GENERAL

A. The minutes of the pre-bid conference and sign-in sheet, Attached, are hereby made a part of the Contract Documents.

CONTRACTOR QUESTIONS/COMMENTS A. Questions/Comments:

On Page 3 of 6, Note 5 in the recommended repair method, it calls to place an epoxy bonding agent after the forms are installed. This cannot be done as the form would prohibit applying the epoxy agent and it cannot be done before the form is placed as the epoxy would not be tacky when placing the concrete. In pile jackets this is not normally done to add a bonding agent. Can we eliminate the bonding agent? The surface will be roughened. Reply: The bonding compound can be omitted, as long as the manufacturer’s requirements are strictly adhered to.

B. Questions/Comments:

The specs call for a concrete mix to be used for the jackets. Will a 4,500 psi grout be acceptable? The grout will provide a better flow in small areas. Reply: The concrete encasement is intended to be a permanent replacement for the deteriorated pile. Structural concrete is required within the encasement, therefore grout will not be acceptable.

BEL LIDO DRIVE BRIDGE REHABILITATION

PRE-BID CONFERENCE MEETING MINUTES TOWN BID NO. 2018-01

(10:00 AM; November 15, 2017)

1. Purpose of Pre-Bid Meeting:

This pre-bid meeting has been scheduled to acquaint the prime contractors and others with the bid documents for the Bel Lido Drive Bridge Rehabilitation Project. Mathews Consulting (MC), a Baxter & Woodman Company, is the Engineer of Record for the project. The Town of Highland Beach is the Owner. Mathews Consulting will provide construction management and part-time inspection services for the Project.

2. Scope of Project:

The work generally consists of the following: Furnishing all labor, materials, equipment, incidentals and appurtenances for the rehabilitation of the Bel Lido Drive Bridge which is located within the Town’s rights-of-way. Construction also includes the replacement of the existing guardrail systems at the east and west bridge approach, MOT's, environmental protection, concrete pavement restoration, and all other restoration work required for a complete and operable system.

3. Permits:

The project is exempt from the Florida Department of Environmental Protection (FDEP) Environmental Resource Permitting (ERP) requirements.

The following permit(s) will need to be secured by the Contractor prior to mobilizing on-site:

Town of Highland Beach Building Department Permit – Contractor to pay permit fee. – Town indicated that Town permit fees will be waived.

The Contractor will be required to coordinate all periodic inspections required through the Town’s Building Department

4. Project Schedule:

Sealed bids (one original and two copies) will be received in the Office of the Town Clerk, Town of Highland Beach, 3614 S. Ocean Boulevard, Highland Beach, Florida 33487 until 10:00 a.m. on December 6, 2017, for a completed project for the Town of Highland Beach Bel Lido Drive Bridge Rehabilitation. All bids received will be opened

and read at the close of bidding in the Commission Chambers of Town Hall. All bidders or their representatives are invited to be present. The work to be performed under this contract as further described in Section 01010 “Summary of Work” shall be substantially complete within 90 Calendar Days from notice to proceed. Final completion of the entire project shall be complete within 120 Calendar Days from notice to proceed.

5. Bid Security:

A Bid Bond, Certified Check, or Cashiers Check in the amount of 5% of the total base bid price must be submitted with the bid. The bidders must utilize the Bid Bond form provided in the Contract Documents. The Bidder shall guarantee the Total Bid Price for a period of 120 Calendar Days from the date of bid opening.

6. Contractor Qualifications

Bidder must fill out Bidders General Information and Supplemental Contractor Questionnaire included in the Bid Proposal. If this detailed information is not provided at time of Bid, the bid may be considered non-responsive.

7. Sub-Contractors

No more than 40% of dollar value of the total contract work may be accomplished by subcontractors. The bid proposal requires that sub-contractors be identified. Detailed information is required for each sub-contractor.

8. Examination of Site:

Each bidder is responsible for necessary site investigations to determine all existing above ground conditions, sub-surface conditions, in-water conditions and the facilities needed to execute the work in accordance with the Contract documents. It is recommended that potential bidders inspect the project site before preparation/submittal of their bid package.

9. Interpretation of Addenda:

All questions about the meaning or intent of the Contract Documents are to be directed to the Engineer in writing. Interpretations or clarifications considered necessary by the Engineer in response to such questions will be issued by Addenda via email or delivered to all parties recorded by the Engineer or Owner as having received the Contract Documents. Addenda will also be published on DemandStar. Only questions answered by formal written addenda will be binding. Therefore, all questions need to be submitted no later than Tuesday, November 28, 2017. Any questions received after this date will not be answered. 10. Bid Proposal:

Contract Price is based on unit price total of Base Bid items 1-15. Each pay item is defined in Specification Section 01025 “Measurement and Payment”. Payment item for mobilization shall not exceed five percent (5%) of the base bid contract price and Payment item for bonds and insurance shall not exceed two percent (2%) of the base bid contract price.

11. Existing Utilities:

Contractor is fully responsible for coordination of all utility companies (e.g. FPL, AT&T, Gas, Comcast, Town of Highland Beach, etc.) and for repairing, at no expense to the Owner, utilities damaged during construction. This includes pot-holing as necessary.

12. Key Concerns:

Overall Working Hours for the Town are from 8:00 AM to 6:00 PM, Monday – Friday (from 5:00 pm to 6:00 pm, the Contractor will be required to only focus on cleanup, safety & MOT removal). No work is allowed on Town Holidays or Weekends without the Town’s written approval.

Submit a Maintenance of Traffic Plan (MOT) at the Preconstruction Meeting for any work which will occur within the Town’s or FDOT rights-of-way. The MOT Plan(s) will need to be approved by both the Town and FDOT, as applicable. – The MOT Plan shall be developed to ensure that all residents have a means of safe ingress and egress at all times. In the event of lane closures the MOT shall include trained flagman to alternate traffic over the bridge. No overnight lane closures shall be permitted unless appropriate devices including temporary signal devices and sign boards are in place and operational.

Contractor shall be required to utilize construction methods and devices, such as

turbidity screens, curtains and floating silt barriers, where necessary, in order to comply with all State and Local Water Quality Standards.

Traffic is to be maintained at all times. Access to private properties shall be

maintained at all times. Duration of lane closures would need to be approved and coordinated with Town.

It is the Contractor’s Responsibility to obtain Staging Areas for Storage of Materials

utilized on this project. The Contractor will need to comply with the Town Ordinances in regards to Staging Area Requirements. The Town will not provide areas for Staging Materials.

Any agreements made with Private Property Owners to secure a staging area will

need to be provided to the Town and Engineer in writing. The Restoration of the Staging Area Location will be the Contractor’s Responsibility to meet the satisfaction of the associated Property Owner.

All areas and/or existing utilities and infrastructure disturbed by construction

activities shall be restored in kind.

Updated as-built record drawings shall be submitted by Contractor each month with payment request. - A professional land surveyor will NOT be required for the preparation of record drawings. The Contractor shall prepare, submit and certify accuracy of the periodic and final project record drawings. Dimensional information shall be included on the project record drawings. Since a PLS will not be required, all dimensions shall be from fixed points not from a known datum. All other requirements of Section 01720 entitled “Project Record Drawings” shall remain in effect.

Contractor to provide independent testing laboratory for specified testing. The testing

laboratory will have to be approved by Town and Engineer. 13. Contractor’s Questions:

A. Questions/Comments: Is there an Engineer’s Estimate available for the project?

Reply: The Engineer’s Estimate for the project is $75,000.

B. Questions/Comments:

What extents are required for the pre-construction video?

Reply: The minimum extents of the pre-construction video shall be from the west right-of-way of A-1-A to the east and the intersection of Bel Lido Drive and Tranquility Drive to the west. If the Contractor anticipates work beyond these areas or intends to use a remote staging area, these extents shall be modified as required to encompass all potential work areas.
