Adding Survey and Results Adding a survey to your site is a great way to collect data from you students. Surveys can be used as quizzes and can provide instant feedback on how your students are progressing toward a lesson’s learning objective. Below are the directions for embedding a survey onto your site. (Remember: before you can embed the survey, you must first create on in Google Docs).
Adding a Survey 1. Logon to Google Sites
2. Select the Site that you want to add a Survey to.
3. Go to the page that you want to add a Survey to.

4. Click on the ‘Edit’ icon in the upper right-‐hand corner.
5. Go to “Insert” in the toolbar and select “Spreadsheet Form”
6. Click on the spreadsheet form that you want to add and click “Select”
7. Specify display options (optional) and click “Save”

8. Choose alignment and text wrap options (optional)
9. Click “Save”
