Outcomes

The outcomes to be achieved through the Initiative are:

– Divisions to increase participation of GPs and Allied Health Service Providers in aged care homes’ quality initiatives;

– Percentage of accredited Aged Care Facilities taking part in the Initiative to increase to at least 75% by 30 June 2008;

Outcomes

– Residents of aged care homes have increased access to primary and medical care; and

– Effective partnerships and collaboration established and maintained between GPs, allied health service providers, Divisions network members and aged care homes.

Mentor Program

• Goal – to formalise a platform that will encourage Divisions to both share their skills and experience, as well as learn from others.

• Divisions register their interest in and capacity to share the knowledge and experience that they hold in a range of areas

Mentor Program

• Divisions register their interest in accessing information provided by other divisions on a specific subject

• Many divisions already work collaboratively with other divisions - there are always more opportunities for cross-fertilisation of ideas and resources

This is a work in progress - there will be opportunities for input from Divisions & SBOs
