Applies  to  Cocker  Spaniel,  English  or  American,  and  Cockapoo  Owners  and  Breeders  with  the  intent  to  produce  infusion  litters.  This  form  should  be  submitted  to  the  Registrar  before  the  mating  as  a  check  and  balance,  to  be  sure  all  health  testing  and  registration  requirements  have  been  met.    Both  Sire  and  Dam  are  required  to  be  tested  to  the  Gold  Paw  Level  in  order  for  the  litter  to  be  registered.    Submit  this  form  to:    Registrar@alaa-­‐labradoodles.com
