This landmark agreement gives us the ability to move this to the next stage and formally integrate our teams and develop more joined-up working to benefit local people and make the most of our combined skills and resources.” Councillor Steve Stubbings, IW Council Executive Member for Adult Social Care and Integration (pictured), said: “This is the first step in our journey towards the one Island pound model; a completely integrated provision of health and social care on the Isle of Wight. I am delighted to see that it was unanimously supported by both the council’s Executive and NHS Trust Board.”

Working better together to improve people's lives on the Island

All signed up to integration A landmark partnership agreement has been signed by both the IW Council and IW NHS Trust for the integration of health and wellbeing services on the Island and will come into effect from Monday."

My Life a Full Life On the Island, a process of significant change is taking place to help us adapt to the challenges we collectively face. With more demands on our health and care services we need to support our community, staff and volunteers to ensure we have sustainable services in the future for everyone."

We need to start by supporting people to lead healthier lives, right from an early age to prevent ill health in the future. We need to give people the right information and advice, focused around their individual needs, so they can support and care for themselves and help those with long-term conditions to plan and manage their future care."

We need to work differently together to make the most of the skills and resources we have and bring together support locally to help people find local solutions to retain their independence and live life to the full. "

In short, we need to develop a new model of care for the Island together that meets our needs as a local community. Under the MLAFL programme, The Isle of Wight NHS Trust, Isle of Wight Council, Isle of Wight Clinical Commissioning Group, One Wight Health (Island GP Practices), and the voluntary, independent health, care and housing sectors are working in partnership."

In these bulletins we'll be sharing our progress and highlighting the difference being made to peoples' lives."

Making headline news!

For the second time in a month, the Island's Integrated Care Hub has featured in a positive national newspaper article."

Last month, the hub was praised in an in-depth report by Guardian journalist Caroline Davies and this week the same facility has been positively featured in an article by Clare O’Reilly in The Sun's 'me' health supplement. Clare wrote that the hub was a ‘state of the art facility that could change healthcare.’"

During her visit, Clare spoke to many staff including an NHS 111 & 999 Call Handler; a paramedic who also Audits NHS 111; Diane Goring, Crisis Response; Lee Haward, Acting Performance Hub Manager; Ian Lloyd IW Council transformation manger, Sharon Betts IW Council Strategic Manager - Business Centre and Chris Smith, NHS Clinical Director."

Alison Toney, IW NHS Communications and Engagement Officer, who guided Clare during her visit said: “She picked up very quickly how everyone wants to work together for the good of the patient. She could instantly see how it works by the set up in the room and the importance of services being located together and talking face to face with each other.”"

Introducing James Seward

We're pleased to announce that a new programme director has been appointed to help lead the health and care system redesign as part of the My Life a Full Life programme. James Seward is an expert health and care strategist and programme director with over 25 years experience. He has successfully delivered transformational national

mental health and wellbeing programmes, including the national mental health policy role, and successfully led the development of Public Health England’s National Mental Health, Dementia and Neurology Intelligence Network and the Government’s flagship Improving Access to Psychological Therapies (IAPT) programme. More recently he has used his skills as a social innovator to bring a new dimension to a digital start up."

Speaking of his new role, James said: “The My Life a Full Life programme is now entering the crucial stage of looking at the ways health and care services need to be redesigned around people in their communities to provide the best care possible that is both affordable and sustainable in the long-term. I am excited to have the opportunity to lead this work and am committed to ensuring that the additional resources that have been secured are used to make certain that the people of the Isle of Wight are fully involved in helping to shape these changes to get the best possible outcomes for the Island.”"

James has already started in his post on the Island - making the commute each week from Tunbridge Wells - and is looking forward to moving with his family – his wife and two young children – to the Island to start a new life here together. Amy Milford joins the team

A key part of the My Life a Full Life programme and in particular the health and care system redesign programme is focused on gathering the views, thoughts and ideas of people working in the voluntary sector on how health and care services should be shaped for the future. To support that aim, Community Action Isle of Wight has appointed Amy Milford as their Communication and Engagement Officer supporting this programme."

Amy previously worked as a probation officer on the Island and in Lewisham and has also been involved in various community and voluntary projects. She is currently a volunteer on the steering group of Parents’ Voice IW and is an independent appointed member on the Hampshire Police and Crime Panel. "

Amy said: “My role as the Communication and Engagement Officer is to enable local voluntary organisations to have their say on the whole integrated systems redesign project, whereby health and social care services will be working much more closely together. I am looking forward to meeting with lots of different groups to discuss the proposals for the redesign and to help record their views as part of this process." "

Amy will be at the Age Friendly Island public forums to help explain how people can get involved."

Shaping the future of health and care services - the next steps

Progress to develop the health and care system redesign , previously referred to as WISR, is gathering pace with a new Lay chair and Programme Director appointed and a team, led by consultants KPMG, in place to help support the roll out of this crucial phase of the My Life a Full Life programme. "

At its very heart, is a very genuine desire to listen, to involve and engage staff, professionals, volunteers and the general public (both those who use services now and those who may need them in the future) in shaping the future of health and care services on the Island."

The programme will essentially have six key phases over the course of this year:"

Phase 1: (now) Looking closely to define the breadth and scope of the current systems on the Island across all sectors, raising awareness of the programme and its aspirations and preparing materials to support the next stages of the process."

Phase 2: (now) Building the ‘Case for Change’ – bringing together all of the issues, challenges and opportunities into a format that everyone can read, understand and recognise."

Phase 3: (March-June) Gathering people’s views, in equal measure, from across the community about the current systems and how these might need to change in the future through a series of events, co-production workshops and other mechanisms."

Phase 4: (July- September) Main consultation phase focused on options for the future, based on the initial pre-consultation feedback, co-production workshops and modelling. "

Phase 5: (October) Audit and analysis of the feedback received."

Phase 6. (November) Producing a ‘roadmap’ that sets out the actions that will re-shape the Island’s health and care services."

We will be sharing more about this process in regular communications in the coming weeks and months to make sure that everyone is kept informed and has the opportunity to contribute to the Island’s future care model."

Health & Wellbeing roadshow rolls into Ryde

Free roadshows, offering residents the chance to discover more about the health and wellbeing support available in their community, continue to go from strength to strength."

The last event at the Aspire Centre, Ryde, at the end of January, was attended by around 20 organisations and attracted more than 100 people."

It was the latest in a series of events from My Life a Full Life designed to give residents the chance to enjoy a cup of tea as they meet some of the local groups, organisations and individuals in their area who can help them lead full, healthy and independent lives."

Event organiser Paul Savill from Community Action IW said the session was the busiest yet and already plans are

afoot for future events."

“It was a really successful event at the Aspire Centre. It shows that people really do like the idea of tapping into services that are there on their doorstep,” Paul said."

“We have organisations that have attended several of the events, which is good as it gives the events some continuity, but we are also getting more and more requests from new groups and organisations which is really encouraging.”"

Paul added that people of all ages were getting benefits from the roadshows."

He said: “Older people are leaving the roadshows with valuable information to make their life more comfortable or how to prevent themselves from going into hospital or needing care at home. Families have also got information on activities near to them and at the Aspire event, four young people took the opportunity to join local Scout groups.”"

The aim is to stage the ongoing events in rotation around each of the three Island localities. The next roadshow events are on:"

Thursday 7th April at The Heights Sandown. Opening time will be 1100-1600. All tables booked will be free of charge. Only 13 tables available though so book now! "

Wednesday 11th May, The Event Centre, Cowes Yacht Haven. 1000-1600. Lots of space available, commercial enterprises will be charged £20 per table."

Tuesday 5th July, Riverside Centre Newport, 1000-1600. Lots of space available, commercial enterprises will be charged £10 per table. "

Prepare to be inspired at Aspire!

Just two years after a dwindling congregation forced Ryde’s Holy Trinity Church into closure, the building is once again at the heart of the community thanks to an innovative new venture."

Thanks largely to the work and support of a dedicated band of volunteers, the 180-year-old building has been renovated to become a hub for a myriad of community uses. Each week, around 1,000 come through the doors to benefit from projects and activities."

Any profits made from the venture are put straight back into services to support the local community and into maintaining the historic Holy Trinity building."

The ethos of Aspire is very much in keeping with My Life a Full Life aim of providing the services that people require in the communities in which they live."

Heath Monaghan who leads the project said: “Absolutely everyone is encouraged to come down to Aspire and get involved. We are very much driven by what the community would like from us and projects are evolved around people and their needs, so if you have an idea, come and talk to us! "

“Our volunteers are friendly, supportive and enthusiastic about the work they do. Aspire hosts lots of groups so there's sure to be a like-minded group of people around for you to socialise with.”"

Among projects hosted by Aspire are Re-paint – where unwanted paint donated by suppliers is redistributed to deserving recipients rather than sent to landfill. Other services include a pop-up restaurant, office facilities for rent, a community hall and rehearsal space. Activities include Kung-Fu, Learn to Dance, Dancing Toddlers, Chairobics and Men In Sheds, a meeting event for DIY and project enthusiasts and Young at Heart where anyone 70-plus is invited to enjoy drinks and lunch once a week."

At the heart of Aspire is the Pure Ground community cafe run for the whole community by a fantastic team of caring volunteers who create a unique, fantastic welcome and atmosphere."

Summing up the community ethos of the centre itself, Pure Ground has recently revived an altruistic custom that began in 1900s in Italy called caffè sospeso – ‘suspended’ or ‘pending’ coffee. This was a tradition whereby anyone who had had some good fortune entered a café and ordered a coffee, wouldn’t just pay for one, but for two cups, allowing someone else less fortunate to have a free coffee later."

Pure Ground thought the custom was a good and decided to introduce it to their venture. So now, customers can choose to pay for a drink, cake or even bread and soup as well as their own meal and this can later be enjoyed by someone in need. It is a concept that is already proving popular with donors."

Developing a centre for excellence

The My Life a Full Life programme is bringing together staff and volunteers across a wide range of organisations to work in a more integrated way. Each of these organisations has its own working practices and as we work more closely together it will be important to learn from each other and build our collective concept of ‘best practice’ in meeting the health and care needs of our community."

As part of this process, two workshops have been arranged by the My Life a Full Life integrated workforce development to explore the concept of a ‘Centre of Excellence’ for health and care on the Island. The purpose of these workshops is to engage with as many people as possible who are involved in the training and development of those people providing health and care to the Island community."

The first workshop will be held on Thursday 3rd March (3-6pm) with a follow up session on Friday 4th March (9am-4.30pm). Both workshops will be held in Room L at the Innovation Centre, St Cross Business Park."

One of the principle aims of the workshop is to collate everyone’s thoughts and ideas, to enable a strategy to be developed ready for implementation."

Family Wellbeing E-Learning

The purpose of the Family Wellbeing Platform (FWP) is to support the development of the most cost-effective, integrated way to deliver a complete, family-centred approach to health improvement on the Isle of Wight that positively impacts on our community's lifestyle, health choices and behaviours."

To achieve this, we recognise that we need to equip all our staff who have a face-to-face contact with individuals and families and to help them develop the confidence to have conversations with individuals and families about their health; to b able to identify health issues and provide basic advice on maintaining good health; to be able to get individuals and families to resolve low level health issues independently; and to make sure they have the knowledge of how to access local health services for high level needs."

At the same time, we realise the practical constraints across our workforce and so we have identified free or low cost e-learning tools which can help practitioners to develop their knowledge of various aspects of health and wellbeing and to develop some key competencies in this area."

These include public health modules such as Hidden Harm, Parental Mental Health, Childhood Obesity and HENRY (Healthy, Exercise and Nutrition for the Really Young) and Alcohol E-learning. Modules also include Making Every Contact Count and Motivational Interviewing."

Care in the Garden

The grounds of St Mary's Hospital will soon be in bloom thanks to a partnership project with a local community interest company."

Care in the Garden, a Wootton-based initiative supporting people with disabilities and long-term health conditions by providing assisted work places and opportunities for training in the horticulture field, has successfully secured a contract to undertake flower planting at St Mary's."

Care in the Garden teams will produce 15 planters made out of recycled pallet wood for provision at various clinics on the hospital grounds. These will be supplied with two rounds of planting in the Spring and Autumn, and maintained throughout the year by Care in the Garden."

The locations for the planters will include entrances to a number of departments including Podiatry, Speech Therapy, Orthodontics/prosthetics, the Allergy Centre, Breast Screening Unit, Laidlaw Day Hospital and the Main hospital entrance."

The work follows a successful trial carried out by Care in the Garden last summer when hanging baskets were maintained at two hospital clinics - Podiatry and Orthotics. "

Charles Joly, Environmental & Sustainability Manager for the Isle of Wight NHS Trust said:"

"A number of patients spontaneously provided positive comments to both departments during the trial last year, so we are pleased to have teamed up with care in the Garden for a more ambitious project this

time around. The provision of the flower planters will positively contribute to carry the image of a welcoming site for patients, as part of a high quality care experience for everyone, every time.""

The project is being funded through the Trust’s Charitable Funds, which can be used for purposes above operational needs, such as enhancing the patients’ environment in this case "

Care in the Garden works with various people including those who have suffered from isolation and exclusion. It supports people with disabilities and long-term health conditions by providing assisted work places and opportunities for training,"

Charles added: "We are delighted that Care in The Garden will be back on site shortly to build on their excellent work from last year. Their work contributes to the development of sustainable clinical and care models for the Island, with less reliance on acute care and more emphasis on self-management and prevention, which is aligned with the goals of the My Life a Full Life programme.""

Wave project to help local children

An innovative surfing project is being expanded on the Isle of Wight following confirmation of support from the Isle of Wight NHS Trust Charitable Funds. The Wave Project will deliver its award-winning peer-led surfing courses, plus a weekly surf club, to help groups of young people, who are referred by services such as the child and adolescent mental health service, social services and mental health charities, feel more confident, improve their outlook and give them a sense of fun."

The funding – totalling £16,959 – will be used to employ a Wave Project Co-ordinator and cover running costs of sessions on the Isle of Wight. Sessions will also be supported with the help of local volunteers. The initiative demonstrates how the voluntary and statutory sectors can work together in line with the principles of the My Life a Full Life programme."

Joe Taylor, Chief Executive of the Cornwall based Wave Project says: “We’re really excited to be working with the Isle of Wight NHS Trust on this project. Thanks to the support of local volunteers, the Wave Project has been running on the Island since 2014, but this new funding will enable us to reach lots more children at risk of isolation."

Army Medical Services Study Day

Do you want a chance to learn about trauma, infection control and leadership from Army Professionals? Army Medical Services are holding a FREE study day and, in line with the My Life a Full Life principles, the training event is open to all healthcare professionals, across a multi-disciplinary, multi-agency audience; doctors, nurses, paramedics, allied health professionals, healthcare assistants, voluntary and independent sector healthcare workers and students."

During the morning you'll hear from key speakers about battlefield trauma and aero-medical evacuation within Afghanistan, the medical response to Ebola in West Africa and infectious diseases and their risk to the NHS. In the afternoon session, topics covered will include pre-hospital triaging and treating multiple casualties in Afghanistan and Iraq and clinical leadership - experiences from Afghanistan and Sierra Leone. "

The event takes place on Saturday 27th February, 8.30am-4.15pm at the Education Centre, St Mary's Hospital, Newport. Admission is free and lunch and a CPD certificate will be provided. Singing for Breathing

The popular SingAbout: Singing for Breathing project run by Independent Arts has received funding allowing it to run for a further two years. The charity has been awarded a grant raised by HealthShape through The Peoples Health Trust, enabling them to offer sessions to people with Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions. "

SingAbout: Singing for Breathing is based on a recognised form of therapy specifically developed for individuals with a respiratory condition. Extensive research has been carried out in this area, with flagship projects being delivered at the Royal Brompton and Harefield Hospital in London. Independent Arts ran its pilot, funded by a grant from My Life a Full Life at the Respiratory Department in partnership with The Isle of Wight NHS Trust. "

Each session includes physical and vocal warm up activities, the vocal production of phonic sounds, singing together and relaxation strategies. Singing for Breathing has been demonstrated to reduce anxiety about breathing difficulty and increase confidence in self-managing everyday activity and living. Participants have described engaging in more social and valued activities and experience less difficulty breathing. "

Jessica Paine, Project Manager said, “We are thrilled to be able to offer Singing for Breathing sessions again after our pilot project proved to be hugely popular with those that attended. All of our previous participants have noticed an improvement in their overall wellbeing, with many citing increased confidence and improved ability to self-manage their respiratory condition.” "
