STATUTORY DECLARATION

I, JAYROME EDU RALA, of legal, age, married and a resident of 005 Eagle Street Marfori Heights Phase 2, Davao City after being sworn in accordance with law, hereby depose and state THAT:

1. I worked as a Project Monitoring Specialist of the United States Agency for International Development (USAID) – Growth with Equity in Mindanao (GEM) Program implemented by the Louis Berger Group Philippines, Inc, with principal address located at Unit 3, 12/F, Export Bank Plaza Sen. Puyat corner Pasong Tamo, Makati City, Philippines. The GEM Program is under the Mindanao Peace and Development Bilateral Assistance Program of the USAID and the Mindanao Development Authority;

2. I am employed in the said Office starting on January of 2010 up to the present time;

3. I wish to apply for Immigration through Skills Assessment Program

undertaken by VETASSESS with the Australian Government and I have opted for the Employer Nomination Scheme (ENS);

4. As part of their documentary requirements, I am made to do the following:

a. To submit A CERTIFICATION from my employer issued in my favor, submitted as an attachment to the said application;

b. More importantly, to declare under oath that at the time of my application, I am here in the Philippines;

5. I declare, in my honest belief and good faith, that I am in the Philippines at the time of my application to the Skills Assessment undertaken by VETASSESS before submitting a migration application to the Department of Immigration and Citizenship (DIAC) of the Australian Government;

6. I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal intent and purpose this may serve.
