    provides a six-week diabetes self-management program along with nutrition and cooking classes. Patients are educated on diabetes risk factors and disease management methods through culturally sensitive education material, brochures, and video tapes. Education is provided in both group and individual settings in English, Spanish, and Haitian Creole. Referral from primary care provider with lab information about HbA1c and lipids needed prior to starting class (this info can be brought at the beginning of class or the PCP can fax it to the provided fax number).

    2. Diabetes Classes at Elmhurst Hospital

    Patients at Elmhurst can be referred by their PCPs to make an appointment for a group class. At these classes, they review what stages the patients are at, give general information, and dedicate time to presentations and the sharing of experiences. Classes are taught in English and in Spanish. In addition, Elmhurst also has CDE, dieticians, and endocrinologists who can offer counseling and education to patients with PCP referrals.
