3 Special samples collected on 06/30/00 and driven to thelab. MDS reported to SAC on 07/01/00 that all sampleswere all absent for Total Coliforms and E-Coli. SACnotified the Health Unit of the results and a Boil WaterAdvisory was not issued.
MOE Role Ministry staff contacted the PUC, Municipality and HealthUnit to advise of the problem on the eve of a longweekend. PUC was requested to sample immediately anddrive the samples to the lab. Asked to commence flushingof the system and increase chlorine residuals. Health Unitwas advised that sufficient chlorine residual was present inthe distribution system when the original samples werecollected (0.32 and 0.42 mg/L free chlorine). Inaccordance with ODWO MOH did not issue Boil WaterAdvisory on the basis of the single sample result (withadequate disinfection) and requested notification of the re-sample result. SAC played a key role over the weekendacting in a coordination role for notification and responseprocedures to the affect parties.

An earlier adverse result initiated in the building being posted. MOE was not copied on these results. Treatment in the formof filtration followed by U.V. light is present however the filterswere clogged and the light required cleaning. Furthermaintenance of the system is required. New UV lamp has nowbeen installed and additional filtration equipment is beingadded to the system.

• A water sample taken June 2, 2000, in the distributionsystem of the Carriage Subdivision Water Works showeda E. Coli count of <1, Total Coliform of 57 andBackground of >2000.
• June 22, 2000, analysis showed the following:4 Deteriorating water qualitiesDucharme Distribution System: two samples from thesame tap showed the following background: #1 110, #2>2000.Carriage Well: backgrounds - raw 700, treated 620 andtwo samples from the same tap - #1 510, #2 <1.
Actions • MOE contacted the Village of Bayfield for a briefing onthis situation. We were informed that the distributionsystem was flushed. A chlorine residual of 0.2 ppm wasdetected. The chlorine dosage was increased. Thesystem was resampled on the morning of June 6, 2000.
• Spoke to Works Foreman on morning of June 26, 2000. He informed me that free chlorine residual is at 0.2 mg/l. He will resample June 26, 2000. He is concerned withlab results on duplicate samples.

The chlorinedosage was increased and the site was resampled on June 2. At that time the samples came back with no fecal or e-coli.

OCWA increased free chlorine at all well heads to 0.8 mg/lfree chlorine from 0.2 mg/l. OCWA also began to overflow thewater tower ( i.e. purge out of system) to bring the highlychlorinated water into the tower. Will continue until tower haselevated free chlorine. Will then use this water to flushdistribution system.
S. Burns contacted LGLD Health unit to provide notification,background, and relay status of OCWA actions. RequestedMOH make decision regarding boil water advisory. Put HealthUnit staff in contact with OCWA operations staff.
MOE liaised with LGLDU and OCWA on corrective actionsand reviewed the results of the Special Samples to ensure thesafety of the water supply.
MOH, Dr. Gardiner in consultation with his staff & Mayordecided a boil water advisory was not required. Rationale wasthat the result was a single sample, no other adverse samplesfor number of years, chlorine residual was present in system,and OCWA initiated corrective actions and special samples. S.Burns informed of this decision on Sunday morning.
OCWA initiated corrective actions that included raising freechlorine from 0.2 to 0.8 mg/l. Operators overflowed the watertower to flush it and bring in higher chlorine water. Onceelevated chlorine levels present, flushing of distribution systemwas initiated. Repeat samples were collected Saturday June10, 2000 and driven to Accutest in Nepean for immediateanalysis. Further sampling was conducted on Sunday June11 and Monday June 12.
The samples collected on June 10 and June 11 confirmed thatbacterial contamination was absent in both the raw andtreated water. Corrective actions were ended June 13following discussions between the LGLD Health Unit, MOEand OCWA. Results tend to conf irm that the single adverseresult was a “bad sample” rather than a problem with the watersupply.

Mississippi Mills (Almonte) system has had excellentbacteriological quality over the years . Records reviewedduring the June 7, 2000 inspection showed no Coliform or E-coli in raw or treated samples for the 2 year period reviewed.

The MOE is not aware of a long history of problems at thislocation. We have been advised by the Health Unit that thetrailer park had a series of adverse sample results in Apr.2000 due to a malfunctioning chlorinator. The chlorinator wassubsequently repaired and water quality remained acceptableuntil June 2000. The Health Inspector, upon investigation, determined that thechlorine concentration in the distribution system was to low. Furthermore, the test kit in use for determining chlorineresidual at the trailer park was inadequate. This problem has been resolved with acquisition of newequipment. Boil water order was removed on June 13, 2000.

OCWA is replacing all the valves and fittings at thissampling location and resampling. This will be completedMonday June 19th.Public Health Inspector is aware of the situation.

A resample showed 0/0 bacteria. Apparentlythe previous sample was collected from an internal tap in oneof the processing barns. This conf irms that the problem isinternal rather than related to the production wells. It isunknown if the facility closure order has been lifted.
MOE Role MOE is involved only in an advisory sense as OMAFRA andthe Health Department are managing the issue.
MOE will continue to work with Medical Officer of Health toprovide advice as required.

OnJune 7, 2000, MOE retested the wells and the lab confirmed,on June 9, 2000, the presence of E coli in TW3 but no coliformin the gatehouse well.

Sampling was undertaken as a due diligence reaction relatedto the PTTW application and the potential for contaminationfrom a leachate plume associated with the Aurora Landfill.

The consultant reported elevated coliform levels were found inthe Gatehouse Well [16 counts/100 mL.] which were reportedto the Ministry in a consultant’s report related to a PTTWapplication for wells TW3 and TW4. Upon review of the reportby the Ministry and discovery of coliform bacteria, MOE didconfirmatory sampling on June 7, 2000, and the follow-upsample showed no coliform in the Gatehouse well, but E coliwas present in TW3; this was reported by the lab on June 9,2000. The consultant also reported low levels of benzene andchlorinated solvents were found in the Gatehouse Well. MOEis waiting for the results of its volatile sampling program todetermine further action related to the non-bacterialcomponents in the well. These contaminants are historical andare known to the Ministry as a by-product of the landfillsituated adjacent to the wells.

MOE resampled the TW3 well on June 7, 2000 and resultswere received June 9, 2000. They showed levels of 14counts/100 mL. E. coli and >150 counts/100 mL. of totalColiform. On June 9, 2000, staf f of Beacon Hall werecontacted by the Health Unit and advised to post a signprohibiting drinking the water. MOE and York Health Dept.attended the scene on June 12, 2000 and advised the ownerthat resampling was necessary to confirm bacti levels in thewell(s). No information is available as yet re. whether the newsamples were collected or what the results might be. Staff willfollow up to confirm information.
MOE Role MOE will continue to assist the Health Department by:confirming uses of the well(s) as TW3 can be used as adrinking water source and the Gatehouse well is reportedlyused only to service two toilets. TW4 is used only for irrigation.MOE will confirm if resampling has been undertaken and willconfirm if signs have been placed over the taps in thewashrooms to restrict potential use as a drinking water source. The construction and the maintenance of the three wells will bechecked to determine if bacteria could enter due to poormaintenance. Staff will conf irm if a strainer is on the lines as ifthis may have impacted the water sample results. MOE willkeep MOH advised of what is happening.

This issue relates to an unpermitted water taking and arosedue to the proponent attempting to re-establish their PTTWs. Information provided to the Ministry in the recent past wasmisleading with respect to the use of the wells and onlyrecently did we confirm that there is a potential that TW3 maybe used a drinking water source. MOE is reviewing the permitapplication submitted by Beacon Hall Golf Course whichincludes the TW3 and TW4 wells. MOE is discussing the issueof the contaminant plume from the landfill site with WMI, whoowns the adjacent landfill, to ensure that the plume is notenhanced by Beacon Hall’s water taking.

Why was sampling initiated? As part of a tunnel construction dewatering project, forwhich a PTTW was issued, the consultant undertooksampling of private wells to determine backgroundconditions prior to construction.
Sample contaminant details Six wells were sampled. Of these, two showed StandardPlate Counts of >500 CFU/mL, with a range of 1550 to55, 000 for the exceedences and 23 to 55,000 overall. Standard Plate Counts are an indicator of deterioratingwater conditions and further sampling needs to be doneto confirm any health implications with respect tobacteria. Three wells had nitrate levels of >10mg/L(ODWO), with a range of 12.8 to 43.8 for theexceedences and 0.4 to 43.8 overall. No further samplingresults are expected.
Resampling details and results Resampling has not been undertaken to date.
MOE Role MOE confirmed with the Director and the Manager of theHealth Services Division of the York Region HealthServices Department on June 13, 2000 that they will actas the lead in advising the homeowners of the need toresample the wells to confirm the presence of E. coli orcoliform bacteria and, pending the results of thesampling, provide advice as to how to disinfect the wellsto address bacteria found. They will also advise theresidents about the potential impacts associated withelevated nitrate levels. MOE will continue to work with Medical Officer of Healthto provide advice as required. As well MOE isreassessing the overall situation and the conditions ofthe dewatering permit (sought for the tunnel construction)to ensure that dewatering operations will not aggravatethe problem in the residential wells.
