ENROLMENT AGREEMENT

This enrolment agreement sets out the responsibilities of the student, parent/carer and the school staff about

the education of students enrolled at Cloncurry State School P-12.

Responsibility of student to:

� A7end school regularly, on 8me, ready to learn and take part in school ac8vi8es.� Act at all 8mes with respect and show tolerance towards other students and staff.� Work hard and comply with requests or direc8ons from the teacher and principal.� Abide by school rules, meet homework requirements and wear the correct school uniform.� Respect the school environment and not leave the school grounds without permission.

Responsibility of parents to:

� Support my child to abide by the school rules, meet homework requirements and wear the correct

school uniform.� Let the school know if there are any problems that may affect my child’s ability to learn.� Inform the school of any absence and of any medical condi8on that may impact on my child's learning.� Support the authority and discipline of the school enabling my child to achieve maturity, self-discipline

and self-control.� Treat school staff with respect and tolerance.� Abide by the school’s policy regarding access to school grounds before, during and a=er school hours.

� Communicate with the class teachers to support my child’s learning.

Responsibility of school to:

� Develop each individual student’s talent as fully as possible.� Inform parents and carers regularly about how their children are progressing.� Inform students, parent and carers about what the teachers aim to teach the students each term

and/or semester.� Teach effec8vely and to set the highest standards in work and behaviour.� Take reasonable steps to ensure the safety, happiness and self-confidence of all students.� Be open and welcoming at all reasonable 8mes and offer oppurtuni8es for parents and carers to

become involved in the school community.� Clearly ar8culate the school’s expecta8ons regarding the responsible behaviour plan for students

and the school’s dress code policy.� Ensure that the parent is aware of the school’s record-keeping policy including the crea8on of a

transfer note should the student enroll at another school.� Set, mark and monitor homework regularly in keeping with the school’s homework policy.� Contact parents/carers as soon as possible if the school is concerned about the child’s school

work, behaviour, a7endance or punctuality.� Deal with complaints in an open, fair and transparent manner.� Consult parents on any major issues affec8ng students.� Treat students and parents with respect and tolerance.

Introduction to the State School Consent Form for Cloncurry State School P-12

This letter is to inform you about how we will use your child’s personal information and student materials. It outlines:

What information we record

How we will use student materials created during your child’s enrolment.

Examples of personal information, which may be used and disclosed (subject to consent), include part of a person’s name,

image/photograph, voice-video recording or year level.

Your child’s student materials:

Are created by your child whether as an individual or part of a team

May identify each person who contributed to the creation

May represent Indigenous knowledge or culture.

Purpose of the consent It is the school’s usual practice to take photographs or record images of students and occasionally to publish limited personal information and student materials for the purpose of celebrating student achievement and promoting the school and more broadly celebrating Queensland education.

To achieve this, the school may use newsletters, its website, traditional media, social media or other new media as listed in the ‘Media Sources’ section below.

The State School Consent Form may, at your discretion, provide consent for personal information and a licence for the student materials to be published online or in other public forums. It also allows your child’s personal information and student materials to be presented in part or alongside other students’ achievements.

The school needs to receive consent in writing before it uses or discloses your child’s personal information or student materials in a public forum. The attached form is a record of the consent provided.

It should be noted that in some instances the school may be required by the Education (General Provisions) Act 2006 or by laws to record, use or disclose the student’s personal information or materials without consent (e.g. assessment of student materials does not require further consent).

Voluntary There will not be any negative repercussions for not completing the State School Consent Form or for giving limited consent. All students will continue to receive their education regardless of whether consent is given or not.

Consent may be limited or withdrawn Consent may be limited or withdrawn at any time by you.

If you wish to limit or withdraw consent please notify the school in writing (by email or letter). The school will confirm the receipt of your request via email if you provide an email address.

If in doubt, the school may treat a notice to limit consent as a comprehensive withdrawal of consent until the limit is clarified to the school’s satisfaction.

Due to the nature of the internet and social media (which distributes and copies information), it may not be possible for all copies of information (including images of student materials) once published by consent, to be deleted or restricted from use.

The school may take down content that is under its direct control, however, published information and materials cannot be deleted and the school is under no obligation to communicate changes to consent with other entities/third parties.

CLONCURRY STATE SCHOOL P-12 For Secondary School Curriculum

Participation

The Student Resource Scheme (SRS) provides parents with a convenient and cost-effective way to access educational resources their children need for school. These resources supplement the school’s resources and enhance students’ learning experience at school.

At Cloncurry State School P-12 we have developed a very cost effective annual SRS charge for the whole of your child’s high school education that covers the resourcing charges for Years 7 to 12. Details will be provided annually of the breakdown of these costs and can be accessed on the school website: www.cloncurryss.eq.edu.au

So what does Cloncurry State School P-12 P&C do? The Cloncurry State School P-12 Parents and Citizens Association (P&C) is a great way to connect with other families, meet new people and play an active role in school life. We plan events and fundraising activities throughout the year to raise funds for much needed resources and encourage everyone to join our P&C association.

Events and fundraisers that our P&C are involved in include:

• Trivia Night • School Fete • Cent Sale • Naidoc cultural celebrations • Mother/Father Day stalls • Raffles • Supporting our Chaplaincy program • Student Art Exhibition

Our P&C association raise funds to purchase equipment and resources for our school.

• Shade structures. • Playground equipment. • Reading Resources • Sponsorship of the ICAS Competitions – Core subjects of English, Maths and Science • Sponsorship of the online literacy program - LiteracyPro • Providing financial support for excursions and activities for students

Volunteers Our volunteers make the world of difference and we cannot operate without them. You can get involved by volunteering at the tuckshop, manning a stand, helping with the BBQ or joining our executive committee that serves and governs the P&C. However you choose to get involved, volunteering is a great way to learn how the P&C runs and to contribute to the outcomes of all CSS P-12 students.

Committee Meetings

You are most welcome to come along and see how our School P&C operates even if your child hasn't started school. Meetings are open to everyone and new faces are always welcome. Meetings are held on the second Thursday of every month (except during school holidays) and begin at 3.00pm in the Inclusive Learning Centre. Please enter via the Administration Office.

School Uniforms

The P&C also run the school uniform shop on nominated days to enable parents to purchase uniforms required for school and sports. So as you can see, we are a busy group of people who are always appreciative of new faces and new ideas. If you would like more information or would like to volunteer for any of our planned events please drop your details into the Administration office.

At Cloncurry State School P-12, the Parents and Citizens Association believe that wearing the school uniform

correctly assists students to identify with the school and to feel positive about their learning environment. We value

neat and tidy personal presentation which prepares students for the workforce and develops pride in being part of a team. Our uniform shop hours are Monday - Friday 8am - 9.30am.

For your convenience uniforms are also available at local outlet Bohemian Dream in Ramsay Street. Bohemian Dream shop hours are Tuesday - Friday 10am - 5pm and Saturday 9.30am - 12.30pm.

All Students - Prep to Year 12SUMMER UNIFORM• P - 12 shirt, Prep - Year 10 Gold/Grey/Black, Year 11 - 12 Senior Grey/Gold/Black.• Plain black shorts, skirts or skorts of a conservative length only, preferred style includes school logo on leg. Long black dress pants or black jeans, (Leggings/tights are NOT permissible.)• Fully enclosed running style footwear with a thick rubber sole and either laces or velcro to enable the shoe to be securely fastened to the foot.• Black leather shoes are acceptable.• Plain white or black socks. (No logos.)• Full brimmed hat (black is the preferred colour. School hats are available from the office)• Wearing winter clothes on summer days is NOT permissible. (Sweatshirts and hoodies)JEWELLERY• Sleepers and studs (as earrings) – plain gold or silver. No other facial jewellery permitted re:safety.• Watch.• Necklaces, bracelets, rings, anklets are NOT permissible.• Costume jewellery and head wear is NOT appropriate.WINTER UNIFORM• The school tracksuit jacket and pants are permitted when weather is cold. Black is the preferred colour.• Leggings/tights may be worn underneath shorts, skirts or skorts, not permissible as an individual item of clothing.• Senior Students are permitted to wear their Senior Jacket.

FREE-DRESS DAYS Clothing that carries offensive designs is unacceptable. Outfits which bare midriffs or revealing or inappropriate clothing must not be worn. Safety procedures cannot be compromised and the wearing of thongs, slip on sandals and singlets are not permitted.

INTER-HOUSE SPORTS DAYS Students may wear suitable clothing in house colours: BURKE – Red WILLS – Green The Cloncurry State School P-12 community expects students enrolled at the school to wear the school uniform. Parent/Caregiver cooperation and support is sought to ensure that our school uniform is worn AS SPECIFIED WITHOUT ALTERATIONS. It is NOT acceptable to wear a mix of uniform pieces.

Cloncurry State School P – 12

Attendance Dear Parents/Caregivers,

Hooray! Every Day! Is our initiative to highlight the importance of children attending school every day to get the best results possible with their education.

Every day, teachers report who is absent to the office. If the school has not been notified by parents/guardian9am, a phone call will or text will be made to ascertain the reason of absence. If contact cannot be made, the school will investigate other follow up including a home visit to discuss how we can work together to ensure Hooray! Every Day!

P - 3 currently have 2 incentive programmes.

- The weekly incentive attendance is based on 100% attendance Monday - Friday each week and is rewarded the following Monday with free play time and an icy pole.

- The term incentive attendance rate is 95% (which is no more than 3 days absence per term). This is usually a lunch and movie afternoon, rewarded in week 2 of the following term. The overall annual attendance awards for grades Prep to 10 are celebrated in the last week of term 4, where a certificate and prize draw is held.

Year 4 - Year 12 currently have their attendance rewards embedded into the Positive Behaviour for Learning rewards system. These can range from a movie and lunch session to a PBL rewards afternoon, where a range of fun activities take place.

Great attendance will be celebrated!

Note: The Principal must be notified and give approval to be considered for the reward. Family holidays are not considered.
