AFFIDAVIT OF SOLO PARENT

I, JOSEPHINE N. GABUNI, of legal age, Filipino, married and a resident of Purok 13 St. Peter St., Crossing Bayabas, Toril, Davao City, after having been sworn to in accordance with law, hereby depose and say that:

1. I am the mother of Bliss N. Gabuni who is now applying for a scholarship under the CHED Student Financial Assistance Programs;

2. my husband and I have long been separated in fact;

3. due to our separation, I am left alone with the responsibility of parenthood for several years already;

4. I am executing this affidavit to attest the truth of all the foregoing and to support the application of my daughter for CHED Student Financial Assistance Programs.
