP3 (Lifelong Learning for Dietitians in Europe). WP2 started its work in the autumn 2010 and has 23 members from eight different partner organisations (Appendix 2). WP3 will start its work in January 2012. The objectives of the Committee are:

propose and implement a lifelong learning (LLL) strategy for European Dietitians that supports the strategies of Member Associations

advise on mechanisms for promoting LLL for dietitians within Europe, using best practice of Member Associations and Higher Education Institutions (HEIs)

encourage LLL opportunities that are multidisciplinary, multi-professional or interdisciplinary

liaise as appropriate with European (health) professional groups, HEIs and others to contribute to LLL in Europe

The business plan for the work of the Committee (within the scope of DIETS2) is given at the end of this report. The aim of WP2 is to perform a needs analysis and mapping of LLL patterns of dietitians throughout Europe in order to identify LLL requirements and associated competences. WP3 aims to develop a database of courses, modules and programmes available in Europe for dietitians (and other nutrition experts). Based on the results from WP2´s work, WP3 will also make recommendations regarding omissions and future development of courses within the HEI consortium. Over the past year, the Committee has had 11 meetings within the work of WP2. These meetings focussed on developing a questionnaire to map the views of European dietitians on competence requirements for work at an advanced level in dietetic practice. The questionnaire includes questions about what kind of further education and LLL the dietitians have undertaken. In addition, the NDAs were asked about whether the title “Specialist Dietitian” exists in their country and, if so, what requirements the dietitians need to meet to get this recognition and title. The questionnaire also collected information on the competences of dietitians in the area of research especially at Master’s and Doctoral levels. This information will be useful for the Research Committee and will allow a more focused support for dietitians in their work towards an evidence-based approach to practice. Preliminary results were presented at the EFAD General Meeting in Vienna in October. The main analysis of the results will be available in a written report on the website. A consultation phase for 2nd and 3rd level competences using a modified Delphi technique with professional associations, HEIs, students and service users (eg doctors/nurses) will start in spring 2012.

The Research Committee Report The Research Committee has had six meetings over the past year. The Committee has 16 members and they are listed at the end of this report. The objectives of the Committee are:

to develop a strategy for promoting, introducing and implementing an evidence-based approach to practice for European Dietetics that supports the strategies of Member Associations

to promote an evidenced-based approach for dietitians working and studying within Europe, using best practice of Member Associations and HEIs

to encourage opportunities to use, share and discuss best practice in multidisciplinary, multi-professional or interdisciplinary research as it affects the nutritional health of European people

to proactively liaise with European (health) professional groups, HEIs and others to contribute to dietetic research in Europe

The Committee and its work is undertaken jointly with the EU-funded Thematic Network project ‘Dietitians ensuring education, teaching and professional quality’ (DIETS2). This is the first year of the Committee and there has been a great deal of activity building the new committee and working very hard to establish an integration of the activities of EFAD and the DIETS Network. Following the successful 4th DIETS Conference in Amsterdam the American Dietetic Association (ADA) made available, for a trial period, their online searchable library of dietetic evidence-based materials. The Executive Committee has also been in discussion with the British Dietetic Association (BDA) to learn more about the possibility of access to the database ‘PEN’ hosted by Dietitians Canada. To this end a speaker from the BDA and Dietitians Canada will come and speak at the 5th DIETS Conference (Building strong evidence based approach to nutrition and dietetics) about using evidence to support practice and specifically about the database. The Research Committee members will also be running a workshop on dietetic research and how to ‘get started’. Through promoting the 5th Conference the Research Committee members hope to integrate the work of practicing dietitians with academic dietitians largely based in higher education teaching and research. There is still much to be done to promote and implement an evidence-based approach but it is hoped that through the integrating activities of EFAD and DIETS2, more can be achieved. Over the past year EFAD has established a list of ‘experts’ in special areas of dietetics. This list includes dietitians who are practising and also undertaking research that will lead to the promotion of more evidenced practice. The Committee will need to consider how it is possible to support the dissemination and exploitation of this expertise more widely. The DIETS2 Newsletter (published 2-3 times a year) promotes the work of dietitians across Europe and carries articles about dietetic research. Work is also underway, lead by the Committee, to establish an e-journal for dietetic students. This e-journal will host short reports of projects that have been completed by students across Europe. An editorial board has been recruited and will begin the task of calling for project reports and the production and distribution of the e-journal.

The Committee is aware that higher education has an important role in teaching students about research methods as well as encouraging research at postgraduate level (at Master’s and Doctoral level). To this end, the Committee has begun mapping the teaching of evidence-based practice to dietetic students in HEIs within the DIETS2 Network at all levels of study. In this way, more information may be available to all European dietitians about the preparation of students for pursing an evidence based approach and also what support from higher education is available to those dietitians that have already qualified. A database of materials that can support dietitians to learn more about undertaking valid and reliable research is also underway. This can be used to support those who want to improve their skills and knowledge. The preparation of a database of materials must be ‘user friendly’ and the Committee is building a framework. It will include valuable links to websites hosted by other health profession organisations. Doing this will promote more integration across Europe. The database will be hosted on the DIETS2 website and freely available to dietitians across Europe. The Committee has also been engaged in work with the Education and Lifelong Learning Committee to collect information on the competences of dietitians in the area of research, especially at Master’s and Doctoral levels. This information will allow a more focussed support for dietitians in their work towards an evidence-based approach to practice. In ending this report, I want to pay particular tribute to the Committee members working on behalf of EFAD and the DIETS2 project. The Chairman of this Committee has had a period of absence and it is quite remarkable that the Committee has made such good progress despite this absence. Appendix 9: Position Papers The Role of the Dietitian in the Management of Obesity Dietitians play a key role in the management of obesity in adults and children at every level. They are uniquely qualified to translate the scientific evidence on energy intake and expenditure, nutrition and behaviour into practical dietary advice and the provision of healthy food. Dietitians use counselling techniques to support individuals and groups who are overweight or obese or have obesity-related conditions, such as diabetes and coronary heart disease, in an effective way both short term and long term (Lichtenstein 2007, Zazpe 2008, Govers 2009, Delahanty 2010). Definition of obesity Obesity is a complex chronic condition that is characterised by the excess accumulation of body fat to an extent that health is adversely affected (World Health Organisation 2000). Lifelong commitment to treatment and management by both the individual and the health care system is required. Role of the dietitian Dietitians are in a key position to promote a healthy lifestyle. They are educated in nutrition, health, counselling and behavioural strategies and are actively involved in public health, health promotion/prevention, clinical treatment, disease management, research, food planning and preparation. Dietitians have the skills and competences to interpret and communicate theoretical knowledge to enable individuals to make appropriate choices about food and healthy lifestyle and to teach people how to sustain healthy behaviour. Dietitians fulfil their role in prevention, treatment, food provision and research in a range of different settings:

The Role of the Dietitian in the Prevention and Management of Malnutrition in Adults Malnutrition poses a considerable public health and economic burden across Europe www.european-nutrition.org. Dietitians, as experts and leaders in human nutrition, are uniquely qualified to develop and implement strategies to prevent, identify and manage malnutrition. They have a key role in the development of policy and guidance, inter-disciplinary working, evidence-based food and nutrition management and nutrition education of carers, health and social care professionals. Malnutrition:1

is a state in which a deficiency, excess or imbalance of energy and nutrients causes measurable adverse effects on tissue/body function and clinical outcome

is a disease in its own right as well as a risk factor in or consequence of other medical conditions

is often under-recognised and under-treated, but could be prevented/limited

expensive, costing Europe around €171 billion a year

affects recovery from illness and increases morbidity and mortality www.european-nutrition.org

Role of the Dietitian

In their professional practice, dietitians apply national and international guidelines and use clinical pathways to improve nutritional health.

With individual clients, dietitians take a person-centred approach to diagnose nutritional conditions and identify social problems, assess individual requirements, recommend and monitor appropriate care/ treatment plans and advise on/provide appropriate food to ensure optimal nutrition leading to improved quality of life.

Dietitians contribute to the prevention of malnutrition through the promotion of screening for those at risk, the provision of education, training, advice and support for carers, health and social care professionals.

Dietitians are involved in the development of policy and guidelines for the prevention and management of malnutrition at local, national and international levels.

Dietitians work across all care settings contributing to the nutritional care of people in hospital, residential care and their own homes.

Dietitians utilise all forms of nutritional therapy from normal food to artificial nutrition and facilitate the transition from one form of therapy to another. The provision of nutritional therapy in an effective and safe manner through the appropriate use of food and prescribing of nutritional supplements has demonstrated substantial cost savings.

EFAD supports the initiatives and recommendations of the Council of Europe, the European Parliament, WHO and the European Nutrition for Health Alliance (ENHA) to raise awareness of the risks of malnutrition and promote nutritional treatment as a multi-professional issue with a shared responsibility for action.

Welcome, Dietitians Improving Education and Training Standards (DIETS) was an EU-funded Thematic Network of Dietetic Associations, Departments of Nutrition and Dietetics in Higher Education and NGOs across 29 countries is Europe. DIETS had over 120 partner institutions, 70 of which were in Higher Education (HEI) and ran from 2006 to 2009. The Network ran three successful conferences and promoted the education and training of dietitians. In 2010 the EU invested further resources in the Network and this time the theme is ‘Ensuring Education, Teaching and Professional Quality’ (DIETS2). DIETS2 has increased its HEI representation and now spreads over 31 countries. The European Federation of the Associations of Dietitians (EFAD) is a prime partner promoting dietetics at European level and representing some 30,000 dietitians across Europe. The website www.efad.org and www.thematicnetworkdietetics.eu provide further details. I am delighted that this year EFAD wishes to join with the Network to be a co-host for the conference. Our 4th conference took its theme ‘Improving the nutrition of Europe through a fully evidenced-based profession of dietetics‘. The 5th conference takes as its theme ‘Building a strong evidenced-based approach to nutrition and dietetics”. The conference will explore not only foundations of education required to ensure a fully competent and educated dietetic workforce, but also areas of dietetics where professional dietitians provide specialist services, such as in diabetes, obesity, paediatrics, oncology, ageing and renal disease. We will be particularly aiming to support the use and production of an evidence base and workshops will support dietitians in that work. The proceedings and abstracts for the posters presented will be proactively promoted to all 30,000 dietitians in the Network as well as NGOs, HEIs and agencies in individual nation states by Dietetic Associations. The Network working with EFAD is already an established guarantor of the quality of education and the future research activity of dietitians through the publication of academic standards both for first cycle degrees and practice. DIETS2 will continue this work, especially focussing on post-qualification practice and their standards. The organising committee and I hope you really enjoy this important conference, in the beautiful city of Barcelona.
