AFFIDAVIT OF CORRECTION

I, MARIA SHEENA SAN JUAN LLORCA, Filipino, single and a resident of

No. 69 20th Avenue Murphy, Cubao, Quezon City, Philippines, after having been

duly sworn to in accordance with the law, do hereby depose and state:

1. That when I enrolled in Roosevelt High School, Cubao Branch I

wrote/used the MA. SHEENA SAN JUAN LLORCA instead of MARIA SHEENA

SAN JUAN LLORCA;

2. That when our School Chief of Records Ms. Ligaya R. Mendoza sent me a

Notice dated October 22, 2004 , hereto attached as Annex A informing me that I

need to change the MA. to MARIA ;

3. To prove that my true name is MARIA SHEENA SAN JUAN LLORCA, I amattaching herewith as Annex B and Annex C respectively copies of myCertificate of Live Birth and Baptismal Certificate to prove that MA. SHEENA SANJUAN LLORCA and MARIA SHEENA SAN JUAN LLORCA is ONE and the SAMEperson.

4.. That I am executing this affidavit to attest to the truth of the foregoing

statements and for whatever legal purposes it may serve.

1. That I am the same CESAR ABAY-ABAY CONCEPCION who wasissued a Philippine Passport by the Department of Foreign Affairs;

2. That I discovered that my Certificate of Live Birth, dulyauthenticated by the National Statistics Office (NSO), has the followingdiscrepancies and/or errors:

a. The last name of my mother, Felicidad Inanoria Abayabay, was

erroneously misspelled as Abaybay instead of ABAY-ABAY;

b. The middle name of my father, Emilio Encabo Concepcion, waserroneously misspelled as Incabo instead of ENCABO.

A copy of the Certificate of Live Birth is hereto attached as AnnexA.

3. That the true and real last name of my mother is ABAY-ABAY andnot Abaybay as reflected in the Certificate of Live Birth dulyauthenticated by the NSO. In fact, I have been using the name ABAY-

ABAY since childhood because the same is the true and legal last name ofmy mother, Felicidad Inanoria Abayabay. Attached hereto, as Annex Bis a copy of the Marriage Contract to prove such facts;

4. That likewise, the middle name of my father is ENCABO and notIncabo as reflected in the said Certificate of Live Birth and the samecould have been due to clerical/typographical error on the part of theclerk who typed and entered such name on the Certificate of Live Birth;

5. That I am executing this affidavit to attest to the truth of theforegoing statements and for whatever legal purposes it may serve.

1. That I am the same Edgardo Barquin Matilac, an applicant forpassport before the Department of Foreign Affairs;

2. That when I submitted my Certificate of Live Birth before theDepartment of Foreign Affairs, I discovered that my middle name wasinadvertently misspelled as Makilac instead of MATILAC. A copy of theCertificate of Live Birth is hereto attached as Annex A;

3. My true and real middle name is MATILAC and not Makilac. Infact, I have been using the said middle name since childhood because thesame is the true and legal family name of my mother, Elpidia M. Matilac.Attached hereto, as Annex B is a copy of the Baptismal Certificate toprove such facts;

4. The error in my middle name could have been due toclerical/typographical error on the part of the clerk who typed theCertificate of Live Birth;

5. I am executing this affidavit to attest to the truth of the

foregoing statements and for whatever legal purposes it may serve.

3. That I since childhood I have been using the name AIDA DIAZDANGOY, the name given by my parents;

4. In fact, even when I attained the age of majority, I have beenusing the name Aida Diaz Dangoy in all my official dealings and recordsuntil just lately, when I applied for a Philippine Passport, that I discoveredthat my Certificate of Live Birth bears the name IDA DIAZ BANGOY;

5. I hereby declare that the name AIDA DIAZ BANGOY as appearingin my other official records, and the name IDA DIAZ BANGOY as

appearing in my Certificate of Live Birth refers to ONE and the SAMEperson;
