On the ANA website, www.myana.org, click “Login/Join”. 2. Under “Not a member?” click “Join Now”. 3. Fill in required information and click “Submit”. 4. On the “Membership Information” screen, click the drop down menu to select your “ANA Membership Type”. Then click the drop down menu for Annals of Neurology Subscription to select how you would like to receive the journal. Note: Associate and Student Members are only eligible to receive the journal electronically. Once you have selected your items, click NEXT. 5. On the “Additional Information” page, select or input information for each field. Note that sections marked with a red asterisk are required fields. Click “Next” to continue. 6. Proof of eligibility is required for all membership types. Upload your documentation by clicking on “Choose File”. 7. A pop-up window will appear that will allow you to select the document on your computer. To upload the document, simply double-click on the file. 8. After the file has uploaded, the title of your document will appear under “Proof of Eligibility”. Click NEXT to proceed to the “Attestation Statement” page. 9. Click in the box under “Attestation Statement” to confirm that the information provided is current and accurate. Then click NEXT. 10. The “Review” page shows the status of your application. Green check marks under “Status” indicate that you have successfully included all required information. When all four (4) sections are complete, click “Submit” on the left-side menu. Note that the “Submit” button will not be available if required information is missing. 11. The Summary of Order page allows you to make changes to your purchase. Once you have reviewed that the information is correct, click “Checkout”. 12. Payment can be made via credit card or paying by check. A. If you will be paying by credit card, click the dropdown arrow next to “Choose Payment Type” and enter your payment information. When done, click “Submit Payment”. B. If you will be paying by check, click “Pay By Check”.

ANA Join Process (See Page 2 for Instructions with Graphic Illustrations)

Before you begin please be sure to have:

• An electronic copy of your full CV (for fellow or member level) OR

• An electronic copy of a transcript or proof of enrollment letter (student level or associate level) OR

• An electronic copy of an endorsement letter from supervisor (associate level)

• Method of payment (for fellow, member and associate level)

1. On the ANA website, www.myana.org, click “Login/Join”.

2. Under “Not a member?” click “Join Now”.

3. Fill in required information and click “Submit”.

4. On the “Membership Information” screen, click the drop down menu to select your “ANA Membership Type”. Then click

the drop down menu for Annals of Neurology Subscription to select how you would like to receive the journal. Note:

Associate and Student Members are only eligible to receive the journal electronically. Once you have selected your items,

click NEXT.

5. On the “Additional Information” page, select or input information for each field. Note that sections marked with a red

asterisk are required fields. Click “Next” to continue.

6. Proof of eligibility is required for all membership types. Upload your documentation by clicking on “Choose File”.

13. A receipt is generated once the payment is accepted. A confirmation email will also be sent to the email address provided

on your member record.
