As a seasoned oral and maxillofacial surgeon, from time to time I reflectonwhereourspecialtywasyearsagoandhowfarwehavecome.Howdidwegettowherewearetoday?Bytakingtheissuesof thepastandinvestigatingnewsolutionswithstate-of-the-artclinicalresearch.Ourspecialty’sresearchersdothiseveryday.Theircontinualinvestigationof areasrelevanttopatientcarebuildoff of pastachieve-mentsanddelveintonewconceptstofindnewanswersandsolutions.Uponcompletedinvestigation,educatorstakethoseideasandteachthemtothenextgenerationof OMSs.Theresult:ground-breakingpatienttreatment.ThiscycleiswhytheOMSFoundationexists.Eachyearthe

OMSFoundationsupportsresearchawardsandeducationalfellowshipsinareasthatwilldirectlyaffectourpatients.

Spoken by James Hupp, DMD, MD, JD, editor of the Journal of Oral and Maxillofacial Surgery (JOMS), these strong words reflect Dr. Hupp’s view of the oral and maxillofacial surgery specialty. “Oral and maxillofacial surgery became the leader in things like orthognathic surgery, ambulatory anesthesia and advanced implant surgery by being innovators. This innovation came through ongoing research. Current surgeons and their patients benefit daily from the research conducted over the past decades.”

Daniel M. Laskin, DDS, MS, explained, “Research is the lifeblood of the specialty.” Dr. Laskin, who was editor of the JOMS for 30 years, added, “other specialties look to us for our contributions in fields like implantology, management of trauma, and anesthesiology.”

Both Dr. Laskin and Dr. Hupp agree that this is an exciting time to be a part of the OMS specialty. Many emerging areas of research are moving the specialty into new frontiers. Advancements in tissue engineering, robotics, minimally-invasive surgery, virtual imaging, and personalized genomics will change the way patients are cared for.

“I can foresee in the future we aren’t going to be taking bone grafts, we’re going to create new bone either in the

laboratory or by putting chemicals into the tissues and having the tissues do these things,” said Dr. Laskin.

The days of waiting decades to put research into practice are becoming a thing of the past. “With many of the advances occurring now, only a few years occur between discovery and clinical application,” said Dr. Hupp.

Dr. Laskin remembers the days when taking out an impacted tooth was done with slow-speed drills or even a mallet and chisel; now, high-speed drills are used. “When I first trained, all we could operate on to correct facial deformities was the lower jaw, and now we can move every bone in the face and head to make major changes.” In addition, cone beam CT has made major differences in how cases are diagnosed. Implantology, once the spark of an idea, is now almost the standard of care. These discoveries were established during Dr. Laskin’s career. Today, “most of what we do is clinical research and that type of research has a very quick translation from the clinic where the research is done to actual patient care,” he said.

Yet, with all of the exciting possibilities of innovation through research, funding for research is becoming harder to secure. According to the National Institute of Dental and Craniofacial Research, in 2011 only 23% of research grant applications were accepted for funding. The National Institutes of Health only funded 18% of research grant applications accepted in 2011. Institutional

support has also dropped off, along with the shift in OMS residency programs that have now become more service-oriented and less research-oriented.

“I believe it will become even more difficult to secure funding, and this is what makes OMS Foundation funding so critical,” said Dr. Hupp. The OMS Foundation’s mission is to support research and education in the OMS specialty. More than $11 million in support for targeted research and advanced education has been disbursed. The OMS Foundation relies on charitable gifts to maintain support for research and education.

“At a time when many are calling for cutbacks in government spending, funds donated to specialty groups such as the OMS Foundation are more important than

ever. They are critical to helping our specialty be able to conduct research and develop innovations to improve care for those who suffer facial deformities, incur traumatic injuries, have serious diseases like cancer of the head and neck, or find it difficult to be employed due to deforming dental problems,” said Dr. Hupp.

Clinical research is the only way to solve the unanswered questions in the specialty. “If we are going to live up to our pledge that we’re going to provide our patients with the best possible care and we are going to engage in evidence-based practice, there’s a need for ongoing research,” Dr. Laskin said. “With the difficulty in obtaining funds in our current economy, it’s important that we step up to the plate and help support these efforts.”

After completing his residency at the University of North Carolina Hospitals and a fellowship in Pediatric Craniofacial Surgery at the Posnick Center for Facial Plastic Surgery, he became Assistant Professor, Department of Surgical and Hospital Dentistry at the University of Louisville, School of Dentistry.

Then he made another, career-defining decision; he applied for a Faculty Educator Development Award (FEDA).

The FEDA, created by AAOMS and financially supported by both AAOMS and the OMS Foundation, was developed in 2002 to encourage promising young oral and maxillofacial surgeon faculty members to remain in a long term faculty career. It was also developed to provide a financial incentive to retain and recruit OMS faculty members to Commission on Dental Accreditation (CODA)-accredited residency training programs.

Applicants to the FEDA may apply during their first five years of being a full-time faculty member. Award recipients must commit to serve as a full-time faculty member for a minimum period of six years.

During the first three years of the FEDA commitment, the award provides a total of $130,000. Recipient faculty members receive $40,000 a year for three years, and the recipient’s institution receives a one-time disbursement of

$10,000 for faculty enrichment to be used solely to support the FEDA recipient.

In 2004, Dr. Tiwana was a recipient of the FEDA. “The FEDA has become a prestigious national award given to faculty members who are rising in the specialty,” said Dr. Tiwana, who is now program director of the Oral and Maxillofacial Surgery residency program at The University of Texas Southwestern Medical Center in the Division of Oral and Maxillofacial Surgery. Prestige was one of the reasons Dr. Tiwana applied for the FEDA, but it wasn’t the defining factor. Financial assistance with his student loans and the opportunity for faculty development motivated him to apply.

Dr. Tiwana emphasized that the assistance with student loan indebtedness allowed him the flexibility to focus on a career in academics rather than generating income to pay back his student debt. He was able to devote more time to developing his aspiring academic career, by attending scholarly meetings and focusing more on initiating research and scientific projects.

Today, student loan indebtedness and a shortage of OMS faculty members are challenges facing the OMS specialty. According to a 2011 survey of dental student seniors by the American Dental Education Association, the average student loan debt for private dental schools is $245,297, and for public dental schools it is $177,795. Upon completion of residency, the average OMS indebtedness is between $250,000 and $350,000. Their debt comes

from undergraduate studies, dental school, and for nearly fifty percent of OMS residents, medical school costs. In addition, the length of OMS residency means that some residents are at an age where they are starting families, adding to their personal financial responsibilities.

“There are residents in my training program whose student loan indebtedness is rising to an astronomical level. We’re talking levels above half a million dollars… six hundred thousand dollars… by the time they get finished,” said Dr. Tiwana. “I think, as time goes on, this will continue to be a big challenge.”

For FEDA recipients, “This kind of award is absolutely crucial to making sure they can begin not only as a faculty member, but assist them in the educational process,” said Dr. Tiwana.

FEDA recipients get their “feet on the ground” academically so that by the end of the six-year commitment, they have already experienced success as a faculty member, and will likely move to the next rank as an associate professor and move forward with their careers.

Dr. Tiwana, who is Chair of the AAOMS FEDA Committee, says the FEDA is vital to attracting and maintaining young people and young surgeons in academic oral and maxillofacial surgery.

“More full-time faculty is only more beneficial to better trained residents, better trained residents equals better trained practitioners, and better practitioners only

reinforce the specialty all the way around,” said Dr. Tiwana.

Another respected academic, James Hupp, DMD, MD, JD, editor of the Journal of Oral and Maxillofacial Surgery agrees with Dr. Tiwana’s assessment. He said, “Student indebtedness impacts the increase in open OMS faculty positions. In too many cases residents and fellows who would prefer an academic career find it difficult to justify turning down lucrative private practice offers due to their educational indebtedness.”

Dr. Hupp stressed the value of an academic career, and doesn’t accept the idea that there is an automatic disparity in academic versus private practice salaries, but it is common. Aside from salaries, he notes that academicians have opportunities to manage a large variety of clinical cases. They also have a specialty-wide impact and receive the fulfillment that comes from preparing the coming generations of OMSs. In addition, doing research that advances our society brings a feeling of accomplishment money can’t buy.

The FEDA is vital to attracting and maintaining young people and young surgeons in academic oral and maxillofacial surgery.

Ambassador Service Award The Ambassador Service Award is given to a leader amongst OMS Foundation Ambassadors who has shown outstanding service. Annual Fund (REAP) Donors

Thank you to those donors who contributed a total of $735,000 to the 2011- 12 Research and Education Advance Patient care (REAP) Annual Fund.Gifts to the Annual Fund maintain funding provided to qualified applicants for Research Support Grants, Clinical Surgery Fellowships, Student Research Training Awards, and Faculty Educator Development Awards (FEDAs). The 2011-12 Annual Fund raised $70,000 more and acquired an additional 100 donors compared to the 2010–11 Annual Fund.

The Alliance is dedicated to supporting and promoting the educational and research activities of the Oral and Maxillofacial Surgery Foundation. The Alliance’s numerous activities at the American Association of Oral and Maxillofacial Surgeons (AAOMS) Annual Meeting each year provide a variety of networking opportunities for spouses, staff, and friends of oral and maxillofa-cial surgeons to get involved in the specialty. Annual Meeting activities sponsored by the Alliance include daily health walks, breakfasts, and the much-anticipated Alliance Annual Event luncheon, which raises support for the specialty.

Alliance ContributionsSince its inception in 1993, the Alliance has contributed over $118,000 to the specialty: $72,500 toward research awards/fellowships $32,000 purchase of computer software for the OMS Foundation $8,500 for Resident Spouse Scholarships $5,000 unrestricted gifts in celebration of the OMS Foundation’s 50th Anniversary

We owe a deep debt of gratitude to those who came before us and made it possible for us to enjoythebenefitsof excellentsurgicaltrainingandcutting-edgetechnologythathasadvancedthecareof ourpatients.Withouttheirinvestmentineducationandresearchtoadvanceoralandmaxillo-facial surgery, we would not enjoy as much professional esteem and the ability to compete for patients as we do today.

Progressdoesnotwaitfortheidle,nordoaspirationsforimprovedqualityof life.Progressiscertain.Itisascertainasthewaywetreatourpatientstodayversushowtheyweretreatedyesterday.Itisjustascertainthatcarewilladvanceinthefuture.Progress,however,takesinvestment.If weasaspecialtydonotinvestinourownfuture,whowill?Willitbetheperiodontists,orthodontists,endodontists,orothers…willwestillberelevant?

TheOMSFoundationistheonlyorganizationthatinvestssolelyinoralandmaxillofacialsurgeryedu-cation and research. Our programs are in place to ensure that the best research is funded and support is availabletoassistintrainingandsupportof residents,facultyandresearchers.

Our Research Support Grants are “seed support” for OMS research. They prepare our most promising researchtobecomecompetitiveforlargerNIHgrants. Thesupportof facultydevelopment,residentsandstudentshelptoensurethatourtrainingresourcesarestrongandthepathwaytoourspecialty’ssuccessfulfutureisnotblockedduetofacultyshortagesorcareerchoices limited by student indebtedness.

AninvestmentintheOMSFoundationhelpstokeepourspecialtystrong,vitalandfocusedonthefuture.Itpreparesthenextgenerationforsuccessandhelpsustomakeadvanceswhichallowustocontinuetoofferthemosteffectivecare.

Thereareavarietyof waysyoucanhelp.AnAnnualMember-ship is $165; your support assists the OMS Foundation with its administrativeexpenses.AcontributiongivenfortheResearchandEducationAdvancePatientcare(REAP)AnnualFundsupports the research and education grants funded by the OMS Foundation. An annual gift of $1,500 or more is recognized throughREAPSocietymembership.ThosewhoprovidefortheOMSFoundationintheirestateplan(will,trust,etc.)arerecognizedwithmembershipintheRobertV.WalkerSociety.
