GENERAL NOTES
1. THE CONTRACTOR SHALL COORDINATE ALL WORK WITH LOCAL, STATE, AND FEDERAL AGENCIES AS REQUIRED, AND WITH ALLADJACENT UTILITY OWNERS, WHETHER PUBLIC OR PRIVATE, INCLUDING COORDINATING ALL WORK WITH ANY OTHER PROJECTSPLANNED OR BEING CONSTRUCTED WITHIN THE PROJECT AREA AS NEEDED.
2. THE CONTRACTOR MUST COMPLY WITH ALL FEDERAL, STATE AND LOCAL CODES.
3. THE ILLINOIS DEPARTMENT OF TRANSPORTATION "STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION," LATESTEDITION, AND ALL ADDENDA THERETO, AND CITY OF CHICAGO "RULES AND REGULATIONS FOR CONSTRUCTION IN THE PUBLICWAY", LATEST EDITION, SHALL GOVERN ALL WORK UNDER THIS CONTRACT.
4. ALL WORK SHALL BE CONDUCTED IN ACCORDANCE WITH OSHA REQUIREMENTS.
5. ALL MATERIALS SHALL CONFORM TO APPLICABLE STANDARDS ESTABLISHED BY ANSI, NEMA, NBFU AND SHALL BE "U.L." LISTEDWHERE APPLICABLE. ALL MATERIALS AND EQUIPMENT SHALL BE NEW AND IN GOOD WORKING CONDITION WHEN INSTALLEDAND SHALL BE OF THE BEST GRADE AND OF THE SAME MANUFACTURER THROUGHOUT FOR EACH CLASS OR GROUP OFEQUIPMENT.
6. THE LOCATIONS AND ELEVATIONS OF EXISTING UNDERGROUND UTILITIES SHOWN OR REFERENCED ON THE PLANS AND PROFILESHAVE BEEN OBTAINED FROM DRAWINGS AND DATA RECEIVED FROM VARIOUS SOURCES. UTILITY DEPTHS ARE LABELED INPROFILES WHEN PROVIDED BY THE UTILITY. UTILITIES NOT LABELED IN PROFILES ARE DRAWN AT ASSUMED DEPTHS.CONTRACTOR TO VERIFY HORIZONTAL AND VERTICAL LOCATION OF ALL UTILITIES IN THE FIELD PRIOR TO EXCAVATION.
7. CONTRACTOR TO CALL DIGGER AT 312-744-7000 A MINIMUM OF 48 HOURS PRIOR TO ANY EXCAVATION TO HAVE EXISITNGUTILITIES LOCATED AND MARKED OR STAKED.
8. THE EXISTENCE AND LOCATION OF UNDERGROUND UTILITIES SHALL BE OBTAINED FROM ALL UTILITY COMPANIES ANDINVESTIGATED AND VERIFIED IN THE FIELD BY THE CONTRACTOR BEFORE STARTING WORK. EXCAVATION IN THE VINCINITY OFEXISTING UTILITIES AND UTILITY STRUCTURES SHALL BE PERFORMED BY HAND. THE CONTRACTOR WILL BE RESPONSIBLE FORTHE MAINTENANCE AND PRESERVATION OF THESE UTILITIES. ANY AND ALL DAMAGES TO EXISTING FACILITIES SHALL BEREPAIRED TO THE SATISFACTION OF THE UTILITY OWNER AT THE CONTRACTOR'S EXPENSE.
9. THE CONTRACTOR SHALL VERIFY ALL DIMENSIONS AND CONDITIONS AFFECTING THEIR WORK WITH ACTUAL CONDITIONS AT THEJOB SITE. IF THERE ARE ANY DISCREPANCIES FROM WHAT IS SHOWN ON THE CONSTRUCTION PLAN, HE MUST IMMEDIATELYREPORT SAME TO THE ENGINEER BEFORE DOING ANY WORK, OTHERWISE THE CONTRACTOR ASSUMES FULL RESPONSIBILITY. INTHE EVENT OF DISAGREEMENT BETWEEN THE CONSTRUCTION PLANS, STANDARD SPECIFICATIONS AND/OR SPECIAL DETAILS,THE CONTRACTOR SHALL SECURE WRITTEN INSTRUCTION FROM THE ENGINEER PRIOR TO PROCEEDING WITH ANY PART OF THEWORK AFFECTED BY OMISSIONS OR DISCREPANCIES, FAILING TO SECURE SUCH INSTRUCTION, THE CONTRACTOR WILL BECONSIDERED TO HAVE PROCEEDED AT HIS OWN RISK AND EXPENSE. IN THE EVENT OF ANY DOUBT OR QUESTION ARISING WITHRESPECT TO THE TRUE MEANING OF THE CONSTRUCTION PLANS OR SPECIFICATIONS, THE DECISION OF THE ENGINEER SHALL BEFINAL AND CONCLUSIVE.
10. THE CONTRACTOR SHALL OBTAIN ALL NECESSARY PERMITS AND PAY THE REQUISITE FEES TO THE CITY OF CHICAGO PRIOR TOCOMMENCING WORK.
11. ALL WORK AND MATERIALS WHICH DO NOT CONFORM TO THE SPECIFICATIONS ARE SUBJECT TO REMOVAL AND REPLACEMENTAT THE CONTRACTOR'S EXPENSE.
12. DO NOT SCALE PLANS FOR CONSTRUCTION DIMENSIONS.
13. THE CONTRACTOR SHALL OBTAIN THE ADJACENT PROPERTY OWNERS' PERMISSION BEFORE LABOR, MATERIALS, EQUIPMENT ORWORK IS STORED/PERFORMED ON THE LANDS OF OTHERS. CDOT RULES AND REGULATIONS SHALL BE FOLLOWED WITHRESPECT TO THE STORAGE OF MATERIALS IN THE PUBLIC WAY.
14. ALL UTILITY TRENCHES UNDER OR WITHIN TWO FEET OF PAVEMENT, SIDEWALK, CURB AND GUTTER, ETC. SHALL BE BACKFILLEDWITH CA-6 CRUSHED STONE (GRADE 8 OR 9), COMPACTED IN 9" LIFTS TO 95% OF MODIFIED PROCTOR.
15. UTILITY CONNECTIONS WITHIN THE STREET RIGHT-OF-WAY SHALL BE ACCOMPLISHED BY SAWCUTTING AND REMOVING THEEXISTING PAVEMENT. BACKFILL AND RESTORATION SHALL BE DONE IN ACCORDANCE WITH CDOT RULES AND REGULATIONS.
16. CONTACTOR SHALL RECORD THE LOCATION AND ELEVATION OF ALL NEW WORK INSTALLED AND OF ALL UTILITIESENCOUNTERED AS WORK PROGRESSES AND SHALL PREPARE AS-BUILT DRAWINGS BASED ON THESE RECORDS, TO BE SUBMITTEDTO COMED UPON COMPLETION OF WORK.
CHICAGO DEPARTMENT OF WATER MANAGEMENT
1. THE MINIMUM VERTICAL CLEARANCE (EDGE-TO-EDGE) FROM ALL WATER MAINS AND SERVICES IS 18-INCHES. FOR GRID MAINS(WATER MAINS LESS THAN 16-INCHES) AND WATER STRUCTURES (VALVE BASINS, SERVICE CONTROL VALVES, ETC.), THEMINIMUM HORIZONTAL CLEARANCE (EDGE-TO-EDGE) IS THREE (3) FEET. FOR FEEDER MAINS (WATER MAINS 16-INCHES ANDLARGER), THE MINIMUM HORIZONTAL CLEARANCE (EDGE-TO-EDGE) IS FIVE (5) FEET.
2. FOR ALL GENERAL CLEARANCE REQUIREMENTS (NON-WATER):DWM REQUIRES A MINIMUM HORIZONTAL CLEARANCE OF ID (INNER DIA. OF THE SEWER) + 4' CENTER-TO-CENTER OR 4 FEET EDGE-TO-EDGE, WHICHEVER IS LARGER, AND A MINIMUM VERTICAL CLEARANCE OF 18" EDGE-TO-EDGE. IF EITHER OF THESE CONDITIONS ARE NOT MET, THE SEWER MUST BE REPLACED WITH DIP OR REHABILITATED WITH AN APPROPRIATE LINER IF DIRECTED BY THE CITY.
3. IN NO CASE SHALL THE INSTALLATION OF ANY PROPOSED FACILITY BE CLOSER THAN FIVE (5) FEET AROUND A FIRE HYDRANT ORFIRE HYDRANT LEAD.
4. WHERE SHOWN ON THE PLANS, WATER MAINS TO BE TEMPORARILY SUPPORTED FOLLOWING DWM STANDARD DETAILS ANDCALCULATIONS PROVIDED ON THESE PLANS, AS REVIEWED AND APPROVED BY DWM.
5. WHEN EXISTING DRAINAGE FACILITIES ARE DISTURBED, THE CONTRACTOR SHALL PROVIDE AND MAINTAIN TEMPORARY OUTLETSAND CONNECTIONS FOR ALL DRAINS, SEWERS, AND APPURTENANCES.
6. EXTREME CAUTION MUST BE TAKEN TO AVOID DAMAGE TO ANY WATER OR SEWER FACILITIES. IF DAMAGE OCCURS TO THISDEPARTMENT'S FACILITIES, THE CONTRACTOR AND/OR OWNER WILL BE RESPONSIBLE FOR THE COST OF REPAIRING ORREPLACING THE DAMAGED FACILITIES.
7. WHERE ANY SEWER WORK IS REQUIRED, INCLUDING ADJUSTMENT OF SEWER STRUCTURES AND REMOVAL/REPLACEMENT OFFRAMES AND LIDS, THE CONTRACTOR MUST OBTAIN A PERMIT FROM DWM AND THE WORK MUST BE PERFORMED BY ALICENSED DRAIN LAYER UNDER PERMITS AND INSPECTION OF THE CITY OF CHICAGO DEPARTMENT OF WATER MANAGEMENT.A DWM SEWER INSPECTOR MUST BE PRESENT DURING THE WORK.
8. FAILURE TO COMPLY WITH THE ABOVE REQUIREMENTS MAY RESULT IN ADDITIONAL EXPENSES TO THE PROPOSED PROJECT TOVERIFY THAT ALL WORK CONFORMS TO DWM'S STANDARDS.
9. ALL SEWER INSTALLATIONS MUST MEET IEPA SEPARATION REQUIREMENTS FOR WATER AND SEWER PIPES. ALL PROPOSED/REPLACED SEWER LATERALS FROM CATCH BASINS/INLETS, SEWER MAINS, AND PRIVATE DRAINS (COLLECTIVELYKNOWN AS 'SEWER FACILITIES') THAT ARE PARALLEL TO WATER MAINS, SERVICES, OR FIRE HYDRANT LEADS (COLLECTIVELYKNOWN AS 'WATER FACILITIES') THAT ARE LESS THAN 18 INCHES BELOW THE WATER FACILITY AND HAVE LESS THAT 10 FEET OFHORIZONTAL SEPARATION FROM THE OUTSIDE EDGE OF THE WATER FACILITY MUST BE MADE OUT OF DUCTILE IRON/WATERMAIN QUALITY PIPE FOR 10 FEET ON EITHER SIDE OF THE OUTSIDE EDGE OF THE WATER FACILITY, ADDITIONALLY, ALL SEWERFACILITIES THAT CROSS PERPENDICULARLY BELOW WATER FACILITIES WITH LESS THAN 18 INCHES VERTICAL SEPARATION MUSTBE MADE OUT OF DUCTILE IRON/WATER MAIN QUALITY PIPE FOR 10 FEET ON EITHER SIDE OF THE OUTSIDE EDGE OF THE WATERFACILITY. IN ANY SEWER FACILITY CROSSES PERPENDICULARLY ABOVE A WATER FACILITY, THEN THE SEWER FACILITY SHALL BE ATLEAST 18 INCHES ABOVE THE WATER FACILITY AND THE SEWER FACILITY MUST BE MADE OUT OF DUCTILE IRON/WATER MAINQUALITY PIPE FOR 10 FEET ON EITHER SIDE OF THE OUTSIDE EDGE OF THE WATER FACILITY. SEWER LATERALS THAT REQUIREDUCTILE IRON/WATER MAIN QUALITY PIPE SHALL BE DUCTILE IRON/WATER MAIN QUALITY PIPE FROM THE CATCH BASIN TO APOINT 10 FEET BEYOND THE EDGE OF THE WATER FACILITY.
PEOPLE'S GAS
1. PEOPLES GAS FACILITIES ARE PRESENT WITHIN AREA OF CONSTRUCTION.USE EXTREME CAUTION NEAR ALL GAS FACILITIES DURING CONSTRUCTION ANDRELATED EXCAVATION ACTIVITIES. HAND EXCAVATION IS REQUIRED TO FIELD VERIFYTHE HORIZONTAL AND VERTICAL LOCATION OF GAS MAIN(S) PRIOR TO CROSSING ANDWORKING WITHIN 3 FEET OF ALL GAS FACILITIES. A MINIMUM OF 3' HORIZONTAL EDGE TOEDGE CLEARANCE IS REQUIRED FOR GAS MAINS WITH DIAMETERS OF 16" OR SMALLER, AND5FT EDGE TO EDGE CLEARANCE FOR GAS MAINS WITH DIAMETERS 18" AND LARGER.MAINTAIN A MINIMUM OF 18" EDGE TO EDGE VERTICAL CLEARANCE WHEN CROSSING GASMAINS 16" OR LESS IN DIAMETER, AND 24" EDGE TO EDGE VERTICAL CLEARANCE WHENCROSSING 18" AND LARGER DIAMETER GAS MAINS. COMED CONSTRUCTION NOTES - DISTRIBUTION
1. USE EXTREME CAUTION NEAR COMED FACILITIES. HAND DIG WHILE CROSSING 69/138/345 kV
TRANSMISSION LINES. COMED TRANSMISSION SHALL BE NOTIFIED 2 BUSINESS DAYS PRIOR TO
THE START OF WORK. TO SCHEDULE AN ONSITE INSPECTOR DURING CONSTRUCTION, CONTACT
LESLIE PASCHAL AT (630) 437-4767.
2. THE CONTRACTOR SHALL PROVIDE ALL MATERIAL, LABOR AND EQUIPMENT REQUIRED TO
FURNISH AND INSTALL THE WORK AS DESCRIBED HEREIN, AND SHALL BE RESPONSIBLE
FOR ALL JOB SITE DELIVERY COSTS AND UTILITY RELOCATION CHARGES AS REQUIRED.
3. PRIOR TO EXCAVATION, THE CONTRACTOR SHALL SAWCUT EDGES OF PROPOSED TRENCH
AND MARK THE PROPOSED CONDUIT ROUTE ON THE PAVEMENT SURFACE. IN UNPAVED
AREAS THE TRENCH SHALL BE MARKED OFF BY MEANS OF A CHALK LINE.
4. PRIOR TO THE START OF WORK, THE CONTRACTOR SHALL VERIFY LOCATIONS OF ANY
OVERHEAD OBSTRUCTIONS. PRIOR TO EXCAVATION, THE CONTRACTOR SHALL PERFORM
TEST HOLES AS NEEDED ALONG PROPOSED CONDUIT ROUTE AND AT PROPOSED OR
EXISTING MANHOLE LOCATIONS TO VERIFY HORIZONTAL AND VERTICAL LOCATION OF
EXISTING UTILITIES.
5. CONTRACTOR TO OVER-EXCAVATE 3-INCHES ALL AROUND PROPOSED STRUCTURES
AND CONDUIT.
6. THE CONTRACTOR IS RESPONSIBLE FOR SUPPORTING THE TRENCH OR EXCAVATION
IN ACCORDANCE WITH OSHA REGULATIONS IN ORDER TO PREVENT TRENCH SIDES
FROM CAVING IN. TRENCH/EXCAVATION SUPPORT SHALL BE SUITABLE TO FIELD
CONDITIONS AND SHALL BE DESIGNED, SUPPLIED, AND INSTALLED BY THE CONTRACTOR
AND SHALL BE APPROVED BY COMED.
7. WHERE EXCAVATION REQUIRES SHORING, THE CONTRACTOR SHALL INSTALL A 6"
CONCRETE MUDSLAB AT THE BASE OF EXCAVATION. CONTRACTOR SHALL DESIGN
AND PLACE SHORING SO AS NOT TO CUT OFF ANY EXISTING UTILITIES.
8. THE CONTRACTOR SHALL DEWATER ALL EXCAVATIONS SO THAT THE GROUNDWATER
TABLE REMAINS BELOW THE BASE OF EXCAVATION.
9. THE CONTRACTOR SHALL REMOVE ALL EXCAVATED MATERIAL ON A DAILY BASIS.
STOCKPILING OF MATERIAL IS NOT ALLOWED.
10. THE CONTRACTOR SHALL THOROUGHLY INSPECT ALL EXCAVATIONS PRIOR TO INSTALLING
PROPOSED CONDUIT OR STRUCTURES. ANY EXISTING INFRASTRUCTURE IN CONFLICT
SHALL BE VERIFIED ABANDONED BY THE CONTRACTOR AND REPORTED TO COMED,
THE UTILITY, AND THE ENGINEER OF RECORD PRIOR TO REMOVAL.
11. THE CONTRACTOR SHALL ASSUME ALL CABLES ARE ENERGIZED AND MUST BE
SUPPORTED SO AS NOT TO STRESS ANY PART OF THE CABLE.
12. THE CONTRACTOR SHALL CLEAN ALL DEBRIS FROM MANHOLES WORKED IN, INCLUDING ANY
DEBRIS PRESENT IN EXISTING MANHOLES PRIOR TO THE START OF WORK. CONTRACTOR TO
PUMP MANHOLES EACH DAY AS REQUIRED.
13. THE CONTRACTOR SHALL NOTIFY COMED AND THE ENGINEER OF RECORD WHEN PROPOSED
DEPTH OF EXCAVATION EXCEEDS THE DEPTH SHOWN ON THE PLANS. CONTRACTOR SHALL
ALSO NOTIFY THESE PARTIES WHEN THE REQUIRED HORIZONTAL AND/OR VERTICAL
SEPARATION FROM EXISTING UTILITIES CANNOT BE MAINTAINED. IN THESE SITUATIONS,
