WELCOME TO THE PRACTICE PRACTICE STAFF

The General Practitioners The Partners:

Dr Andrew Power(m) MBChB Qualified in Leeds in 1985

Dr. Power was born in London. After completing training he joined a practice in Leeds. He spent a brief period working in private medicine before coming to Almshouse as a salaried doctor and then a partner, becoming senior partner when Dr Barker retired. Dr. Power enjoys all aspects of general practice but has a special interest in dermatology. He is also the practice lead for the Patient Group.

Dr Dena Smith(f) MBChB DFFP MRCGP DRCOG Qualified in Manchester in 1990 Dr. Smith was born near Liverpool and studied in Manchester. She joined Almshouse practice in 2001 and became a partner in 2005. She has special interests in family planning including coils, gynaecology, child health and also attends regular palliative care meetings with our district and Macmillan nurses.

Dr Omar Alisha(m) MBChB MRCGP Qualified in Mosul in 1997 Dr. Alisha trained in the UK from 2002, obtained MRCGP in 2006 and qualified as a GP. He joined the Almshouse Surgery as a registrar in 2005, became a salaried GP in 2006, a partner in 2008 and a trainer in 2010. Dr. Alisha’s special interests are; Training GP registrars, Practice IT, Diabetes, Asthma/COPD and Substance Misuse Service.

Dr Jigar Daru(m) MRCGP, MBBS, DRCOG, DFFP Qualified in India 2002 Dr. Daru came to UK in 2002. After completing his training in 2007 he joined the Almshouse Surgery, he is currently a partner in the practice. Dr. Daru enjoys all aspects of general practice but has a special interest in mental health. He also runs regular cryotherapy (freezing) clinics for simple skin lesions e.g. warts/verrucae.

Dr Abdul Mustafa(m) MBChB MRCS MRCGP Qualified in Arbil 1990 Dr. Mustafa qualified in Arbil in 1990. He has a special interest in Minor Surgery and training students within the practice. He is responsible for the FY2 and our Medical Students from Leeds University.

Dr Penelope Navarro(f) MBChB, DFSRH Qualified in Spain in 2000 Dr. Navarro was born in Madrid and was recruited by the British Council in 2006 to work in the UK. She practiced in Leeds until she started working at Almshouse Surgery in 2010 and became a partner in 2012. She has special interest in women's health, diabetes and palliative care. Dr. Navarro is on maternity leave until September 2014.

Salaried GPs: Dr Sarah Renga(f) MBChB Qualified in Leicester 2006

Dr. Renga has a special interest in assisting in training the medical students and the trainee GP’s.

Dr Lisha Jose(f) MBChB, MRCGP, DRCOG, DFFP Qualified in Uganda and the UK Dr. Jose was born in Kenya and came to the UK to do her General Practice training. After training she worked at Stuart road surgery for 3 years and when opportunity arose, she started work at Almshouse surgery as a Salaried GP. Dr. Jose works part time and has special interests in gynaecology.

The Practice Nurses Morag Pick, Adillah Thomas, Sandra Musgreave and Susan Bell (CPN) are your practice nurses. They are all qualified and registered general nurses and between them also have extra training and qualifications in family planning, coronary heart disease, asthma and diabetes, and nursing in general practice. Our practice nurses are able to refer you to other colleagues in the practice team, such as the podiatrist (chiropodist), health visitors, district nurses, community psychiatric nurses, social services and the dietician.

Health Care Assistants (HCA) We have two HCA’s - Donna Moore and Leigh Fisher. They deal with Home Visits for our elderly patients who cannot attend the surgery to check on their Medical Problems. They hold phlebotomy clinics and surgeries for New Patient Health Checks, Spirometry, ECG, Blood Pressure etc.

Triage and Minor Illness Nurses Requests for urgent appointments and home visits are handled by our team of Minor Illness Nurses, led by Team Leader Chris Blyth. Our Triage Nurses run their own Minor Illness Clinics and some are able to prescribe for a wide range of acute ailments.

Administration and Management Carol Beedham is the Practice Manager; she is responsible for the management of the practice. If you have any concerns about the services offered by the practice that cannot be resolved at reception please contact her in confidence, either by phone or in writing at the Almshouse surgery address. Jayne Richardson is the Assistant Practice Manager and is responsible for private insurance and medical reports and can help with queries about reports and copy records for insurance companies and solicitors. Samantha Crone is the Reception Manager; she is responsible for the service provided at both surgeries.

Secretaries Michelle Stringer is our Medical Secretary and can help with queries about referral letters and hospital appointments.

Reception We have a team of receptionists who work across both surgeries. They will be more than happy to help with any queries about our services.

Medical Students & Trainee’s We are a registered training practice and regularly have Medical Students from Leeds University sitting in sessions with our GP’s. Patients should always let a Doctor or Nurse know if they are not comfortable having a student present. Almshouse Surgery is a training practice and has Registrars who are trained Doctors working with us for six month periods to complete specialist training in General Practice.

Drug and Alcohol Workers We have two specialist workers who have weekly clinics at Almshouse Surgery, Trinity Medical Centre, to help people who want to deal with drug and alcohol problems. You need to be referred into these services by your doctor.

Midwife The practice-based midwives work with our doctors and hospital consultants to ensure that pregnant women receive care throughout their pregnancy and after baby is born. Clinics are held at both Almshouse Surgery and at Sandal Castle Medical Centre.

District Nurses A team of district nurses are attached to the Practice. Mainly working with housebound patients in their own homes, our district nurses also hold clinic sessions at Almshouse Surgery and Sandal Castle Medical Centre for patients who are mobile and only requiring simple treatments. Clinic session appointments can be made through surgery reception. Alternatively, the District Nurses can be contacted on telephone number 01924 327220 between 9.00 a.m. and 4.30 p.m.

Health Trainers We have a Health Trainer. The clinics are held at Almshouse Surgery & Sandal Castle Medical Centre. Referrals are made by our doctors and nurses and appointments can be made through reception. A health trainer deals with many things for example stress, dieting, alcohol, smoking and exercise on prescription.

Health Checks If you are aged between 16 and 75 years and you have not been to the surgery for three years or more, you can make an appointment to see one of our nurses for a health check. If you are over 75 years old and you have not been to the surgery for 12 months or more, you can make an appointment to see one of our nurses for a health check. We would encourage our new patients to complete the New Patient Health Questionnaire and to make an appointment with a nurse for a new patient health check.

Trinity Care Trinity Care is a new service offered by a team of nurses in addition to our own Triage service. The service is run from Trinity Medical Centre and is available for patients registered at the following surgeries;

Warrengate Medical Centre

King Street Health Centre

Maybush Medical Centre Trinity Care offers same day appointments for patients registered at any of the above surgeries who feel they need medical advice on the same day. You will speak to a fully trained nurse who will either offer medical advice over the phone or will arrange for you to see a GP at either your own surgery or at Trinity Medical Centre depending on the time. The Trinity Care team also has access to The Community Support Team who can assist with a patient who needs help in the community through illness, accident or a Carer not visiting. The Community Support Team are able to provide short-term support and assist in putting together the help needed, or advise on health services which could be of benefit to the patient.

APPOINTMENT INFORMATION

Appointments Routine Appointments may be made by telephoning the surgeries after 8.00a.m. The telephone numbers are:

We aim to offer routine appointments to see any doctor within 48 hours. An appointment to see a doctor of your choice may take longer than this. You are able to pre-book appointments, normally up to two weeks in advance. You may contact our doctors and nurses by telephone if you feel your query can be dealt with without an appointment. We will take your message and telephone number and the doctor/nurse will telephone you back. Please cancel an appointment if you are unable to attend at the appointed time. If you persistently fail to arrive for an appointment we may ask you to leave our surgery and register elsewhere.

Same Day Appointments Same day appointments can be made by telephoning 01924 784100 for both surgeries. Your request for a same day appointment will be assessed by a Nurse and, where it is needed, you will be seen by a Duty Doctor on the same day. The duty doctor for same day appointments is based at Almshouse Surgery.

Home visits Home Visits can be made by telephoning 01924 784103 if you are too ill to attend surgery. Where possible please telephone us as soon after 8.00 a.m. as you can. Please provide all the information you are asked for as this will help our doctors to arrange visits in accordance with clinical priority. Home Visits are reserved for patients who are genuinely too ill to attend surgery or are housebound. Whenever possible you should attend the surgery where we have the necessary range of equipment and access to patient records to ensure that a consultation is safe and appropriate. We would remind you that you are responsible for your own transport arrangements.

Night, Weekend and Bank Holiday Cover If you urgently need a doctor out of normal surgery hours please dial 111. Calls to the NHS 111 service are free from both land lines and mobiles. When you call 111 you will be assessed, given advice and directed to the service that can help you best. If you have a life threatening medical emergency please ring 999.

Saturday SurgeriesAlmshouse Surgery is open from 9am on Saturdays for ROUTINE appointments. These are available for booking throughout the week via our website or by calling the appointments line on: 01924 784 103. These appointments are designed for our patients who struggle to get to a weekday surgery. There are limited appointments available. The surgery is only open for appointments that are pre-booked. There are no other services available on Saturday.

Cancellations, Non-Attendance & Time-Keeping We ask that all patients let us know as early as possible if they cannot keep an appointment. If you do not notify the practice within a reasonable time frame and the appointment cannot be offered to another patient then this will be marked as a missed appointment. 4 consecutive missed appointments will result in removal from the practice list. If you are running late for an appointment please call us as soon as possible so that we can notify the doctor. If you have missed your appointment time by 10 minutes this will be recorded as a non-attendance.

PRACTICE INFORMATION

All our doctors work at both Almshouse Surgery and Sandal Castle Medical Centre. Patients can always request to see a specific Doctor should they choose however we are a Group Practice and so all records are shared by all doctors. We are not a Limited partnership.

The Almshouse practice has always been a town centre practice and effectively came into being in 1968 when two practices, that had previously operated out of houses, joined together to move into their first purpose-built surgery on Almshouse Lane. The doctors involved at this stage were Drs Gibson, Lyle and Shaw. A further move into new premises, still on Almshouse Lane, took place in 1983 as a result of the development of The Ridings Shopping Centre. In 1992 a branch surgery was opened in Sandal and finally in June 1998 the practice relocated its Almshouse Surgery into Trinity Medical Centre.

Opening Times Almshouse Surgery 8.00 a.m. - 6.30 p.m. Monday to Friday Sandal Castle Medical Centre 8.00 a.m. - 6.00 p.m. Monday to Friday

Registering as a new patient New patients are requested to attend in person, complete a registration form and new patient questionnaire and provide photo identification e.g. passport and something to confirm your current address e.g. utility bill.

Repeat Prescriptions Repeat prescriptions are issued only with the doctors’ consent. Repeat prescriptions can be ordered;

- Online via our website www.almshouseandsandal.co.uk - In person at either surgery. - By sending a stamped address envelope to the surgery with your request. - Via telephone by calling either surgery. This will allow you to leave a message with your request.

Please allow 48 hours for your prescription to be processed. Prescriptions not collected within 4 weeks will be destroyed and need to be re-ordered. Prescriptions may be collected from our local chemist by prior agreement with us.

SystmOnline The Practice offers patients an online system which enables the booking of appointments, ordering repeat prescriptions, viewing of your summary medical record, completing questionnaires and changing contact details. To gain access to your personal online system please visit reception with photo identification and we will give you the login details you require. This service can be accessed through our practice website www.almshouseandsandal.co.uk

Test Results We encourage all patients to ring the Triage team who can give you results and explain them if necessary. They can be contacted on 01924 784100. Please ring for your results after 2pm. We would ask everyone who has tests arranged by the doctor or nurse to phone us for the results. If a doctor feels they need to talk to you about the results the surgery will contact you to make a telephone consultation or ask you to visit the surgery.

Privacy at desk If you wish to talk to a Receptionist in private, please ask to have the discussion in a private room.

Data Protection We need to provide you with the best possible care, to do this we need to keep accurate and up-to-date records. We follow strict rules to ensure your personal information is protected at all times. Only authorised members of staff are allowed to see personal information held in your records.

Complaints We try to give the best possible service, however if you have a complaint or suggestion, please let us know. We hope that most problems can be sorted out easily and quickly, preferably at the time they arise, and with the person concerned. If your problem cannot be resolved in this way, and you wish to make a complaint, we would like you to let us know as soon as possible. We have produced a separate, more detailed leaflet, which tells you about our complaints procedure, and operate a practice based complaint procedure as part of the NHS system for dealing with patient complaints.

We have suggestion box at each reception, but request you do not put complaints in here.

Family & Friends Test As part of the NHS Family & Friends scheme, when you have visited the practice, you will be asked to complete a form that asks you if you would recommend us to your family and friends and gives you the option to explain your answer. The forms can be left in a box at each reception and will remain anonymous.

Access to records You have the right to see your health records, subject to limitations in the law. Your records will be kept confidential at all times and only people involved in your care will have access to their contents, including specialists in hospitals etc. Your signed consent is required to allow anyone else to have access to information in your health records, e.g. your employer, or an insurance company. Application for access to records must be made in writing.

Translators If English is not a patient’s first language, we will do our best to organize an interpreter. (Telephone Interpreting Service) Adequate notice is required if this service is required. We have a webcam based on reception at Almshouse Surgery should the need for a sign language interpreter arise.

Chaperones You will be offered a chaperone for all intimate examinations, but please ask for a chaperone if you feel uncomfortable with any examination that the doctor or nurse deem necessary.

NHS Zero Tolerance Policy We ask that you treat the doctors and practice staff with courtesy and respect. We will not tolerate violence, abuse, or threatening or aggressive behaviour towards any of the staff. Such behaviour will result in immediate removal from the practice list.

Disability Access The Practice provides clearly marked and wider disabled parking bays at both surgeries. We have dipped curbs and automatic doors. We allow guide or other assistance dogs into the premises. We have a hearing loop available on reception and a portable one for use in the consultation rooms. We have a disabled access toilet at both surgeries and lift access to first floor rooms where required. Disabled patients can make appointments by on line booking, or letter. We also offer a large font Practice leaflet on request.
