 AFFIDAVIT

I, ELENITA D. TAPUSOK, of legal age, Filipino, married and a

resident of Davao City, Philippines, after being duly sworn to in

accordance with law, do hereby depose and say that:

1.I am the Attorney-in-fact of Ms. Melba Lao, the owner of LJ-Sky

 Ticketing Services to transact and process its documents with

Business Bureau, BIR and other local government agencies by virtue of an SPA notarized by Atty. Angela Librado denominated AS

Doc No. 225, Page No. 45, Book CCLXVIII, Series of 2015;

2.LJ-Sky Ticketing Services is only engaged in purely ticketing

 business;

3.It is not engaged in travel and tour business;

4.I execute this affidavit to attest to the truthfulness or veracity ofthe foregoing facts and to support the membership application of

my daughter with the Office of the PHIC and for whatever legal

purposes this may serve.
