1 | 2017 Annual Enrollment Quick Start Guide for U.S. Salaried and Non-Union Hourly Employees

HEALTH CARE BENEFITS — Provide the option to enroll in medical, dental and vision coverage.

MEDICAL: Coverage for a broad range of medical services, as well as prescription drugs.

DENTAL: Coverage for a broad range of dental services and procedures, including preventive care.

VISION: ENHANCED FOR 2017! Coverage for an annual eye exam and eyewear, such as glasses or contact lenses.

OTHER VOLUNTARY BENEFITS — Choices that include coverage for legal

Mondelēz International offers a variety of cost-effective benefits for you and your family that help boost your physical, personal and financial wellbeing. During Annual Enrollment, you can make elections for the following benefits:

WHAT’S NEW WHAT IT MEANS TO YOU

Medical Contributions

2017 medical contributions reflect increases in our claims experience. Health care costs continue to go up each year due to health care inflation and increased utilization.

You will pay between 7% - 10% more (depending on base salary) for coverage in 2017. View contributions on your Benefit Options Worksheet or through MyBenefits Online.

Vision Coverage

Enhancements for 2017 include frames covered every 12 months (instead of 24) with a higher $150 in-network allowance (increased from $130). As a result, your contributions will increase slightly.

You will now be able to get new frames more often with more money to use toward the cost.Plus, you have access to all sorts of discounts at eyemed.com and through their online partner, glasses.com.

WHAT’S NEW WHAT IT MEANS TO YOU

Legal Plan

New identity theft protection and services are now included. Look for more information from ARAG in the mail.

With the legal plan, you will have access to identify theft insurance, credit monitoring, lost wallet services and more.

FUEL4LIFE WELLNESS PROGRAM — Mondelēz will continue to reward you with wellness

incentives deposited into your HSA. In 2017, you can receive $500 as an employee enrolled in

Mondelēz medical coverage. Spouses/domestic partners can participate in wellness activities,

but will not earn incentives. Look for more information later in the year.

TAX-SAVING ACCOUNTS — Provide opportunities to save money by using tax-free

dollars to pay for eligible expenses.

WHAT’S NEW WHAT IT MEANS TO YOU

HSA Limits The IRS contribution limit for individual coverage is increasing from $3,350 to $3,400.

You can contribute more tax-free dollars in 2017 under individual coverage.

HSA Match Company matching contributions of up to $500 will continue in 2017 but only for those who make less than $100,000 (based on your salary as of 9/30/16).

While you will not receive a company match if your base annual salary is greater than $100,000, you continue to have an opportunity to save on taxes and grow your account for the future when you contribute to the HSA.

HSA Provider

Optum Bank will replace Wells Fargo on January 1, 2017. This is a result of Wells Fargo selling its HSA business to Optum Bank in 2016.

Optum Bank — an industry leader with 12 years of experience administering HSAs — offers access to a number of services and tools you can use to understand and make the most of your HSA.

You will receive an Optum Bank welcome kit with details (in November) and a new debit card (in December).

If you have a current account you will need to register as a new user on Optum Bank’s site and reassign your beneficiaries (they will not carry over). You can do this prior to January 1, 2017.

HEALTH SAVINGS ACCOUNT (HSA): A tax-free account you can use to pay for health care costs and save for your future (available if enrolled in the Mondelēz Global Medical Plan and you meet eligibility requirements).

HEALTH CARE FLEXIBLE SPENDINGACCOUNT (FSA): Allows you to reimburse yourself for eligible health care expenses with pre-tax dollars (available if you are not eligible for the HSA or are not enrolled in the Mondelēz Global Medical Plan).

DEPENDENT CARE FSA: Allows you to reimburse yourself and save money by using tax-free dollars to pay for eligible child or elder care expenses if you work or go to school full-time.

OR

INCOME PROTECTION COVERAGE — Options to purchase additional disability,

life insurance and accident insurance to supplement coverage you automatically

receive from the company.

Note: In 2017, Long-Term Disability (LTD) will transition to a fully insured plan that has slightly lower premiums.

REMINDERS

You pay less for medical coverage if you are tobacco free: If you make the healthy choice not to use tobacco, you will pay less for your medical coverage than those who do use tobacco. An additional monthly surcharge of $100 applies to those who use tobacco products — smoke cigarettes (including e-cigarettes), cigars, pipes or use chewing tobacco.

Spouse/domestic partner medical surcharge: A $125 monthly surcharge generally applies if you enroll your spouse/domestic partner in Mondelēz Global Medical Plan coverage when other employer-sponsored coverage is available and not elected.

Other benefits and programs: You have access to other benefits and programs that are not part of the Annual Enrollment process. This includes the 401(k) Savings Plan, the Fuel4Life Wellness Program and a number of other resources to help you boost your physical, financial and personal wellbeing. Learn more at mdlzbenefits.com.

If you do not enroll: Annual Enrollment is your chance to think carefully about your needs and consider your benefit elections. Take the time to review your options to make sure you're in the coverage that is best for you and your family. If you do not enroll, most but not all of the benefits you have today will automatically carry over into 2017:

Mondelēz International invests in benefits and programs for you and your family — but it’s ultimately up to you to use all the resources available to deliver on a personal commitment to live a healthy, productive life, while preparing for a financially secure future. The decisions you make and actions you take can make a real difference for your health and your wealth.

The free Tobacco Cessation Program offered through Fuel4Life can help. In addition to avoiding the tobacco surcharge in 2017, think about all the money you'll save on tobacco products and potential health care costs down the road.

DEPENDENT ELIGIBILITY AND VERIFICATION

Even if your medical, dental, and/or vision coverage continues automatically, you are still responsible for making sure that each individual you have enrolled continues to meet the plan definition of an eligible dependent.

New dependent verification process for 2017: You will need to verify the eligibility of any new dependents you enroll in coverage. (Note that new hires will need to verify all dependents they enroll in coverage.)

READY TO KICK THE HABIT?

A new discount program is coming in 2017. As a Mondelēz International employee, you can save money by taking advantage of special pricing and discounts on a variety of products and services. Look for more information coming soon!
