STATEMENT OF QUALIFICATIONS AND SEALED COST PROPOSALREQUIREMENTS AND PROJECT INFORMATION

1.1 PROuECT SCHEDULE

1.2 PREPROPOSAL MEETING

1.3 EVALUATION, SELECTION, AND PROJECT AWARD

A Sealed Cost Proposal (SCP) must accompany each Statement Of Qualifications (SOQ) to beconsidered. Each SOQ will be evaluated by a selection committee, individuatry scored by eachcommittee member based on the proposal requirements and scoring criteria, and the scoresaveraged for each SOQ. SOQ shall not contain proposed costs for the project. Any SOQ

packages with proposed costs included will result in disqualification of the SOQ/SCP. SCP fortthe three hishest scoring SOQ will be opened. The selection committee will forward to the CftyCouncil a recommendation based on the SCP and SOQ.

1.4 SUBMITTAL REQUIREMENTS

Each respondent mLlst submit five (5) copies Of its SOQ and one (1 ) SCP together jn asealed en`/elope or package to Grantsville City. SCP that have not been indMduallysealed within the larger SOQ package will result in disqualification of the SOQ/SCP. TheSSOQ shall not inelLide any proposed costs from the SOP. Any SOQ packages withproposed costs included will result in disqualification Of the SOQ/SCP.

1.5 SOQ ORGANIZATION AND CONTENT

All requested information must be included and assembled in the following order:

1.5.1 Transmittal Letter

A cover/transmittal letter with a statement confirming the respondent's intent to participate in theProject. The letter shall be provided on official business or company letterhead and shall includethe following:

a. A statement that the respondent will comply with all terms and conditions of theSOQ/SOP.

b. A statement indicating the respondents business structure, i.e. corporation, partnership,etc.

c. A statement of affirmative action that the respondent does not discriminate in itsemployment practices with regard to race, color, religion, age (except as provided bylaw), sex, marital status, political afflliation, national origin, or handicap.

d. A certification statement to the effect that the person signing the SOQ/SCP is authorizedto do so on behalf of the respondent.

e. Names of a primary, secondary, and tertiary contact for the respondent together withtheir titles and telephone numbers.

f. Name and complete mailing address of the respondent along with a business telephoneand fax number.

1.5.2 SOQ Requirements

TThere is no page limit for the SOQ. However, respondents are cautioned to keep theirresponses concise and specific to the Project conditions and requirements. Each SOQ shallcontain the fdilowing information:

a. General Information: General description Of the respondent's company/business and

qualifications to undertake and successfully complete the projectb. Key Personnel: Identify superintendent, foreman, and other key personnel that wiH be

responsible for the Project.c. Relevant Project Experience: A description of six (6) projects completed within the past

three years Of similar size and scope together with the total project cost and key

personnel.d. Relevant Project References: Provide references for the six (6) projects complcte with

their titles and phone numbers.e. Project Schedule: Provide a Project schedule incorporating aH fourteen sub-projects

together with their completion dates. The completion date for the entire Project isSeptember llth, 2o2o.

1.5.3 SOQQ Scoriria

SSOQ will be evaluated on responsiveness and content as fctlows:

a. General Information: 10 Pointsb. Key personnel: 10 Pointsc. Relevant project Experlence: 20 Pointsd. Relevant project Reference Responses: 20 Polntse. Project schedule: 20 Pointsf. Proposed project Fee schedule; 20 Points

1.6 PAVING TEMPERATURE REQUIREMENT

Ambient temperatures must be at least 50°F for asphalt paving.

1.7 SCP ORGANIZATION AND CONTENT

Ssoreen captured aenal Google Earth® images for the Project and twelve sub-projects areaattached. The sub-project images include schematic work limits for reference. The SCP shallinclude a lump sum cost for each sub-prQiect and a total PrQiect lump sum cost.

The Contractor shall assess the work associated with each sub-project and provide a lump sumccost for each sub-project and a total Project lump sum cost. Any conditions encountered duringconstruction that are considered to be beyond the project scope Of work as described in thesolicitation wouid be negotiated in good faith between the City and the Contractor awarded thePrgiv.

The SOP shall also include a unit price per square foot for two (2) items which are:

1. Subgrade Removal and preparation2. Soft spot Repair

These items shall be complete with aM necessary incidentais, labor, equipment, materials,QA/QC, and traffic control.

Replace concrete waterways per city standards. The City willsubmit and obtain a UDOT encroachment permit associated with this sub-project.

1.7.2 Sub-Droiect 2: North St. Overlav and Intersection Rehab

place and compact 2inch APWA 14-inch 50 overlay complete with all necessaryincidentals, labor, equipment, materials, QA/QC, traffic control, adjusting surfece utilitycomponents (manholes, valve boxes, etc.) and clean-up.

Density testing shall be performed by a third-partytesting agency and shall be the responeibilfty Of the contractor. Remove Existing cattle guard near the intersection of Pear St. and West St. Removeand properly dispose of concrete cattle guard south Of Pear St. Replace southernmostcattle guard with steel cattle guard removed just north of Pear St including reroutingtraffic to the shoulder during replacement.12-inches Of import roadbase shaH be placedon shoulder to allow for heavy truck traffic throughout the replacement (~3,000 SQ FT).Import roadbase and compact to 95% to fill void Of removed cattle guard near Pear St.Sawcut and remove a 2-foot strip Of pavement at each transition to existing asphalt,

1.7.14 Sub-oroiect A1 : Alternate -Keari Street (~12.000 SQ FT)

Sawcut and remove a 2-foot strip Of pavement at each transition to existing asphalt,

place and compact 2-inch APV\IA %-inch 50 overlay complete with all necessaryincidenfals, labor, equipment, materials, QA/QC, traffic control, adjusting surface utilftycomponents (manholes, valve boxes, etc.) and clean-up.

1.7.15 Unit Price for SoftsDot repair

Contractor shall provide a unit price for softspot repair. Softspot repair shall includeimport roadbase, compaction efforts, and grading efforts required to fill soft spots.Assume 500 SQ FT softspot repair per location and provide a SQ FT unit price forrepairs.

MContractor shall be required to develop and submit a traffic control plan for each

project prior to start of work. If the contractor does not properly perform the approvedtraffic control plan, costs associated will be deducted from their payment.H

1.8 SUBMITTAL OWNERSHIP

All SOQ/SCP and the information contained therein shall become the property of the City. Priorto submitting, proposers should carefully consider any information that would not be considereddisposable. SOQ/SCP submitted may be reviewed and evaluated by any persons at thediscretion of the City. No SOQ/SCP shall be returned to the respondent regardless of theselection outcome. Costs for developing SOQ/SOP are entirely the responsibility of the proposerand shall not be chargeable in any manner to the City.

1.9 CITY USE OF PROPOSAL IDEAS

The City reserves the right to use any or all ideas presented in the SOQ/SOP. Selection orrejection does not affect this right.

1.11 ACCEPTANCE OF PROPOSAL

1.11.1 Riaht to Reiect and/or Terminate

a. The Cfty reserves the right to reject any or all proposals for any reason and to waiveminor irregularities with to do so would be in the best interests Of the City. Minorirregularities are those which will riot have a significant adverse effect on overallcompetition or performance levels.

b. The Cfty reserves the right to r?ject any sub-projects or parts Of sub-projects within theaccepted proposal. The Cfty reserves the risht to make these rejections at any time andfor any reason at its sole discretion.

c. The respondent agrees that the Cfty may terminate this procurement procedure at anytime and for any reason, and the City shall have no liability or responsibHfty to therespondent for any costs or expenses inourred jn connection with SOQ/SOP.

1.12 DISQUALIFICATION

1.12.1 Disqualification of SOQ/SCP

TThe City reserves the right to reiect any and all SOQ/SOP received dy reason Of this solicitation,or to negotiate separately with any source whatsoever in any manner necessary to serve thebest interests Of the City. The Cfty may or may not award a contract solely based on thisssolicitation and will not pay for the information solicited or obtained. The information obtainedwill be used in determining the proposal that best meets the City's needs and is the mostaadvantageous proposal received. No oral, telegraphic, or telephonic proposals or modificationswill be considered.

1.12.2 Reasons for Disaualification Of SOQ/SCP

TThe occurrence of any of the following may result in disqualification of SOQ/SCP

a. Failure to respond by the established submission deadlineb. Failure to completely answer all questions posed in this solicitationc. Use Of any other type Of form or format other than those indicated hereind. Failure to provide requested documentation at the time Of submissione. Inclusion of any cost proposal within the SOQf. Illegible responses

9. Including any provisions reserving the rieht to accept or reject and award or to enter intoa contract pursuant to an award, or any other unauthorized conditions, liniitatione, or

provisionsh. Failure to provide evidence of a satisfactory record Of integrityi. Failure to confirm a capabilfty to legally contract for work anticipated dy this solicitation

j. Any contact with City staff not authorized dy the Mayor, Cfty Attorney, or Public VvorksDirector

1.13 WITHDRAWAL OF SOQ/SCP

A SOQ/SOP may be withdrawn upon request Of the submitter without prejudice, prior to, but notafter, the submittal deadline.

2.0 GENERAL REQUIREMENTS

2.1 AMENDMENTS

No oral medificatione or amendments to this solicitation shall be effective. If it becomesnecessary to revise any part Of the SOQ/SCP an addendum will be issued by the Cfty.

2.2 EIVIPLOYMENT STATUS VERIFICATION

Any contractor selected through this solicitation shall fully comply with the Private EmployerVerification Act, Utah Code Annotated §1347-101 et seq. Any contractors, subcontractors,contract employees, staffing agencies, or any other contractors, regardless of their tier mustregister and fully comply with Employer Verification Act, Utah Code Annotated §13-47-101 etSeq.

Any contractor selected through this solicitation shall agree to abide by the Federal and Stateregulations pertaining to Equal Opportunity Employment that requires project participants not todiscriminate because of race, color, religion, sex, age, disability, or national origin. The City willmake every effort to ensure all respondents are treated fairly and equally throughout theevaluation, scoring, and selection process.

2.3 NON-COLLUSION

Submittal of a SOQ/SCP shall be an implicit guarantee that the respondent did not collude withany other respondent to fix any overhead, profit, or cost.

2.3 OMISSIONS

Should this solicitation not contain sufficient information to enable a respondent to gain a clearunderstanding of the services required by the City, or should it appear that the instructions arenot clear or contradictory, potential respondents may request and obtain written clarificationfrom the City unless such request is made within 24-hours of the submittal deadline. A copy ofany written clarification shall be included in the SOQ/SOP.
