Free, drop-in fitness program that offers classes with no registration and no commitment. Membership to recreation centers are not required. Classes include dance fitness, aerobics, kickboxing, high intensity interval training, walking, jogging, running, yoga, etc.

2. Diabetes Class at Jacobi
Jacobi patients referred to Diabetes Self-Management care can participate in group discussions and one-on-one sessions. The topics depend on patient’s needs as well as reason for referral. Topics include but are not limited to: disease management, disease process, chronic/acute complications, meal planning, exercise, problem solving, medications (oral agents/insulin teaching), and monitoring blood glucose.

At patient’s request or doctor’s suggestion, patients can receive counseling from a dietician or a public health educator concerning lifestyle adjustments related to diabetes.

Program at Kings County
provides a six-week diabetes self-management program along with nutrition and cooking classes. Patients are educated on diabetes risk factors and disease management methods through culturally sensitive education material, brochures, and video tapes. Education is provided in both group and individual settings in English, Spanish, and Haitian Creole. Referral from primary care provider with lab information about HbA1c and lipids needed prior to starting class (this info can be brought at the beginning of class or the PCP can fax it to the provided fax number).

Month long program consisting of four classes. First class covers what diabetes is, myths, risk factors, etc. Second class is nutrition, discussing diet management, blood sugar, carbs, proteins, fats, etc. Third class discusses blood pressure, A1c, cholesterol, hypo- and hyperglycemia, taking sugar levels, and management skills. Fourth is long-term complications (eyes, kidneys, feet, etc.), exercise, and smoking cessation. If there’s a fifth class, it is a support group;

This group hosts casual, judgment-free runs that vary in length from 2 miles to 10 miles, but participants are encouraged to run however much they’re comfortable with. Runs conclude with brunch. The Club also hosts track workouts and tempo runs consisting of warm-up

24. Citibikes These rental bikes allow individuals a healthier way to travel through the city, turning commuting into an opportunity to exercise regularly. Day passes for an unlimited number of 30 minute rides cost $12, 3-day passes for an unlimited number of 30 minute rides cost $24, and

NYC Parks and Recreation Department maintains many facilities dedicated to fitness and physical activity. These recreation centers offer exercise equipment, fitness and dance classes, gymnasiums, indoor tracks, indoor pools, and sports programs. Annual membership is $150 for adults ages 25-61 for access to all facilities, while membership to facilities without indoor pools goes for $100. Six month memberships are $75 for all centers and $50 for centers without pools. For people aged 18-24 or above 61 years old, annual membership is $25.

1.Diabetes support group at Queens Hospital
Free support groups for people to ask questions, stay engaged in their health, and learn about various topics related to diabetes including healthy eating, exercise, traveling, kidney disease, etc. Sometimes speakers attend. Groups are free and open to the public, but they are also intended to bring people into the Queens Hospital for treatment and one-on-one counseling with doctors and dieticians. Still, individuals may attend the support group without necessarily being members of the facility. PCP referral is needed for treatment and counseling

2. Diabetes Classes at Elmhurst Hospital
Patients at Elmhurst can be referred by their PCPs to make an appointment for a group class. At these classes, they review what stages the patients are at, give general information, and dedicate time to presentations and the sharing of experiences. Classes are taught in English and in Spanish. In addition, Elmhurst also has CDE, dieticians, and endocrinologists who can offer counseling and education to patients with PCP referrals.
