Dental Insurance. At Triad Smile Center, we

wanted to make sure that we can help all of our

patients by making dentistry more affordable

and simple. We’ve created the SMILE PLUS

DENTAL SAVINGS PLAN, which is an

in-house discount program. This is NOT

an insurance policy. Our in-house plan is a

great value with an emphasis on preventative

treatments and offers discounts on ALL services

for a low annual fee. Ultimately, patients will

save money and more importantly, have better

oral health by having consistent dental care. We

welcome you to join our dental family.

*Couple Plan = Parent/Child or Spouse/Partner

**Family Plan = Includes family members and

children through age 25. Each additional

family member is $250.00.

• Initial Administration Fee of $25 per

individual or $50 per couple or family.

• All annual fees are paid in full for Individual

Plans.

• All annual fees are paid with initial down

payment of $350 for Couple & Family Plans.

Balance paid in 1-3 equal installments by

debit or credit card, depends on plan. All

subsequent payments made via automatic

draft or credit card and renewable annually.
