AFFIDAVIT OF DISCREPANCY

I, ALOYSIUS, FLORENTINO J. AURELIO, of legal age, Filipino, married, and a resident of Pagdalagan Norte, San Fernando City, La Union, Philippines, having been duly sworn in accordance with law, hereby depose and state:

1. That in my Certificate of Live Birth as recorded in the Office of the Civil Registrar of Laoag City, Ilocos Norte, Philippines, my first name that was registered was “ALOYSIUS, FLORENTINO”;

2. Knowing that my name is only “ALOYSIUS”, I had used “ALOYSIUS” only as my first or given name in all pertinent records including all my school records from my grade school to my post graduate studies;

3. That I only recently found out that my correct name was “ALOYSIUS, FLORENTINO” when I secured a certified true copy of my birth certificate from the National Statistics Office (NSO);

4. That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing facts and to explain the discrepancy in my Certificates of Live Birth, as regards my correct first or given name; and

5. That I further declare that from now on, I will be using my correct first or given name as “ALOYSIUS, FLORENTINO”.
