A year for the history books!We are on the brink of significant changes in access to healthcare in this country and SouthEast Lancaster Health Services has a critical role to play. We have been asked to educate and enroll the uninsured in the

federal insurance Marketplace. We expect this will increase the demand for the high quality and affordable healthcare we deliver. Having spent the past year improving our financial stability and adding providers for increased capacity, we believe we are well positioned to serve the community in this way. Both board and staff members have taken to heart our strategic priorities –

improve patient access, equip staff for optimal performance, seek out new opportunities, and build community relationships.

The Board of Directors has increased its focus on strategic governance while holding staff accountable for building a strong and effective organizational infrastructure. We look forward to becoming the medical and dental home for increasing numbers of Lancastrians.

We help our patients live better lives.• During FY12-13 SouthEast experienced a dental home ratio of 3:1. This indicates that we provide

three visits for every one patient (on average) which demonstrates success in providing preventive dental care and adopting a patient-centered dental home model of care.

• 71% of our dental patients complete their initial treatment plan and enter into a preventive maintenance cycle.

• 68% of our adults with high-blood pressure were able to attain a healthy blood pressure rate.

• 92% of our babies were born at a healthy birth weight.

• 87% of our children have received all of the recommended immunizations.

In FY12-13, we served 19,074 patients with 76,626 medical and dental visits.
