Acknowledgement

The proponents express their gratitude to the Barangay Lumbia headed by Honorable

Introduction

(Community Organizing Participatory Action Research (COPAR) is a continuous

and a sustained process of educating people, working with people and mobilizing with

people (Hilado,2009).)

It is an important tool for community development and people empowerment

because this enables the community health care team to encourage active participation

of activities that facilitates development in a community. It is where community members

unite to assess, plan and evaluate their conditions to aid in the improvement of their

community.))

The data gathered in this community exposure were tabulated and compiled as

well as with the plan of activities and projects to be implemented. Documentation, in this

part, is a vital component of this project for it shows the exact detail ofthe happenings in

tha area during the assessment process along with the identified problems. with this,

the proponents were able to develop realistic and futuristic goals for the implementation

of projects.

Sitio Baluarte, Lumbia, Cagayan de Oro City remained as one of the recipients of

Xavier University Nursing students for the past years in terms of providing community

services. Few projects have already been planned, implemented and evaluated for the

benefit of the clientele such as the residents and groups in the community.

In addition, this comprises the general profile of Barangay Lumbia, over and

above are the information about the outline of the current governance, general area of

assessment, individual family profile, health threats, health deficits, forseeable crisis,

and the suitable interventions which will be presented. Furthermore, this exposes the

proponents the opportunity to apply the nursing process and the principles of Nursing

Leadership and Management.

This project is in line with the vision and mission of Xavier University that is to

become men and women for others, as well as for the vision of the Xavier University-

College of Nursing, to be competent nurses in the future and to respond to the needs

and demands of the society. In addition, this project will highlight the importance of

applying the eleven key areas of responsibility such as safe and quality nursing care,

communication, collaboration and teamwork, health education, research, and

management of resources and environment.

Finally, the progress and improvement of the community from the projects

implemented will serve the purpose of this documentation. This will provide necessary

information about the proponents' exposure at Sitio Baluarte, Lumbia, Cagayan de Oro

City.

COMMUNITY PROFILE

A. PHYSICAL PROFILE

1. Location

Barangay Lumbia is situated at the southern most part of Cagayan de Oro City. It is

eleven kilometers away from the City proper bounded at North by Barangay Carmen,

Pagatpat, and Canitoan. On its Eastern part are Barangay Balulang and Cagayan River.

Barangay Bayanga on its southern part and at West by Barangay Pagalungan and

Tagpangi.

It consists of 21 Sitios:

C. SOCIAL PROFILE

1. Health 

Barangay Lumbia has one Health Center with three Health Personnel (Doctor, Nurse,

and the Midwife); eleven Barangay Health Workers and five Barangay Nutritional

Scholars. There are 75 households with water sealed toilets while 25 have Antipolo

toilets and the remaining 6 doesn't have their own toilets.

2. Education

The Barangay has:

> Upper Kiam-is Elementary School

- 1 Secondary school:

> Lumbia National Highschool

- 5 Day Care Center located at Airport, Cabula, Upper Palalan, Patpat, and Centro

under the management of CSWD

- 6 Kiddie Learning Center under the supervision of the Dartment of Education

The Alternative Learning System Program is currently present in Barangay Lumbia,

reaching the farthest of the Sitios to teach the out of school youths, less privileged and

elders for accreditation and equivalency for them to have a diploma and a certificate.

> An Inter-Organization of Sports Tournament is to be conducted by the Sangguniang

Kabataan to be participated by the different youth organization and agencies in the

Barangay.

> Major livelihood are sari-sari store, cashew nuts and banana chips production and

handicraft making.

F. LUMBIA BARANGAY HEALTH CENTER PROFILE

The WHO has defined “public health” as publicly funded or publicly owned health services. Public health is considered as one of the core elements in a government’s efforts to ensure the improvement and promotion of its people’s welfare (WHO 2003).

From hygiene and sanitation to control of communicable diseases, public health has evolved to address other emerging issues such as the effects of globalization and new technologies, among others. Unfortunately, there is evidence that current public health systems and services are not able to cope with these new challenges. Thus, there are current efforts to identify essential public health functions (EPHF) in order to define more clearly and systematically the core areas for public health work (WHO 2003).

In the Philippine health system, public health is often associated with the various program packages for the prevention, management, and control of diseases and promotion and protection of health. These health program packages cater to the various levels of health care delivery (from community-based to tertiary level facilities), to various population groups (mothers and infants, children and adolescents, adults and older persons), and to specific diseases (tuberculosis, malaria, cardiovascular diseases, cancer).

Since the implementation of health sector reforms, initial steps leading to an integrated approach to implementing public health programs have been undertaken.

a. DOH Mission and VisionVision by 2030

A Global leader for attaining better health outcomes, competitive and responsive health care systems, and equitable health financing.

Mission To guarantee equitable, sustainable, and quality health for all Filipinos,

especially the poor, and to lead the quest for excellence in health.

DOH Profile The Department of Health (DOH) holds the over-all technical authority on

health as it is a national health policy-maker and regulatory institution.

Aside from being the regulator of all health services and products, the DOH is the provider of special tertiary health care services and technical assistance to health providers and stakeholders. Under DOH are public

health services which are outposts to monitor communities and accessible health care services for the people. Lumbia Barangay Health Center is the accessible health care facility for Baluarte, Lumbia.

b. Lumbia Health Center Mission and VisionMission

Vision “We the health care managers and providers pledge to provide quality

health care through regulative, preventive, creative, rehabilitative services.”

Philosophy “Quality service all the time.”

Services Offered in Lumbia Health Center1. Maternal and Child Program

They offer prenatal check-ups, deliveries of cephalic presentation, full-term, without complications, Tetanus Toxoid and Ferrous Sulfate, and postpartum care. They also foster “Unang Yakap” for newborns to promote bonding with mother after birth. Prenatal check-ups, Tetanus Toxoid immunizations and ferrous Sulfate guide the mother for a safe pregnancy until postpartum. The goal of this program is to provide mothers and newborns quality care throughout the perinatal period and post-partum period. This program has the most records in the health center.

2. Family Planning ProgramThis program is to provide education among individuals and couples about

different family planning methods (artificial and natural) to promote self-awareness and control the number of children.

Moreover, this program encourages the participation of both the woman and the man with their decision-making ability to provide optimum child-rearing as well as strengthen their parenthood responsibility.

3. Expanded Program for ImmunizationThis program provides free vaccines which include Bacillus of Calmette

and Guerin (BCG), Diptheris-Pertussis-Tetanus (DPT), Hepatitis B, Oral Polio Virus (OPV), and Measles which is given among infants. The program aims to

reduce the mortality rate of the diseases that are prevalent which can be preventable with these vaccines.

4. Integrated Management of Childhood IllnessIMCI focuses on health promotion, disease prevention, early identification

of illness, and prompt intervention to palliation cure and recovery. It entails management of illness based on signs and symptoms the children presented ages 0-2 months and 2 months to 5 years. Children and their parents are advocated through health education, regular visits and proper referrals to the health center and nearby based hospital for a well-monitored care for the child.

5. Disease Free Zone InitiativeThis program aims to eradicate five diseases by year 2013. The diseases

are leprosy, filariasis, malaria, schistosomiasis, and rabies. For filariasis, the DOH will conduct a mass drug administration for every place. As for Barangay Lumbia, responsible pet ownership is also being promoted to get rid of rabies. Responsible pet ownership requires pet owner to submit their pets particularly dogs for vaccination.

Aside from the above mentioned steps, the public health nurse shall as well promote deeper awareness to these diseases in the place assigned to her so that proper referrals and actions will be done.

6. Intensified Disease Prevention ControlThe goal of this program is to prevent and control diseases as well as to

reduce the number of reported diseases in the certain area for the following diseases: TB, Dengue, HIV/AIDS/STI, vaccines preventable diseases, and emerging and reemerging diseases.

7. Healthy Lifestyle and Risk Management ProgramThis program aims to promote healthy lifestyles to those patients having

CVD, Diabetes, and Cancer. This also offers services and programs on dental and nutrition.

ECONOMIC SECTOR

Title of the project: "Sa Atsara, Kita Makakwarta"

Planning:

Name of the community: Sitio Baluarte, Barangay Lumbia, Cagayan de Oro City

Target Beneficiaries: Residents of Sitio Baluarte

Background: 

As assessed by the group, it has been determined that the community experiences a

common predicament- most of the residents at Sitio Baluarte, specifically the women,

are unemployed and are more likely to face isolation by being left at home doing the

usual household chores.

Goal of the project:

The College of Nursing of Xavier University-Ateneo de Cagayan will conduct the project

entitled “Sa Atsara, Kita Makakwarta". The project aims to help the residents of Sitio

Baluarte, especially the women, make use of their free time in making profitable goods

such as papaya relish (atsara). The ingredients to these goods are papayas, which is

abundant in the area. Moreover, this project will increase the resident’s knowledge on

papaya relish making and use these skills in producing goods to sell in other areas

which promotes independence with their livelihood.

Objectives:

At the end of 6 weeks of implementation, this project will be able to:

1. promote the project to the residents of Sitio Baluarte;

2. encourage the women to participate in the said project;

3. make the residents of the community see and understand the benefits of participating

in the said project;

4. explain and educate the residents about the process of papaya relish making;

5. facilitate the community to become independent in implementing the said project;

6. implement the project to the residents permanently and independently;

Resources:

The resources which will be used in the said project will consist mainly of papayas. this

is highly abundant in their area and others will be supplemented by the group through

sponsorship and donations. Furthermore, the materials which will be used for the

resident’s education about papaya relish making will be outsourced from the group's

funds, namely, manila papers, pentel pens and adhesive tapes.

Constraints:

ACTUAL POTENTIAL

- current knowledge about papaya relish

- ability of the residents to respond and

continue the project

- residents lack of participation and

collaboration during the educative phase;

- shortage of goods

Strategies:

To prevent these constraints, the group has identified the following strategies:

1. Educate those interested people about the project and its benefits.

2. Encourage the leaders to have a continuous monitoring of the project with its

progress and updates.

3. The selected leaders must a weekly evaluation of the project and evaluation to the

co-workers must also be done.

Mechanics:

For this project to be sustainable and applicable for the resident’s of the Sitio Baluarte,

the group prepared the following mechanics for this project:

1. Select leaders that are responsible and capable in conducting the said project;

2. Coordinate with the selected leaders for the implementation of the said project;

3. Reorient the community about the importance of the project;

4. Encourage the residents to have an active participation in the project;

5. Increase the resident's awareness of the benefits of the said project;

6. Evaluate the community's understanding about the project;

7. As a starting point, the group will raise funds through selling junks such as scratch

papers, plastic bottles, etc. The raised funds will be given to the association to serve as

capital for the economic sector;

8. The economic sector will coordinate with the community's association for the

coordination in planning the proposed projects;

9. Each member if the community will be encouraged to sell their papaya fruits(if any) to

the association, this will serve as economic sector's resources;

10. The interested residents of the community will be the one to make the papaya relish

11. Educate the residents on the process of papaya relish making to be led by a trainer

from ALS

12. The first product to be produce by residents will be tested by the community and

student nurses, if the results of the product is good in terms of taste and quality , the

said product will be marketed at the Xavier university canteen.

13. The capital used in producing the product will be separated from the profit , The

profit will be divided into the following:

10%- given to the BADECO

10%-emergency funds

10%-maintenance

70%-labor

14.If ever there will be shortage of resources such as the papayas, carrots can be a

good alternative.

15. Encourage the leaders to continue maintaining and monitoring the project;

Organization

the group is divided into subgroups which are tasked with different responsibilities:

1. project coordinator/head - tasked to coordinate with the other committees in the

evaluation of the project

2. Secretary- responsible in managing the activities of the entire project

3. Auditor- will responsible in monitoring the overall expenses and profit.

4. treasurer- responsible in recording all the expense and plans the proper allocation of

resources.

System and to the Coop of Sitio Baluarte (BADECO).

2. Student Nurses will start raising funds which will be use as

the capital for this project.

Recruit and encourage community people to join participate

with the project, specifically those women who doesn’t have

work.

1. The speaker from ALS will share her knowledge about the

livelihood program and motivate those community people.

2. Training and cooking demonstration will be done by the

representative from Alternative Learning System.

Return cooking demonstration will be done by the community

people who joined the said project.

1. The product of the return cooking demonstration will be

tested with its taste and quality. It will be tested by the

community people.

2. Student Nurses will buy the first set of product.

Evaluation and survey with its taste and quality will be

conducted by the leaders of this project, feedbacks and

suggestion will be appreciated for the improvement of this

products.

February 11, 2013 The selected leaders will conduct a meeting to the members,

the system of dividing the profit will be discuss and some ideas

or suggestion for the improvement of the project will be

discuss.

February 12, 2013 The leaders will buy the ingredients for the next set of product.

February 13, 2013 The treasurer of this project will plan for the proper allocation of

any expenses for the implementation.

February 14, 2013 Student nurses will be having an assessment on the progress

and updates of the said project.

February 15, 2013 Member of the project will be conducting a survey on how

many papayas will be available for the next set of product.

University Canteen.

February 20, 2013 Student nurses will be facilitating communication between the

Head of the project and the head of the Xavier University

Canteen.

February 21, 2013 Student nurses will be meeting the leaders of the project to

discuss if there is any problem encountered.

February 22, 2013 The Leaders will be having a meeting for the explanation on

how the profit will be divided.

February 23, 2013 Student Nurses will be editing and finalize the project

document.

February 25, 2013 Student nurses will have a meeting for both block A and D for

the progress and updates of the project.

February 26, 2013 Another set of product will be produce which will be facilitated

by the community leaders.

February 27, 2013 The student nurses will endorse all the responsibilities of this

project to the selected leaders.

February 28, 2013 Community will be auditing all the expenses and profits of the

said project, also will be conducting an evaluation of the

effectiveness of this project

March 01, 2013 Printing and bookbinding of the documented project.

Controlling

To have control in this project, residents will have a weekly meeting to discuss about the

progress of the project and to evaluate their co-workers. the head will be the one who will

facilitate the meeting.
