OutcomeOverall, some improvement in function measured at several months.

“Miracle” recoveries occur, but most are likely clots that break up on their own, or TIAs, or mimics.

And some patients have devastating intracranial hemorrhage after tPA.

We’ve been treating stroke as a single entity, with limited knowledge, averaging results from multiple study patients.

Four Lessons

The Window is going – out the window.

Sure, time is brain, but in many patients there is collateral flow and viable ischemic brain MANY hours after the “window” closes.

Unfortunately, there are also patients with large hemispheric infarction an hour after symptom onset (who bleed if lysed – explaining some of the patients on the NINDS graph).

They are the strokes that are best treated with MT (mechanical thrombectomy) using the latest stent-retrievers by Neuro Interventional Radiologists (NIR).

• They MAY be helpful in selecting patients more likely to have ELVO strokes.

security in patients who actually have large vessel occlusion.

was based on averages of patients in studies.

• It turns out that brains have variablecollateral vessels.

• So the SAME clot in the SAME place may cause very DIFFERENT infarct/ischemic patterns in two different patients
