Warm, inviting place where friendships are made Engagement in meaningful activities of their own choice Person-centered Care Service including respite for

caregivers Supervision Assistance with personal care or other daily living

Services include: Social and recreational activities Daily exercise program Crafts and gardening Reminiscence groups Personal care assistance Entertainment and guest speakers Information and referrals Nutritious hot meals (Brea location)

PROGRAM IS FOR: ADULTS WITH EARLY MEMORY LOSS OR PHYSICAL

DISABILITIES 50 YEARS OR OLDER (BREA) OR 60 YEARS OR

OLDER (REDONDO BEACH) AMBULATOR OR USE OF MOBILITY AIDS

PROGRAM IS UNTIL: No longer interested in receiving services No longer need this level of intervention No longer a danger to selves or others

BOTH INDIVIDUAL AND SMALL GROUP MAXIMUM RATIO OF 1 STAFF TO 6 PARTICIPANTS *$65/PARTIAL DAY (UP TO 5 HOURS) *$85/FULL DAY (5 TO 8 HOURS) $95/EXTENDED DAY (8 HOURS +) HOURLY RATES AVAILABLE AT REDONDO BEACH

LOCATION*Adult day services generally cost between $40 and $100 a day depending on one's geographic location. This is less than the cost of nursing home, assisted living or in-home care, but is not covered by Medicaid.

HOW DOES ONE PAY FOR THIS TYPE OF SERVICE?

Many participants receive funding through state and local agencies that cover all or a portion the cost of adult day services. Although the funding available varies in each state, most offer Medicaid waivers for individuals with specific needs and financial support through the Administrations on Aging and Veterans' Administrations. Some participants also pay through long-term care insurance or private pay.

RESIDENTIAL SERVICES PROVIDES INTIMATE HOME SETTING FOR INDIVIDUALS WHO HAVE LIVED IN STATE

DEVELOPMENTAL CENTERS MOST OF THEIR LIVES PERSON-CENTERED FOCUS ON OPPORTUNITIES, TOOLS & ACTIVITIES MADE BY

CHOICE INDIVIDUALS LIVE IN A HOME WITH 24/7/365 SUPPORT INCLUDING MEDICAL,

THERAPEUTIC, NUTRITIONAL, RECREATIONAL AND BEHAVIORAL PROFESSIONALS
