• HfH 7 day service - from 2012• Aimed at 65+, without support network• HfH Integrated within rapid response

team at St Helier Hospital• Low intensity practical support for 4

weeks following discharge

• Includes 2 weekly visits /phone calls• Community Helpers – one to one

friendship and support to re-engage with community life

• Caring Neighbours – extra support to stay well in winter

• Aim to support independence and increase resilience

• Links to ongoing ‘circle of support’

• 200 volunteers in Age UK Sutton

• 26,500 volunteer hours in 2013

• Wide range of roles

• Ongoing recruitment

• Careful matching

• 60 supporting HfH & CH

• Nearly 2,000 hours supporting HfH & CH in last 6 months alone

Volunteer-led organisations have decades of experience at being nimble – resourcing major projects quickly, to reach more of the most vulnerable

• 20 contacts per HFH patient• Exceeding targets• 82% of patients rated service as

good or excellent

• Sustained improvements measured via LEAF outcome scores

• Low hospital re-admissions
