2.SearchfortheStudent

IntheStudentInformationsection,theSIDdefaultisyourown.Searchforastudentbyenteringthestudent’sSIDorclickingthemagnifyingglasstosearchforthestudentbyvalue(SID)ordescription(name).

Selectthecorrectstudent.

Thestudent’sinformationwillpopulate.

NOTE:TheeFormIDislistedinthetoprightcorner.YoumaywanttosavethisIDsothatyoucanquicklylocatetheformatafuturedate.

RequestInstructionswillpopulatebasedonthestudent’sacademiccareer.YoucancollapsetheseinstructionsbycheckingtheHideInstructionsbox.3.FillouttheeForm

RequestTerm:First,entertheterminwhichtherequestwillbeeffective.

Actions:Next,choosethedesiredaction.**Seechartabove**• Add:Addacollege,major,minor,orsubplan.

• Change:DropandaddanewCollege,Major,MinororSubplaninonerequest.YoucanalsouseChangetochangethestudent’sexpectedgraduationdate.

• Declare:Todeclareastudent’sfirstMajor.

Iftherearesupplementaryfilesyouwishtoinclude,youcanuploadthosehere.ClickUploadtochoosethefileandincludeadescriptionoffileyouhaveattached(optional).ClickAddFileAttachmenttoaddadditionalfiles.ClickDeletetodeleteuploadedfiles.

Example:Ifthestudentturnsinapaperformrequestingamajordeclaration,youcanincludethatherewiththedescription“MajorDeclarationPacket”.

Ifyouwouldliketoleaveanycomments,clickthedownarrowtoexpandthecommentsbox.Thestudentwillbeabletoviewthesecommentswhenhe/shenavigatestoViewSubmittedeFormsintheStudentResourcessectionontheirCalCentralMyDashboardpage.
