Part (1) GENERAL INSTRUCTIONS

1.1 Background

On October 4, 1988 Hillsborough County voters approved Ordinance 88-28 creating the Children’s Board of Hillsborough County (CBHC). The local referendum established an organization dedicated to improving supports and services to our county’s children and families. Thus, the Children’s Board was established in 1989 as a Special Taxing District allowing a levy not to exceed 50 cents per $1,000 of the assessed property tax. The Children’s Board was the culmination of many years of research, advocacy and communication efforts by the Hillsborough Constituency for Children and other community leaders who wanted dedicated funding resources to establish an organization to identify, address and respond to issues and opportunities to support the needs of Hillsborough County children and families. The governing Board is comprised of ten members including the Superintendent of Schools, a School Board member, a County Commissioner, a Judge of the 13th Judicial Court, the Regional Administrator - Department of Children & Families and five (5) gubernatorial appointees who establish Board Policies, hire the Executive Director and approve funding to support community services, programs, initiatives and internal operations. 1990-1991 CBHC releases approximately three million dollars in funding through a Request For Proposals (RFP) for the first time using nine of the thirteen need/issue areas identified in the Hillsborough County Needs Assessment (HCNA) as the platform/funding plan. These priority areas were seen by the board as the most urgent children’s needs in Hillsborough County. The Need Areas were Child Care, Families with Special Needs, Family Violence, Financial Hardship, Maternal and Other Preventive Health Care, Mental Health, and Substance Abuse. Approximately seventy programs were funded in this initial funding cycle. Priority was given to programs primarily serving families with children ages 0-12, and children and families residing in high-risk environments. For additional CBHC historical milestones and information please refer to Attachment (3) – CBHC Highlights and Timelines. Project Overview CBHC is requesting proposals for production of two (2) videos:

1) A Video #1 celebrating the 30th Anniversary of our organization with interviews of historical figures associated with the organization; and

2) A Video #2 for general audiences providing a narrated voice over to include testimonials, historical information, content and impact of CBHC investments in Hillsborough County.

We seek a qualified individual or firm with the creativity, experience and resources to produce high quality video products within negotiated deadlines. Estimated Budget: Up to $25,000.

1.3 Definitions

1. “Addendum” means supplemental information and/or changes made to this RFP after release of the original procurement document and before deadline for submission.

2. “Children’s Board” or “CBHC” means the Children’s Board of Hillsborough County, a special district political subdivision of the state of Florida. The Children’s Board of Hillsborough County is not a corporation. All records submitted to the Children’s Board become public records under Chapter 119, Florida Statutes, unless exempted by state law. The Children’s Board is also subject to the Florida’s Sunshine Law, see Chapter 286, Florida Statutes. The Children’s Board is not subject to Chapter 120, Florida Statutes.

3. “Contractor” or means the successful Proposer awarded the contract.

“Firm” means any individual, agency, partnership, corporation, or other entity experienced and capable of providing professional services to develop 30th Anniversary video products.

4. “General Instructions” means the content of this RFP, Part 1.

5. “MANDATORY Pre-Proposal Conference” is a required pre-proposal submission meeting for any firm or individual interested in submitting a proposal in response to this Request for Proposal. Proposals will only be accepted from firms or individuals who attend this meeting.

6. “Notice of Intent to Award” means the selection and notice that the Children’s Board intends to enter into a contract with the Independent Contractor. The selection and notice does not create any contractual obligation or liability for the Children’s Board until a contract is signed by both parties. The Children’s Board reserves the right to reject any or all Proposals.

7. “Pre-Proposal Conference” means a conference held to review the RFP before proposals are submitted.

8. “Project Manager” means Paula Scott, Director of Public Relations.

9. “Proposal” means the statement of qualifications and proposal submitted by a person or legal entity in response to this RFP and includes any written clarifications submitted by the Proposer in response to a written request from the Project Manager.

10. “Proposal Submission” is the process and final submission of proposal to the Children’s Board by Proposer.

11. “Proposer” means the individual or firm who submits or intends to submit a response to the CBHC pursuant to this procurement document.

12. “Public Entity Crimes“ refers to Section 287.133(3)(a), Florida Statutes and the convicted vendor list of the Florida Department of Management Services.

13. “RFP” or “Request for Proposals” means this request for proposals and includes any addendum and the answers to Proposers’ questions.

14. “Services” means video production and related services described in Section 2.2, Scope of Services.

1.4 Equal Opportunity and Nondiscrimination

The Children’s Board encourages the participation of minority/women business enterprises (M/WBE) in all contracts. No person or legal entity will be excluded from participation in, denied the benefits of, or otherwise discriminated against in connection with the award and performance of any procurement by the Children’s Board on the basis of race, color, religion, national origin, age, sex, disability, sexual orientation or marital status.

1.5 No Oral Interpretations

Interpretations, explanations, corrections and changes in this Request for Proposals will be made only by written answers to Proposers’ questions submitted prior to the deadline for such questions and/or by addendum. Interpretations, explanations, corrections and changes of the Request for Proposals made in any other manner or made orally by Children’s Board staff will not be binding on the Children’s Board and Proposers shall not rely upon them.

1.6 Questions from Proposers and Children’s Board’s Answers

Answers to Proposers’ written questions will be furnished in writing and posted on the CBHC website and www.demandstar.com website. Should any questions or response require revisions to the Request for Proposals, such revisions will be by addendum only and will be posted on CBHC website. See Section 1.2 Timeline and Summary for how to email questions.

1.7 Addendum and Incomplete Proposals

Proposers are responsible for complying with any addendum issued. FAILURE TO RESPOND TO ANY ITEM, INCLUDING ANY REQUESTED INFORMATION, OR FAILURE TO FOLLOW THE RFP, MAY RESULT IN THE SUBMISSION OF AN INCOMPLETE PROPOSAL AND MAY RESULT IN DISQUALIFICATION OF THE PROPOSAL FROM FURTHER CONSIDERATION.

1.8 Assignment/Subcontracting/Corporate Acquisitions and/or Mergers

The Contractor shall perform this contract. No assignment or subcontracting shall be allowed without prior written consent of CBHC. If a Proposer intends to subcontract or outsource work, the Proposer must disclose that intent in the Proposal.

In the event of a corporate acquisition and/or merger, the Contractor shall provide written notice to CBHC within thirty (30) business days of its notice of such action. Action by CBHC awarding a contract to a firm or individual that has disclosed its intent to assign or subcontract in its response to the RFP, without exception, shall constitute approval of such for purpose of this Agreement.

1.9 Right to Seek and Consider Clarifying Information

The Children’s Board may seek clarifying information regarding any Proposal. Such clarifying information shall be provided by the Proposer in writing. Such clarifying information may not modify any material portion of the Proposal, affect the price, or give one Proposer an advantage not afforded by the other Proposers.

1.10 Rejection of Proposals or Proposers

Children's Board reserves the right to reject all Proposals with or without cause and to waive technicalities or informalities. Cost of preparing a Proposal is an operational cost of the Proposer and shall not be passed on to or borne by CBHC.

Any Proposal determined to be non‐responsive to any specification or requirement of this Proposal, including instructions governing submissions of Proposals, may be disqualified without evaluation.

Proposers who violate the provisions of this RFP may be rejected from the selection process.

1.11 Restricted Communications

From November 21, 2017 until December 27, 2017, Proposers may not communicate regarding this RFP or any Proposer’s qualifications with Children’s Board personnel and/or Board members, other than Paula Scott. Firms or individuals that engage in prohibited communications with Children’s Board personnel or Board members may be rejected from the process.

1.12 Written Reference Letters

Proposals shall include written reference letters from at least three organizations having employed the submitting firm specifically for the intent of video production similar in scope and content of this RFP.

1.13 Contract

By submitting a Proposal, the Proposer agrees that, if the Children’s Board selects such Proposer to perform the Services, the Proposer will, upon such selection, execute a contract in substantially the same form of the contract attached hereto as Attachment (1). Execution of the contract will be contingent on availability of funds as determined in the sole discretion of Children’s Board.

1.14 Award of Contract

The contract for the Services will be awarded based on the criteria in this RFP. No contract shall exist between the Children’s Board and the Contractor until the written contract is signed by the Children’s Board.

1.15 Prohibitions on Use of Former Employees and Board Members

The Children’s Board of Hillsborough County will not award a consulting or services contract for a period of two years from the date any Children’s Board employee or Board member has ceased his/her employment or Board member duties with CBHC.

1.16 Public Entity Crimes

Pursuant to Section 287.133(3)(a), Florida Statutes:

A person or affiliate who has been placed on the Convicted Vendor List following a conviction for a public entity crime may not submit a bid, proposal, or reply on a contract to provide any goods or services to a public entity; may not submit a bid, proposal, or reply on a contract with a public entity for the construction or repair of a public building or public work; may not submit bids, proposals, or replies on leases of real property to a public entity; may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity; and may not transact business with any public entity in excess of the threshold amount provided in s. 287.017 for CATEGORY TWO for a period of thirty-six months following the date of being placed on the convicted vendor list.

1.17 Public Records

1.17.1 Proposals Are Property of the Children’s Board

All Proposals become the property of the Children’s Board and will be a public record subject to the provisions of Chapter (119), Florida Statutes.

1.17.2 Proposals Are Subject to Public Inspection

Unless made confidential or exempted by law, all Proposals are subject to public inspection and replication under Florida’s Public Records Law, Chapter (119), Florida Statutes.

1.18 Qualified Proposer

The Children’s Board shall determine whether the Proposer to be selected is fully qualified to render the required Services. Among the factors to be considered in making this finding are the capabilities, adequacy of personnel, past record, and experience of the Firm or individual.

1.19 Conflict of Interest

The Proposer represents that it presently has no interest and shall acquire no interest, either direct or indirect, which would conflict in any manner with the performance or Services required hereunder, as provided for in Fla. Stat., §112.311, and following sections. The Proposer further represents that no person having any such conflict of interest shall be employed for the Services. The Proposer shall promptly notify the Children’s Board’s Project Manager, in writing by certified mail, of all potential conflicts of interest for any prospective business association, interest, or other circumstance, which may influence or appear to influence the Proposer's judgment or quality of Services being provided hereunder. Such written notification shall identify the prospective business association and interest or circumstance.

1.20 Compliance with all Laws

The laws of the State of Florida apply to any purchase made under this Request for Proposals. Proposers shall comply with all local, state, and federal directives, orders and laws as applicable to this Proposal and subsequent contract(s) including but not limited to Equal Employment Opportunity (EEO), Minority Business Enterprise (MBE), and OSHA as applicable.

1.21 Prohibition Against Contingent Fees

The Proposer warrants that it has not employed or retained any company or person, other than a bona fide employee working solely for the Proposer to solicit or secure the contract with the Children’s Board, and that it has not paid or agreed to pay any person, company, corporation, individual, or firm, other than a bona fide employee working solely for the Proposer, any fee, commission, percentage, gift or any other consideration, contingent upon or resulting from the award or making of this contract.

1.22 This section intentionally not used.

1.23 Joint Ventures

Proposers who are joint ventures must provide an affidavit attesting to the formulation of a joint venture and provide either proof of incorporation as a joint venture or a copy of the formal joint venture Agreement between all joint venture parties, indicating their respective roles, responsibilities and levels of participation for the project.

1.24 Term of Contract

The contract for services shall be for six months with the option for the Children’s Board to extend the contract. The Children’s Board may terminate the contract upon seven (7) days’ written notice to the Contractor without providing cause.

1.25 Multiple Awards

The Children’s Board reserves the right to make multiple awards pursuant to this RFP.

Part (2) SPECIFICATIONS

2.1 General

The minimum requirements and the specifications for the Services, as well as certain requests for information to be provided by Proposer as part of its Proposal, are set forth below. As indicated in Section (1.3) of this RFP, the successful Proposer is referred to as the “Contractor.”

2.2 Scope of Services

CBHC is requesting video production services to produce two videos. Conceptual requirements are as follow:

A. Video #1 – Longer video capturing and celebrating the creation and history of CBHC from 1988 – 2018.

The video must convey historical and important actions taken by Hillsborough County child advocates and community leaders to mobilize private and public support in an effort to convince Hillsborough County voters to approve the referendum to establish CBHC as well as unique events, milestones, initiatives, and people involved over the last 30 years. Refer to ATTACHMENT 3.

B. Video #2 - Short video for general audiences providing a narrated voice over

with a few testimonials, historical information, content and impact of CBHC

investments and in Hillsborough County.

This video will include a synopsis of our first thirty years and deliver information about CBHC funding and impact on Hillsborough county children, families, child welfare system(s), community leadership and advocacy initiatives.

C. Technical Requirements:

1. Video 1 - should be no more than 15 minutes in length. 2. Video 2 - should be no more than 5 minutes in length. 3. Both videos should include a collage of existing video footage or photographs of

CBHC events, activities, providers, and testimonials; as well as new footage filmed and/or photographed by the contractor.

4. The videos should include music, creative graphics and a voice over. 5. Contractor will provide copywriting for voice over. 6. Videos must be shot using standard HD with broadcast quality in multiple formats,

i.e. MP4, CD, USB and web enabled. 7. Video 1 is due on March 16, 2018 and Video 2 is due on May 30, 2018 or sooner.

D. Contractor Requirements:

The Contractor is expected to communicate with CBHC staff throughout all phases

of project execution, meeting as needed in person, by phone or email and providing

weekly written progress updates. The Contractor is expected to produce all

elements of the video, which may include, but are not limited to, scripting, voicing,

lighting, graphics, animation, editing, coding, and music.

The Contractor will be required to secure its own production facilities and equipment.

Any assignment or subcontracting by the selected Contractor for the work to be

performed or goods and/or services to be provided, in whole or in part, and any

other interest in conjunction with this request for proposal shall not be permitted

without the express written consent of CBHC.

Part (3) – PROPOSAL CONTENT AND SUBMISSION REQUIREMENTS

Proposals shall be organized and numbered as shown in Part (3).

Submit one (1) original paper copy (UNBOUND), five (5) paper copies and one (1) electronic copy on a USB Drive in a sealed envelope. If the original of the Proposal contains any color fonts, graphics, or objects, the copies must also be in color.

Proposals must be typewritten on 8 1/2'' by 11" paper with at least (1)-inch margins on all sides. Such typed portions shall have page numbers at the bottom of each page.

12-Point Times New Roman or Cambria Font is preferred.

The Contractor will deliver the finished product(s) using standard HD in the desired formats including: MP4, AVI, and Web-­‐compatible formats. The finished products will include two (2) broadcast quality videos that are also web-­‐compatible. Part 4 – EVALUATION CRITERIA

4.0 EVALUATION CRITERIA and POINTS Available.

After determining that a Proposal satisfies the requirements stated in the RFP, the comparative assessment of the relative benefits and deficiencies of the Proposal in relationship to the published evaluation criteria shall be made by a Proposal Review Committee.

Proposals will be reviewed and scored using the following criteria and assigned point values for each criterion up to the amount indicated for each criterion.

The award of a contract resulting from this RFP shall be determined by the Proposal receiving the highest point total based upon the evaluation criteria stated below:

INDEPENDENT CONTRACTOR AGREEMENT BETWEEN

CHILDREN’S BOARD OF HILLSBOROUGH COUNTY AND

THIS AGREEMENT is made as of the _____ day of _______ 2017, by and between the Children's Board of Hillsborough County (hereinafter the “Children’s Board”) and [vendor] whose address is: [city, state, zip code] and whose telephone number is ____________________ and e-mail is _____________________ (hereinafter referred to as the Independent Contractor). For the mutual promises contained herein, the parties agree as follows: A. TERM OF AGREEMENT

This Agreement shall be effective from [date] through [date] and Independent Contractor shall finish all Services by [date].

The Children’s Board may terminate this Agreement upon 30 days’ written notice to the

Contractor. If the Contractor breaches this Agreement, the Children’s Board may terminate this Agreement immediately upon written notice to the Contractor. If terminated early by the Children’s Board, Independent Contractor shall be paid for Services successfully performed to the date of the notice of termination.

B. SCOPE OF SERVICES

The term “Services” means the work to be performed by the Independent Contractor under this Agreement. The Independent Contractor shall perform the Services described below:

C. COMPENSATION

The Children's Board shall review and determine the end results of the Independent Contractor’s Services and shall pay compensation set forth below if the products and final performance are in accordance with this Agreement.

See Attachment B, which is attached hereto and incorporated into this Agreement by reference. The Children's Board’s Project Manager and representative for approval of the Services for

payment is [CBHC staff Director].

The Independent Contractor’s total compensation for performance of the Scope of Services is described in Attachment B.

Certain expenses may be reimbursed if the type of expenses and the amount or rate of

reimbursement are also described on Attachment E. The Children's Board may request additional information to verify performance.

When the Children's Board determines that the invoices are satisfactory, the Children's

Board shall pay the Independent Contractor within 30 days after submission of a complete invoice. The final invoice must be received by the Children’s Board no later than 45 days after the end date of the Agreement.

D. BENEFITS

The Independent Contractor is not an employee of the Children's Board and is not entitled to any employee benefits. The Independent Contractor shall not receive nor be entitled to:

Leave Insurance Workers’ compensation benefits Unemployment compensation benefits

Withholding of federal income taxes Social security taxes or similar taxes Employee travel expenses Pension plan benefits Retirement benefits

The Independent Contractor is not protected by the immunity granted to public employees under Section 768.28, Florida Statutes. The Independent Contractor shall be liable for her or his accidents or negligence (including automobile accidents) while performing this Agreement. The Independent Contractor is not entitled to Civil Service protection.

E. MANNER OF PERFORMANCE OF SERVICES

The Independent Contractor has complete and exclusive authority over the means and methods of performing the Services. The means and methods of performing the Services shall not be subject to the direction or control of the Children's Board.

The Independent Contractor may perform the Services according to her or his own schedule

at the offices of the Children's Board or at any other location. The Independent Contractor need not follow the Children's Board work policies or

procedures applicable to employees regarding manner or method of performing work. The Independent Contractor shall comply with Children's Board written and unwritten procedures regarding security, format of documents, use of equipment, use of facilities and record keeping, to ensure the Services and any work product are performed in a manner capable of being incorporated into the operations of the Children's Board.

The Children’s Board and the Independent Contractor shall assume toward one another a

relationship of good faith and fair dealing.

F. OTHER SERVICES BY INDEPENDENT CONTRACTOR

This is not an exclusive Agreement for the Services or the time of the Independent Contractor. The Independent Contractor may perform similar or dissimilar Services for other entities at any time.

G. ITEMS TO BE FURNISHED BY INDEPENDENT CONTRACTOR

The Independent Contractor shall supply the following items necessary for performance of Services:

H. ITEMS FURNISHED BY THE CHILDREN’S BOARD

The Children's Board shall furnish the Independent Contractor the following items necessary for performance of Services:

I. EXPENSES

The Children's Board shall reimburse the Independent Contractor the following expenses upon submission of the invoices paid by the Independent Contractor and such other documentation as the Children's Board may reasonably request:

J. LIMITATION ON AUTHORITY OF INDEPENDENT CONTRACTOR TO ACT AS A REPRESENTATIVE AND PUBLIC RECORDS

The Independent Contractor has no authority to speak as a spokesperson for nor to act or

represent herself or himself as an agent of the Children's Board except in the following circumstances:

See Attachment F, which is attached hereto and incorporated into this Agreement by reference. All records prepared by or received by the Independent Contractor in the performance of

this Agreement shall be considered public records when received by the Children's Board. If such records are not provided to the Children's Board and the Children's Board does not view such records, then those records shall not be deemed to be public records. However, both the Independent Contractor and the Children's Board shall comply with chapter 119, Florida Statutes, and any contrary provision of this Agreement shall be void.

K. LIMITATION ON AGREEMENT

The Independent Contractor may not assign nor subcontract any of its rights or obligations under this Agreement without the prior written consent of the Children's Board.

L. NO THIRD PARTY BENEFICIARIES

This Agreement is for the benefit of the Children's Board and the Independent Contractor. No third party is an intended beneficiary. No third party has any cause of action to enforce the terms of this Agreement or a cause of action for damages due to its breach. M. INDEMNIFICATION

The Independent Contractor agrees to be liable for and to indemnify the Children's Board against all claims, suits, judgments, or damages, including court costs and attorney’s fees, arising out of the negligent or intentional acts or omissions of the Independent Contractor, or arising out of the violation of any copyright law by the Independent Contractor, in the course of the performance of this Agreement. If the Independent Contractor and the Children's Board commit joint negligent acts, the Independent Contractor shall not be liable for nor have any obligation to defend the Children's Board with respect to that part of the joint negligent act committed by the Children's Board. In no event shall the Independent Contractor be liable for nor have any obligation to defend the Children's Board against such claims, suits, judgments, or damages, including costs and attorney’s fees, arising out of the sole negligent acts of the Children's Board.

N. LITIGATION COSTS AND EXPENSES

If any party institutes any legal suit, action, or proceeding against the other party arising out of or relating to this Agreement, the prevailing party in the suit, action, or proceeding shall be entitled to receive, and the non-prevailing party shall pay, in addition to all other remedies to which the prevailing party may be entitled, the costs and expenses incurred by the prevailing party in conducting or defending the suit, action, or proceeding, including any costs that are taxable pursuant to any applicable statute, rule, or guideline (including, but not limited to, the Statewide Uniform Guidelines for Taxation of Costs in Civil Actions), as well as costs not taxable thereunder and including all attorneys' fees and expenses, and court costs even if not recoverable by law including, without limitation, all fees, taxes, costs, and expenses incident to appellate, bankruptcy, reasonableness of the amount of attorneys’ fees and costs and post-judgment proceedings. The prevailing party shall be that party which shall have prevailed on a majority, but not necessarily all, of the material issues which were adjudicated in such proceeding. O. COPYRIGHTS The Children's Board shall own and has the exclusive right to copyright, patent, or trademark all original works created or produced by the Independent Contractor, its subcontractors, or consultants for performance of this Agreement (such materials are called Work Product in this paragraph). It is expressly agreed by the Independent Contractor that any Work Product for use as a contribution to a collective work, as a part of a motion picture or other audiovisual work, as a translation, as a supplementary work, as a compilation, as an instructional text, as a test, as answer material for a test, or as an atlas, that is created by the Independent Contractor shall be considered a work made for hire. It is expressly agreed by the Independent Contractor that all of its right, title and interest in and to Work Product that is not a work made for hire shall be assigned by Independent Contractor, its subcontractors or consultants to the Children's Board without further compensation. The

assignment shall be of all the Independent Contractor’s, sub-contractor’s or consultant’s right, title and interest in and to the Work Product in the United States and elsewhere. Upon request, Independent Contractor, its subcontractors or consultants will sign all applications, assignments, instruments and papers and perform all acts necessary or desired by the Children's Board to assign the Work Product, or any portion thereof, fully and completely to the Children's Board and to enable the Children's Board, its successors and assigns to secure and enjoy the full and exclusive benefits and advantages thereof. The Independent Contractor, its subcontractors, and consultants shall do all acts and sign such documents necessary or requested by the Children's Board to implement this paragraph O. The Independent Contractor shall require all contracts with its subcontractors or consultants to contain the requirements of this paragraph O. Any derivative works Independent Contractor, its subcontractors or consultants intends to create or has created for the performance of this Agreement are called Project Derivative Works in this paragraph and are listed on Attachment G which is attached hereto and incorporated into this Agreement by reference. Any work created by Independent Contractor, its subcontractors or consultants that is not listed on Attachment G shall be construed to be Work Product. The Children’s Board claims no rights in or to the derivative works listed on Attachment G. This paragraph shall survive termination of this Agreement. P. NO MODIFICATION UNLESS IN WRITING/AUTHORITY OF PROJECT MANAGER No modification of this Agreement shall be valid unless in writing and approved by the Children's Board and the Independent Contractor. The Children’s Board’s Project Manager shall have the authority to administer this Agreement on behalf of the Children’s Board and to issue written approvals and directions for the proper and efficient performance of this Agreement by the Independent Contractor. The Project Manager shall not have authority to amend this Agreement.

Q. VENUE AND LAW Venue for any action arising out of this Agreement shall be in Hillsborough County, Florida. This Agreement shall be construed and enforced in accordance with Florida law without regard to the conflicts of law provisions of Florida law. R. INTEGRATION OF AGREEMENT

This written Agreement contains the complete agreement of the parties on the subject matter of this Agreement. There are no such inducements, promises, or terms other than those contained in this written Agreement.

IN WITNESS WHEREOF, the INDEPENDENT CONTRACTOR and the CHILDREN'S BOARD

have executed this Agreement by their authorized officials.

CBHC does not assume responsibility for any of Independent Contractors’ supplies and equipment left on the premises.

Independent Contractor will maintain general liability insurance, professional liability insurance, worker’s compensation insurance, and automobile liability insurance, including hired / non-owned autos with minimum limits required and approved by the Children’s Board of Hillsborough County during the duration of this agreement. Independent Contractor will provide written proof of insurance on an approved ACORD form, naming the Children’s Board of Hillsborough County as an Additional Insured at our office address.

ATTACHMENT 2 I N S U R A N C E R E Q U I R E M E N T S

Firm's Liability Insurance: The Firm shall procure and maintain in force such insurance as will protect individual, firm or corporation from claims under Workers' Compensation laws, disability benefit laws or other similar employee benefit laws; from claims for damages because of bodily injury, occupational sickness or disease, or death of their employees including claims insured by usual personal injury liability coverage; from claims for damages because of bodily injury, sickness or disease, or death of any person other than their employees including claims insured by usual personal injury liability coverage; and from claims for injury to or destruction of tangible property including loss of use resulting there from, any or all of which may arise out of or result from the Firm’s operations under the contract, whether such operations be by himself or by any sub consultant or anyone directly or indirectly employed by any of them or for whose acts any of them may be legally liable. This insurance shall be written for not less than any limits of liability specified in the Procurement Documents or required by law, whichever is greater, and shall include contractual liability insurance. Before starting the work, the Firm will file with the Children’s Board of Hillsborough County, certificates of such insurance, acceptable to the Children’s Board; these certificates shall contain a provision for cancellation as found in paragraph 5 of Section B immediately below. Insurance Required: A. General Before starting and until acceptance of the Work by the Parties, the Firm shall procure and maintain insurance of the types and to the limits specified in paragraphs B. (1) through (4) below. All policies of insurance under this contract shall include as additional insured Children’s Board of Hillsborough County, and its employees. All policies shall provide for separation of insured's interests such that the insurance afforded applies separately to each insured against whom a claim is made or a suit is brought. B. Coverage The Firm shall procure and maintain in force during the life of this contract the following types of insurance coverage written on standard forms and placed with insurance carriers acceptable to the Partiesand approved by the Insurance Department of the State of Florida. The amounts and type of insurance shall conform to the following requirements:

1. Worker's Compensation Insurance - The Firm shall procure and shall maintain in force during the life of this contract, Workers' Compensation Insurance providing statutory benefits, including those that may be required by an applicable federal statute, for all of his/her employees to be engaged in work on the project under this contract. In case any class of employee engaged in hazardous work on the project under this contract is not protected under the Workers' Compensation statute, the Firm shall provide employer's liability insurance for all said employees.

6. Employee Fidelity Bond-Crime Insurance: N/A 7. Certificate of Insurance and Copies of Policies - Certificates of Insurance furnished by the

Proposer evidencing the insurance coverage specified in the previous paragraphs B. 1. through 4. inclusive, and on request certified copies of the policies required shall be filed with the Director of Operations on a timely basis. The required Certificates of Insurance not only shall list Children’s Board of Hillsborough County, as additional insured, for the operations of the Firm under this contract (excluding the worker's compensation, automobile liability, and professional liability policies) and shall name the types of policies provided and shall refer specifically to this contract.

If the initial insurance expires prior to the completion of the contract, renewal Certificates of Insurance shall be furnished thirty (30) days prior to the date of their expiration. The required limits for insurance may be achieved through a combination of primary and umbrella policies. These policies will provide that: the insurer(s) waive their rights of subrogation against the Children’s Board of Hillsborough County, and their officials, employees, agents, and consultants. Cancellation - Should any of the above described policies be canceled or non-renewed before the stated expiration date thereof, insurer will not cancel same until at least thirty (30) days prior written notice has been given to the below named certificate holder. This prior notice provision is a part of each of the above described policies.

CBHC Highlights

1945 The Juvenile Welfare Board (JWB) of Pinellas County is created. The Juvenile Welfare Board was conceived by a Juvenile Court Judge, an attorney, and a concerned group of professionals as well as private citizens who were unhappy with the lack of resources and services being provided to at-risk youth and families. Established by

statute in 1945 and approved overwhelmingly by voters in a referendum in 1946, JWB was created with a mission to provide needed services to children throughout Pinellas County.

1986 Florida adopts legislation enabling each of its counties to ask voter support for a special taxing district

for children’s services.

1988 Children’s Board of Hillsborough County (CBHC) is created by voters. Groundwork was laid for

justification of Hillsborough County Children’s Services Council by Hillsborough Constituency for Children. This founded

the CBHC and provided it’s funding source to levy up to one-half mill (50 cents per $1,000 of assessed property value) to

support services aimed at ensuring the welfare of children and their families in Hillsborough County.

1989-1990 Hillsborough County Needs Assessment (HCNA) is completed by the Partnership and submitted to the

Community at large and the leadership of Hillsborough County government, civic and religious organizations, public and

private human service providers, the United Way, and the business community. Thirteen Issue/Need Areas are

identified by the Community Partnership made up of 90 Community leaders, service providers, and representatives of

the public and private sectors.

1990-1991 CBHC releases approximately 3 million dollars in funding through a Request For Proposals (RFP) for the

first time using nine of the thirteen need/issue areas identified in the HCNA as the platform/funding plan. These

priority areas were seen by the board as the most urgent children’s needs in Hillsborough County. The Need Areas were

Child Care, Families With Special Needs, Family Violence, Financial Hardship, Maternal And Other Preventive Health

Care, Mental Health, Services Applying To A Range of Problems, Substance Abuse and System Wide. Approximately 70

programs were funded in this initial funding cycle. Priority is given to programs primarily serving families with children

ages 0-12, and children and families residing in a high-risk environment.

1997-1999 CBHC refocuses approximately 13 million dollars in funding on Primary Prevention and Early

Intervention outcomes and services that the board feels will best impact children and their families in the community.

Approximately 51 direct service programs are receiving funding through collaborative initiatives, neighborhood based

initiatives, and services addressing the reduction of teen pregnancy.

2002 The 2012 Plan is released. With board support and approval CBHC further focuses approximately 29

million in funding from a Needs Based or Charity strategy to an Investment Based model with a priority to target school

readiness for children birth to age 8. CBHC would accomplish this in collaboration with Providers through three key

Strategy Areas: Healthy Births, School Readiness and Early School Success. CBHC used Annie E. Casey Pathways

Mapping Initiative as the framework for implementing this focus. CBHC continued work with Public Consulting Group to

implement Revenue Maximization strategies within the County.

2002-2003 CBHC introduces Empowerment Evaluation (EE) to funded Providers. Based on the works of David M.

Fetterman and Abraham Wandersman and working consulting with Dr. Barbara Morrison Rodriguez (former CBHC Board

Member) EE provided a means of measuring outcomes and impact on a consistent and regular/quarterly basis in

partnership with Providers.

2003-2005 CBHC went through an exhaustive Strategic Fit/Alignment Process at the request of the board.

Programs were rated and recommended for approximately 33 million dollars in continuation funding based on specific

criteria: Age Focus, Program Focus, Outcomes and Measurable Indicators, System Criticality and Geographic Focus.

2011 The 2012-2016 Plan is released. The goal of the plan was that children in Hillsborough County would be

succeeding by Third Grade. CBHC embraced Results Based Accountability (RBA) based on the work by Mark Friedman.

RBA allows communities to track the impact of outcomes at the program and community level and change the curve.

CBHC works with Providers to answer the question “Did anyone get better as a result?”

2012 Reduced property tax and funding base (around 26 million dollars) creates need to focus funding even

more towards Evidence Based Models and Practices. With board commitment to process CBHC releases all program

dollars through a competitive RFP with funding to begin 10/01/2012 – Fiscal Year 2013 (FY13). The focus of the RFP

targets pregnant women and their children 0-8 years of age. The goal was that Children Are Healthy and Ready To

Learn. This would be accomplished through three, now four Result Areas: Children Are Healthy And Safe, Children Are

Developmentally On Track, Children Are Ready To Learn And Succeed, and Children Have Supported And Supportive

Families. A total of 43 programs were funded for FY13 through the highly competitive RFP process.

2012-2013 (Twenty Five Years Later):

Many Organizational and Staff changes throughout the Agency

Implementing Evidence Based Practices across the board with Providers was impacted by new program start-up

activities, adoption of evidence based models within programming, training of staff to properly use

measurement tools and instruments.

More rigors was required in the evaluation and measurement of program’s adherence to evidence based model,

fidelity to model, customer outcomes and impact of services.

Lessons Learned throughout FY13 and the Program Evaluation Process implemented required modifications to

adjust programs being recommended for FY14 funding.

There were Board member transitions in FY13. New Board members required training and orientation in order

to better understand the direction and focus of CBHC.

CBHC Funding Platforms were shared with the community: Investment Grants (Over $500,000 but not to exceed

$3,000,000), Uniting Grants ($75,000 to $500,000), Leading Grants ($5,000 to $74,999), and Match Grants

(Based on Funding Availability).

A Funding Timeline is Released alerting Funded Providers as to when they would need to reapply for Funding

through a competitive process.

2014 - 2016 CBHC begins to align community towards branding and support of the agency as it moves towards

Referendum on the Ballot. Voters will determine whether CBHC continues to operate through revenue created through

a percent of property tax millage.
