and both children really miss their mom. David seems to pick on the Prestons’ four-year-
old son a little. But, according to Mrs. Preston, it is “just adjustment stuff.” Mrs. Preston
set up medical appointments for next week.
Worker asked Mrs. Preston when would be best for visitation. Mrs. Preston states “any day
after 2:30 pm, when David gets home from school, would be fine.” Worker asks whether
visitation today would be possible. Mrs. Preston says that “it’s not a problem.” She agrees
to supervise the visits at her home, noting that would be “less stressful for the children.”

Ms. Wilson introduced herself and foster care worker, Ms. Mercado, to Ms. Dobson and
Mr. Patterson, and explained the CPS worker role and the foster care worker roles. Ms.
Dobson and Mr. Patterson indicated they understood these roles. She explained the
allegations in the CPS report and the nature of the 60-day investigation. Ms. Dobson
stated, “there doesn’t need to be much of an investigation because what Ms. Wilson
described is true,” i.e., Ms. Dobson and Mr. Patterson admitted to the allegations. Ms.
Dobson said that she wanted to go out and party and thought David could watch his sister
because “he has done so before.” Mr. Patterson replied, “Any fool knows a 7-year-old can’t
watch a baby, especially a kid as bad as David.” Ms. Dobson answered him by saying,
“Maybe he wouldn’t be so bad if you didn’t beat him when you’re around.” This worker
responded that their frustration is understandable and these are some of the exact issues
that need to be dealt with.
The CPS worker explained how and why the children were removed. This review included
the children being left alone, Fatima being injured on broken glass, and David having
access to a gun and drug paraphernalia left in several rooms in the home. The CPS
worker told Mr. Patterson that, although he is not David’s father, he is legally responsible
and was named as a respondent. The CPS worker also discussed allegations that Mr.
Patterson used excessive corporal punishment with David on several occasions. Mr.
Patterson admitted this was true.
This worker asked if there were any family resources for the children. Both parents said

there were not. Ms. Dobson said, “My dad and brother are dead and my mom is in a
hospice facility.” Mr. Patterson related that he is an only child, never knew his dad, and
has lost touch with his mother, who, according to Mr. Patterson, was “very abusive” to him
as a child. The CPS worker asked Ms. Dobson about David’s father and Ms. Dobson stated
that she was “ashamed to admit I have no idea.” Ms. Dobson says she thinks she
conceived David at a party she went to during her senior year of high school. She
recalls that she was “high as a kite” that night, and that she didn’t catch the name
of the guy she slept with.
The time frame for the court appearance on the Temporary Order of Removal (TOR) was
explained. Ms. Dobson and Mr. Patterson said they knew the court appearance was
scheduled for this afternoon, as they had already gone to Legal Services and showed them
the removal notice. The worker commented that this was good, that it really showed their
interest in the children.
A full-disclosure discussion was conducted and mutual understanding achieved. The CPS
worker discussed, in collaboration with the parents, what changes need to be made to get
the children returned home to a safe environment. CPS worker explained what caused the
children to be in imminent danger (lack of supervision of the children, the existence in the
home of firearms, drugs and drug paraphernalia, and environmental hazards such as
broken glass) and the need for placement as the only way to promote their safety needs.
The parents state they realize they need to change behaviors that jeopardize the safety of
the children; both said they would make the needed changes in six months. Both stated
they would clean their residence up as a start.
This worker discussed the impact that drugs and drug paraphernalia have on the children,
and both Ms. Dobson and Mr. Patterson said they would go to treatment, if necessary. Ms.
Dobson said she and Mr. Patterson “would do whatever it takes to get the kids home.”
The CPS worker explained that in order to promote the children’s safety during visits, the
parents would not be able to visit if it was suspected that they were under the influence of
substances when they arrived for the visit. Neither Ms. Dobson nor Mr. Patterson thought
they had “that big of a problem” or that they couldn’t control their drug use, but both
agreed to an evaluation which Mr. Patterson said would prove their point. They did agree
to be sober when attending visitation with the children.

Appeared before Judge Judy Silver in FC. Parents represented by attorneys, Michael
Logan for Ms. Dobson, Patrick Harrigan for Mr. Patterson, Erica Burke law guardian.
Ms. Dobson, through her attorney, admitted to the neglect petition and agreed with the
previously mutually-agreed-on plan the Department proposed. Mr. Patterson also
admitted to the allegations in the petition. The plan required Ms. Dobson and Mr.
Patterson to complete a drug and alcohol evaluation in order to identify the cause of their
use and triggers and how it affects their ability to supervise, care for, and protect their
children. The plan also includes that they will complete any treatment recommended
from the evaluation. Mr. Patterson and Ms. Dobson agreed to enroll in, attend, complete,
and apply the parenting skills learned in parent education training in order to strengthen
their relationship with their children and for Mr. Patterson to learn and implement
alternative methods of discipline that promote the children’s safety and well-being needs.
They will participate in supervised visits with their children at least weekly. The children
were ordered into the care and custody of the commissioner until the permanency
hearing. Date certain was set for the initial permanency hearing.

The worker discussed visitation with Ms. Dobson, Mr. Patterson, and the Prestons. Ms.
Dobson stated that she “hated the idea that of having to have supervised visits.” The
worker explained why the court ordered the visits to be supervised. It was also explained
that the worker would support and assist them in implementing the necessary changes to
meet children’s safety needs so they would be able to have unsupervised visitation in the
future. Mrs. Preston volunteered to have the weekly visits at her home and supervise. She
stated this would be less disruptive for the children; Ms. Dobson and Mr. Patterson agreed.
All agreed to set up the visits on Thursdays from 2:30 p.m. until 5:00 p.m., so the parents
can help the children with their afternoon snack, play with Fatima, and help David with his
schoolwork. Visits were set for 3/13/20xx and 3/20/20xx and 3/27/20xx. The worker told
the Prestons, Mr. Patterson, and Ms. Dobson that a written plan would follow.
The worker then asked Ms. Dobson and Mr. Patterson if they could come to the DSS office

The worker reached mutual understanding with Ms. Dobson and Mr. Patterson regarding
their children’s needs and mutually developed a service plan. Another full-disclosure
discussion occurred in which the worker reviewed the roles of everyone in the case,
important time frames, and expectations for parents to participate in change-supporting
activities.
Worker reviewed the following change-supporting activities parents previously agreed to:
1. Drug and Alcohol Evaluation: Ms. Dobson said, “I know I need to get to the
evaluation, but I don’t believe it will help.” She says she can quit “on her own” and, in
fact, she uses just enough to get out of feeling “dope sick” and only drinks on
weekends. This worker explained the evaluation could help identify change-
supporting activities which could help her meet the children’s needs for safety,
permanency, and well-being. Worker offered Ms. Dobson and Mr. Patterson the use
of the phone to call for the evaluation. Ms. Dobson said she will think about it and
declined to call. Mr. Patterson said he will make the appointment when Ms. Dobson
does.

2. Parenting Classes: Worker explained that the Department is offering a parenting class
for parents of young children to strengthen their relationship with their children and
identify and implement alternative methods of discipline that do not place the
children at risk of harm beginning next month and gave Ms. Dobson and Mr.
Patterson a pamphlet about the class. Ms. Dobson and Mr. Patterson agreed to
attend. Worker agreed to sign them up for the class.
3. Visitation: Court ordered weekly visits. Worker reminded Mr. Patterson and Ms.
Dobson of the plan that had been set up for weekly visits at the Prestons’ home. Mr.
Patterson and Ms. Dobson agreed to this schedule and Ms. Dobson stated that she
plans to visit this afternoon.
4. Counseling: Worker talked to Ms. Dobson about grief counseling to discuss all the
losses she has experienced. Ms. Dobson indicated she is “not ready” for this yet.

Ms. Dobson arrived on time for the visit. Mr. Patterson was not with her. Ms. Dobson
stated she was “not really in a very good mood.” When this worker inquired about Mr.
Patterson’s whereabouts, Ms. Dobson said Henry was around “only occasionally now
cause he blames me for the kids getting taken away.” Ms. Dobson said she felt like
she should end her relationship with Henry, but feared doing so as she would then
be “totally alone.” She told this worker how much she loves her children and of
her desire to be like the strong, caring mother she was losing. She informed the
worker that her mother had slipped into a coma and that it “probably wouldn’t be
much longer.” Ms. Dobson then began to cry and shared that just this past week
she had found out she was pregnant again by Mr. Patterson and thought she was
probably “about three or four months along.” This worker reflected Ms. Dobson’s
feelings and concerns. When Ms. Dobson told Mr. Patterson of the pregnancy, he
left Ms. Dobson’s residence and she heard he has been on a “major binge” since
then. This worker helped Ms. Dobson identify a clinic where she could receive
prenatal care and together they began to identify treatment options. Ms. Dobson
said, “I know I have to get clean for the baby’s sake” and immediately agreed to
attend an inpatient treatment program that she had called from the worker’s
office. This worker transported Ms. Dobson to the program intake and she was
admitted immediately to the 21-day program. Ms. Dobson signed necessary
releases for the treatment program to share information about Ms. Dobson’s
progress.

to discuss their mutual concerns for both Ms. Dobson’s and her unborn child’s well-being if
she continues to misuse substances. This worker informed Mr. Banks that she will try to

were found.” The children are doing okay, but David is getting into fights at school and
picks on her son Ned “a lot.” The worker suggested that Mrs. Preston call the school to see
if counseling services are available. This last visit with Ms. Dobson went fairly well. Ms.
Dobson read a book to Fatima and David and helped David with school work. She fed
Fatima and changed her diaper. Mrs. Preston stated she “tried to get LaVerne involved in

Ms. Dobson and Mr. Patterson arrived at this worker’s office this morning, asking to see
this worker as soon as possible. This worker met with the couple, who wanted assistance in
seeking inpatient substance abuse treatment for Ms. Dobson.
Mr. Patterson apologized for missing the previous visits with the worker and the children.
He said he had to “get away to clear his head.” The worker discussed with Ms. Dobson her
reasons for leaving treatment the first time. She said that she “couldn’t stand those people
in there.” Ms. Dobson admitted her continued use of substances currently while pregnant,
although she said she “was never high” when she visited with the children. She said that
she could not stop using. Ms. Dobson agreed to go back into treatment, but only if “Henry
got help too.” The worker advised Ms. Dobson that she will be able to get priority for a
program admission as a pregnant woman and the worker identified another inpatient
program for her to attend. The worker then asked Mr. Patterson if he still plans on going
to a treatment program. He said he did.
The worker suggested they call now for an appointment for Ms. Dobson. Ms. Dobson
called from the worker’s office and was told she could be admitted tomorrow. Ms. Dobson
and Mr. Patterson said they want to keep the visit today with the children.
The worker encouraged their attendance at this visit and reminded them that visitation is
important in building attachment and learning to parent the children in a way that
promotes their safety and well-being. The worker did point out that getting into drug
treatment; however, was the priority for both parents, especially Ms. Dobson. The worker
and parents agreed that the parents will follow up on the parenting classes and counseling

The worker asked Mr. Patterson where he is living. He said he is “between places” and
currently staying with Ms. Dobson. The worker asked Mr. Patterson for his mailing
address and he stated he “hasn’t gotten around to getting a mailing address yet.” The
worker reminded Mr. Patterson that he needs to let her know where he is so she can work
with him. He agreed to keep her informed.
The worker also advised the parents that during the visit they should explain to the
children that their mother would not be able to visit for awhile and why. Also, worker
reminded Mr. Patterson that he should keep his scheduled visits with the children while
Ms. Dobson is in treatment. He agreed.

Ms. Dobson spoke with the worker today from her inpatient treatment program and stated
she is “doing better and feeling better, but still feels depressed about all the people I’ve
lost in life.” She seemed excited about the baby now, but anxious about “losing him too.”
She talked about this with her counselor and in group and it helped. The worker praised
Ms. Dobson for her progress to date.
Ms. Dobson wanted to know if she could call her kids. The worker advised Ms. Dobson
that she has always been able to call and suggested she write to them also. Ms. Dobson
responded, “That’s dumb, what would I say to them? Fatima can’t even read. On the
other hand, David did like it when I read him stories.” The worker reminded Ms. Dobson
that David is a very good reader and suggested that she could draw a picture for Fatima.
Ms. Dobson said she would think about it.
The worker told Ms. Dobson that she had just received the children’s progress reports

As part of her treatment, she receives methadone, which will continue on an outpatient
basis. Ms. Dobson appeared well-rested and healthy. Her mood was very good and she
was very excited to see the children later in the day. She said she has been examined by an
“ob-gyn person” while in the program who told her “so far, so good, but the baby appears
small.” The medical staff told her that may be due to the drug and alcohol use.

Mr. Patterson was present for today’s meeting. He apologized for being out of contact
with the worker for so long and provided her with a new mailing address and phone
number. He stated that he was “trying to get it together.” When the worker inquired
about his attendance at the outpatient treatment program, he said he had “missed a few
group meetings” but was scheduled to meet with a counselor there early next week. Mr.
Patterson signed all necessary releases at this time and his attendance and participation in
visits was discussed.
Ms. Dobson said that Mr. Patterson couldn’t move back in with her until he was “totally
clean.” She also explained to the worker that she met her old “best friend” from
grammar school recently. This friend encouraged her to attend their childhood Baptist
church. Ms. Dobson said that she has felt a “hole in her spiritual life” since leaving the
Islamic faith and decided to begin going back to the Baptist church. Ms. Dobson further
stated that she felt “close to my mom” by attending this church, where her mother had
attended regularly before her illness. Ms. Dobson stated that the same minister was there
from her childhood and he talked to her and has agreed to provide individual counseling
to her. He extended his offer to Mr. Patterson as well. Ms. Dobson asked if the worker
could go with her to meet the minister for her an upcoming appointment. The worker
agreed and called the minister to confirm an appointment for 5/12/20xx at 10:00 a.m. Ms.
Dobson reports that the minister even knows the Prestons, as they began attending his
church about five years ago. He said they are “good people.” Ms. Dobson says this makes
her feel better. Mr. Patterson agreed to come to the session as well.

Rev. Wheeler indicated he has known Ms. Dobson since her childhood. He has begun to
provide grief counseling to Ms. Dobson, focusing on her grief related to her mother’s
terminal illness. Rev. Wheeler would “like to see the children back with their parents,” but
states he “believes that the drug problems need to be handled first.” Ms. Dobson and Mr.
Patterson agreed. Ms. Dobson continues her methadone treatment and they discussed Mr.
Patterson’s commitment to treatment as well. Rev. Wheeler also said that the parents need
to think about a “permanent plan for the children, given the fact that they have struggled
on and off for many years with their drug use,” even though they are making progress
now. Worker provided another full-disclosure discussion emphasizing the children’s
needs. Ms. Dobson and Mr. Patterson just nodded.
Rev. Wheeler stated that he knows the Prestons, they are also his parishioners. He believes
the Prestons would “care for the children on a long-term basis; they could do it
permanently if the can’t be reunified with their parents. LaVerne and Henry could
probably have some contact with the children. I know that the Prestons would provide a
good home.” Rev. Wheeler also told Ms. Dobson and Mr. Patterson that what he wants
most for the children is to go back to them and will work hard with the couple to help
them be reunited with the kids. He said to Ms. Dobson and Mr. Patterson, “as difficult as
this discussion is, you need to be realistic; drugs are a powerful force in your life.” Both
parents said they would continue to attend counseling with Rev. Wheeler. They said they
know they need to be able to meet their children’s needs for safety, timely permanency,

and well-being. Worker discussed the barriers to participating in change-supporting
activities of the mutually-agreed-upon service plan and strategies to overcome these
barriers. Ms. Dobson said they have been “thinking it over” and while they appreciate all
the Prestons have done to help their family, they have “no intentions of giving our kids to
the Prestons.”
Worker reminded Mr. Patterson and Ms. Dobson of the mutually-agreed-upon service plan
and that although they were making progress, there were still some significant changes
that needed to be made in order for the children to return home and be safe. Both
agreed.
The worker suggested the couple continue to talk about their feelings in counseling with
Rev. Wheeler and reminded Ms. Dobson and Mr. Patterson how important the time
frames they have discussed are. The worker reminded Ms. Dobson and Mr. Patterson of
their next two visits with the children. All participants agreed to meet again next month,
6/22/20xx, to review the family’s progress.

The worker called the Dobson residence to speak with Ms. Dobson regarding her recent
missed visits with both the worker and children. Ms. Dobson said that her and Mr.
Patterson “had it out” over his continued drug use, despite his attempts to stop and get
treatment. She says they “broke up once and for all” and that she was “too depressed” to
deal with visiting the children last night. The worker reflected Ms. Dobson’s feelings and
then inquired how she was physically feeling. She said she “felt fat” and “was run down.”
The worker asked about her last prenatal appointment and she said she “missed one or
two of those recently” as well and told the worker it really “wasn’t your business.”
The worker commented that Ms. Dobson seemed agitated with her today. Ms. Dobson
said she didn’t want the worker to “start up about the clock ticking again; I know I only

The foster parents reported that Reverend Wheeler came to the foster home last week to
support Ms. Dobson in telling the children about their grandmother’s passing; however,
Ms. Dobson did not show for the visit. Reverend Wheeler then told the children. David
was appropriately upset about his grandmother’s passing and did talk with the Prestons
and Rev. Wheeler about his feelings. Worker spoke with David today to tell him she was
also a resource if he wanted to talk more about his feelings regarding his grandmother, or
any other feelings he was having.
Mrs. Preston said she did talk with Ms. Dobson by phone before the funeral and Ms.
Dobson had asked her to bring the children to the service at the church and their presence
“was such a comfort” to their mother. Ms. Dobson and Mr. Patterson did miss the
scheduled visit two days after Ms. Dobson’s mother died. She called to cancel. Mrs.
Preston stated she has not had any contact with Mr. Patterson since he visited while Ms.
Dobson was in treatment. Ms. Dobson has called several times over the last few days and
Mrs. Preston reports that this seemed “to ease David’s mind. He was worried about his
mom since his grandmother died.”
The worker spoke with David and Fatima about their concerns and answered their
questions; both looked well and were smiling. David and Ned, the Prestons’ son, asked to
be excused to play their new Lego racing game on the PlayStation. Later in this visit,
though, the worker observed David picking on Ned and pinching him. Mr. Preston
intervened. Mrs. Preston stated that David’s problems in school continue, despite talking
to the school counselor bi-monthly. He is still aggressive at home and school. She stated,
“The poor little guy is so angry about life he just lashes out.” The worker advised Mrs.

Worker made an unannounced home visit to the Dobson residence. Ms. Dobson was
surprised to see the worker, but did invite her in. Her appearance was somewhat
disheveled and she seemed very sad. This worker expressed concern regarding Ms.
Dobson’s lack of visitation, lack of attendance in any of her change-promoting activities,
including her visits with the worker. Ms. Dobson explained that since her mother died she
“couldn’t stop feeling down” and felt like things were “hopeless.” She said she was starting
to get “anxious” about the baby’s birth and was worried that she would “screw his life up
too.” Another full-disclosure discussion occurred related to the urgency to achieve the
permanency goal of reunification and the need for consistent visitation with the children.

In a phone call, B. Wilson, the CPS worker, stated that DSS received a CPS report on Ms.
Dobson’s newborn baby, Jamal. Jamal was born six weeks premature and with a low birth
weight. The CPS worker reported that the birth was difficult as the cord was wrapped
around the baby’s neck and the doctor had to do a C-Section. Ms. Dobson smelled of
alcohol when she was admitted. Ms. Dobson said she had a “few beers before going into
labor” but denied using drugs. The baby had a positive toxicology for methadone and
alcohol, so a report to the SCR was made.
Ms. Wilson suggested that she and the foster care worker visit Ms. Dobson together at the
hospital to talk about the report and her plans for the care of Jamal, who will likely remain
in the hospital’s neonatal intensive care unit for some time.

tremors and difficulty feeding, as he does not have a fully developed suckling reflex yet.
CPS worker Wilson informed that a report was received because Jamal tested positive for
methadone and alcohol. She explained to Ms. Dobson that the methadone toxicology was
to be expected as Ms. Dobson continued methadone during the pregnancy as part of her
treatment. However, the report also stated that Ms. Dobson appeared intoxicated when
she arrived at the hospital and the baby also had a positive toxicology for alcohol. Ms.
Dobson admitted that she may have had “a drink or two,” but denied being intoxicated.
When this worker asked her whether she had been drinking during the pregnancy, she
admitted she “occasionally” has since her mother passed away. Ms. Dobson didn’t seem
concerned about the possible effects of alcohol on the baby, saying “It’s not like I was
shooting heroin again, so what’s the big deal!” When the CPS worker asked about
preparations for the baby, Ms. Dobson said “Henry is out buying things now for the baby
to come home.” Mr. Patterson came to the hospital last night after hearing from a friend
that Ms. Dobson had Jamal. The workers inquired about Mr. Patterson’s current
whereabouts, and Ms. Dobson said Mr. Patterson would be staying with her again to help
her take care of the baby.
Both workers suggested that Ms. Dobson have a visiting nurse, and told her that Jamal
needs to be seen by his doctor for perinatal follow-ups upon his release.
The foster care worker scheduled a meeting with Ms. Dobson in four days at 2:00 p.m. at
her home, following her release from the hospital. Hearing on violation petition has been
postponed until Ms. Dobson has recuperated from surgery.

This worker made an unannounced home visit to the Dobson residence in an attempt to
see Mr. Patterson. Mr. Patterson was home and allowed the worker to come in. There
were empty beer cans all over the kitchen. Mr. Patterson said he felt guilty about not
visiting the children in foster care, but seeing Jamal earlier today made him “really
serious” about “getting my act together.” The worker suggested he may need inpatient
treatment, but Mr. Patterson said he couldn’t do that with the baby coming home soon.
He did agree to call his outpatient treatment program while the worker was with him to
see whether he can begin attending again immediately. The program scheduled him for
an individual appointment in two days.
There were no noticeable arrangements made for the baby to come home yet. Mr.
Patterson said he was “working on it” and the worker again provided a referral for the
Salvation Army and another program to help low-income parents receive necessities for
new babies. Mr. Patterson said he would be at the worker’s meeting with Ms. Dobson upon
her discharge and would attend the next scheduled visit with the children at the Preston
foster home. Worker provided him with a copy of scheduled visits and reviewed his rights
and responsibilities related to the violation petition.

Progress Notes Narrative:
Ms. Dobson and Mr. Patterson were both at today’s scheduled visit. Ms. Dobson was
released from the hospital last evening. She was sad about the baby being “left behind.”
Ms. Dobson was also angry that Mr. Patterson “hasn’t held Jamal at all.” Mr. Patterson
said he “didn’t like hospitals” and “got freaked out” when he saw how small Jamal was
when he saw him in the NICU window. He also expressed anger that Ms. Dobson had
listed Jamal’s last name as “Dobson” instead of “Patterson.” Ms. Dobson said that was
because she couldn’t trust that he would be around to raise their son. This worker
suggested they contact Rev. Wheeler to begin couples counseling again.
Preparations for the baby to come home have yet to be made. The doctors believe it will
be at least 3-4 weeks before Jamal will be able to be released. The couple discussed there
concerns about Jamal with this worker and again said they would follow-through on the
referrals for the necessary baby items. Worker reminded parents of upcoming visits.
Worker will return in 10 days to check for necessary baby items. Another full-disclosure
discussion occurred and worker spoke with parents about their rights and responsibilities
regarding the violation petition.

Both Ms. Dobson and Mr. Patterson attended the visit today. Fatima was very shy around
both parents and seemed apprehensive of Mr. Patterson. Mr. Patterson appeared angry
that Fatima was “ignoring” him and the worker spoke with him about children’s reactions
to separation and stressed the importance of visiting. David was cool to Mr. Patterson, but
seemed excited to see his mother. Both children were excited to hear about their baby
brother and to see his picture.
Mrs. Preston and the worker reminded Ms. Dobson of the Prestons’ desire to take the
children on vacation with them. Mrs. Preston said they will be gone from 8/3/20xx to
8/10/20xx. She noted that Ms. Dobson and Mr. Patterson will miss two visits. Ms. Dobson
said, “That’s okay, the children won’t ever have the chance to ever do this again.” The
worker stated that the visits can be made up at a later date. Ms. Dobson and Mr. Patterson
agreed and signed the necessary forms.

This worker contacted NICU staff to assess Jamal’s present medical condition and to try to
determine his release date. Staff believe discharge will likely occur in the next week or two
as Jamal is making great progress. After working with an occupational therapist, he is
eating on his own now; however, they suspect he may have fetal alcohol spectrum disorder.
Worker inquired about how often Ms. Dobson or Mr. Patterson visit and the nurse
reported that Mr. Patterson hasn’t seen the baby since he was born, other than through the
nursery window and Ms. Dobson hasn’t seen him in over a week. She stressed how
important parental contact is for preemies.

Ms. Wilson stated that Ms. Dobson is still refusing to allow for a visiting nurse for Jamal
and says she will no longer attend her drug and alcohol treatment program. Mr.
Patterson’s whereabouts are unknown and Ms. Dobson said he’s “out of the picture.” The
Department filed a neglect petition and asked for a removal order for Jamal. Ms. Wilson
stated that she contacted the Prestons and that they will take Jamal when he is discharged.
Ms. Wilson said, “I gave notice to LaVerne Dobson that the Department was asking the
court to place Jamal in foster care and sent Henry Patterson notice at his last known
address, as well as at Ms. Dobson’s residence.” This worker offered to send a postal
clearance form and contact local police and probation departments, the county jail, and
the department of corrections to look for Mr. Patterson. Ms. Wilson said she also provided
information about the court hearing and information about visitation to Ms. Dobson. Ms.
Dobson was very angry and asked me to leave her home.”

Worker returned Mrs. Preston’s phone call re: unpleasant messages. Mrs. Preston stated
that Ms. Dobson called her quite upset because the Prestons “took her baby and took her
other children on their family vacation” to Disney World in Florida. Ms. Dobson told Mrs.
Preston that she plans to “sue them and the Department because the trip cost her two
visits.” Mrs. Preston reminded Ms. Dobson that she had agreed that the children could go
on the trip and, according to Mrs. Preston, Ms. Dobson got even more angry and said
something like, “I never went to see some overgrown mouse as a kid and I still lived.”
Ms. Dobson then told Mrs. Preston that she plans to come and see the children after she
hangs up the phone in order to make up for some of the missed visits. Mrs. Preston
advised Ms. Dobson that she does not have the authority to schedule visits and told Ms.
Dobson to call Ms. Mercado to schedule the makeup visits. Ms. Dobson hung up on her,
but has left additional “unpleasant” messages since. Mrs. Preston asked the worker to
speak with Ms. Dobson before her scheduled visit on 8/22/20xx. Mrs. Preston does not
want LaVerne to focus on the trip during the visit and upset the children, especially with
Jamal being so sensitive.

Worker asked Ms. Dobson if she had received her phone messages. She said she did, but
didn’t call back because “it wouldn’t do any good.” Ms. Dobson said that she is “really mad
because the Prestons took my kids to Disney World and then took my baby too. I am their
mother. I am more important than their vacation and I am the only one who can take care
of Jamal.” Worker reminded Ms. Dobson that she agreed to the family vacation plans for
David and Fatima with the understanding that she could make up the visits that were
missed. Ms. Dobson stated, “Yeah, that was then and this is now. I want my visits. I’m fed
up with everything being taken away from me.”
Worker stressed how important it was for her to visit frequently now that Jamal has been
placed. Visitation has increased to twice weekly in order for Ms. Dobson and Jamal to
bond. The worker asked Ms. Dobson if she was planning on attending the scheduled visit
with Jamal and the other children later today. Ms. Dobson said that would be okay, but
she is still mad about it. Worker reminded Ms. Dobson of Jamal’s need for bonding with
her and asked her to focus on building her attachment to him, rather than her anger.
Worker asked Ms. Dobson not to talk about the missed visits or argue with Mrs. Preston
about the trip because the children are still excited about the trip and would get upset.
Ms. Dobson said, “I’m not that stupid.” Worker reminded her of upcoming SPR. Worker
also inquired whether Mrs. Dobson had attended her drug and alcohol evaluation. Ms.
Dobson said it was “rescheduled” but couldn’t “remember the exact date” and told the

Mrs. Preston called to report that Ms. Dobson has not attended any of the scheduled visits
in the last week. Mrs. Preston explained that despite his early gains, Jamal is still not
feeding as much as a normal infant and that she has an appointment for him with the
pediatrician next week and the hospital arranged to have the visiting nurse come in two
times per week.
Mrs. Preston also reports that David is doing a little better since he has been involved with
the CFL and counseling. He still demonstrates aggressive behavior on occasion and does

Ms. Dobson showed up at worker’s office and asked receptionist to find this worker
immediately. Worker met with Ms. Dobson, who was crying, and stated that she knew she
had “screwed everything up with the kids” and admitted that she had been using heroin
again, “on and off” over the last month and that she didn’t attend visits when she was high.
Ms. Dobson had stopped attending her treatment program and had stopped the
methadone once she resumed actively using. This worker offered to help her get back into
treatment. They called the outpatient treatment program Ms. Dobson had been in and set
up an appointment for Ms. Dobson to meet with her substance abuse counselor later that
day.
Worker and Ms. Dobson discussed the importance of maintaining sobriety, visiting the
children, and particularly of developing a strong bond and attachment with Jamal.
Another full-disclosure discussion occurred and this worker laid out all the responsibilities

her on the need to get sober and said they could discuss a concurrent plan at a later date.

Mr. Patterson stated he hasn’t been able to complete the treatment requirements
from his drug and alcohol evaluation yet, but “has a line” on getting into another
program. He stated, “I want to get cleaned up for good this time. For my new son,
for Fatima.” Mr. Patterson said he realized he has to stop using in order to meet
the needs of his children. He stated, “I will never give up my kids, but it’s hard
and takes a lot of time.” The worker commended Mr. Patterson on his tentative
plan to get into a program and about his role as a parent with Jamal and Fatima.
Worker reviewed Ms. Dobson’s treatment plan sent from her drug and alcohol
counselor and how she has been doing in the program. She says “it’s tough
work…some days are good, some not so good.” Worker discussed mutually-
agreed-upon service plan and discussed the barriers to the achievement of the
outcomes. Mr. Patterson has not completed the treatment requirements laid out in
his evaluation, and although Ms. Dobson has successfully completed inpatient
treatment, she has relapsed since then and has not completed the other treatment
requirements. Mr. Patterson has only visited the children a handful of times since
they have been placed and has not kept DSS informed of his whereabouts. Neither
parent has enrolled in parenting classes yet, although everyone agrees that dealing
with their drug and alcohol issues must come first. Both Mr. Patterson and Ms.
Dobson said they know what they need to do.
Worker reminded Ms. Dobson and Mr. Patterson of their afternoon visits with the
children. The worker encouraged them to attend all visits and reminded them
how important the visits are to their relationship with the children.
The worker summarized what the parents need to do to get their children returned
home and meet their needs for safety, permanency, and well-being:

Ned stuck out his tongue at David and David responded, “Just wait until I get done.” The
worker noted that this type of reaction is not uncommon for children of David’s and Ned’s
ages. Mrs. Preston noted that David is still in the afterschool program and Ned is

Preston said, “He’s upset that you haven’t visited in such a long time.” Mrs. Preston said he
keeps saying something like, “If she doesn’t have enough time for me, why should I have
time to waste on her?” Mrs. Preston has had David talk to his therapist about this. Ms.
Dobson said, “He’ll change his mind when he sees me.”
Ms. Dobson entered the family room and ran to David and Fatima. Fatima backed away
behind Mr. Preston and David just glared at his mom. Ms. Dobson said, “Aren’t you glad
to see me?” David responded, “Why should I be, you never come to see us!” Ms. Dobson
tried to explain, but David didn’t want to hear it; he said, “I hate you.”
Mrs. Preston suggested that the children could draw their mom a picture of their new cat.
David said “no” at first, but then went with Fatima to the table to help her. Ms. Dobson
then picked up Jamal. He immediately started to cry and would not stop. Mrs. Preston
tried to give Ms. Dobson some ideas on what might comfort Jamal. Ms. Dobson just kept

Mrs. Preston and he immediately stopped crying.
Mr. and Mrs. Preston reviewed Jamal’s recent evaluation with the worker and Ms. Dobson.
The pediatric specialist suspects Fetal Alcohol Spectrum Disorder, but says they will need
to follow Jamal for a while before they can make a definitive diagnosis. He will also start to
receive Early Intervention services. Ms. Dobson became a little hostile towards the Mrs.
Preston at that point, saying, “You think I’m such a terrible person cause he’s got

Progress Notes Narrative:
Ms. Dobson apologized for being hostile to the worker last week. She said, “I was upset
because the kids weren’t happy to see me and that Fatima cried and David was so angry
with me.” Ms. Dobson says she discussed this at day treatment because she really wanted to
get high after the visit, but they helped her realize that it had been such a long time since
she visited and she must accept responsibility. She said she does. Worker asked Ms.
Dobson if she received the worker’s letter. She said she did. Ms. Dobson stated that she
has signed up for a new parenting class. She hasn’t had a chance to see Rev. Wheeler
again because she is “really busy” with day treatment. She promised she will call before
their next visit.
The worker conducted a full-disclosure discussion with Ms. Dobson to review important
time frames and her progress towards reunification with her children. Worker said she
realizes Ms. Dobson just started with another treatment plan designed to help her be able
to make the changes to provide permanency in a safe environment for her children. Ms.
Dobson stated she is aware that she needs to make changes and believes she is finally on
the right track. The worker agreed; however, she stressed the children’s need for timely
permanency, the 15-month mark when a decision will have to be made on the children’s
future, the fact that the children have already been in care for about nine months, and
questioned whether Ms. Dobson will be able to make the changes necessary in order to
have the children returned to her care or they will need to go to an alternative
permanency resource. The worker reviewed the visits that have been missed, Ms. Dobson’s
relapses, and her lack of participation in other change-promoting activities thus far, such
as parenting classes. Ms. Dobson stated she will not make an alternative plan for her
children’s future, as she will get them back, and that is her only plan. Ms. Dobson said,
“You know I have no relatives left and neither does Henry. I don’t even know where
Henry is, and I won’t tell you what you want to hear, that the Prestons can have the
children.” The worker stated, “There has to be a plan for the children’s permanency
needs if you can’t meet them, and the Prestons would be a viable plan.” Ms. Dobson said,
“I’m not ready to deal with it yet.” The worker advised Ms. Dobson that it is something
they do need to plan for, and reflected Ms. Dobson’s reluctance to make such a plan. This
worker advised her again of the benefits of developing an alternative permanency plan
and asked Ms. Dobson to seriously think about her desires for the children’s care if they
could not be reunified with her. Ms. Dobson agreed to “at least think about it,” and the

Ms. Dobson appeared at worker’s office today, demanding to immediately see her. After
Ms. Dobson calmed down, worker agreed to meet with her. Ms. Dobson was very upset
about “your agency taking my kids and giving them to the Prestons.” She said she “just
heard” about the second Permanency Hearing. Worker reminded her that it has been
scheduled for over six months and that worker made numerous attempts to reach Ms.
Dobson to remind her of hearing. Worker also said she provided Ms. Dobson with a copy
of the permanency hearing report, which Ms. Dobson says she did receive. Worker
reviewed outcomes of permanency hearing, including the PPG change to adoption. Ms.
Dobson began crying and begged the worker to “give me one more chance to change.”
Ms. Dobson said after she was evicted from her last apartment, she was forced to stay with
her stepfather, even though he is “a terrible man.” She recounted that things were “so bad

Ms. Dobson called worker, stating her stepfather had given her the letters the worker had
sent. She stated, “You can’t take my kids away, I just need some more time to get myself
back together.” The worker told Ms. Dobson that time is running out, she hasn’t visited in
over six weeks, and hasn’t made the necessary changes to get the kids home. They
scheduled a visit in the workers office for next Monday at 11:00 a.m. (6/16/20xx)

Ms. Dobson stated she realized she has wasted a lot of time and has failed to plan for her
children’s return. She said that Mr. Patterson’s death put her over the edge and she
couldn’t stand the pain of his death. She said, “Now it will take me even longer to get the
kids returned.”
Worker reviewed outcome of most recent permanency hearing, which Ms. Dobson did not
attend. Worker informed Ms. Dobson of court’s decision to order a TPR to be filed and a
change of the PPG to adoption. Worker told Ms. Dobson she will continue to help her
work towards reunification and making the changes necessary to meet her children’s needs
for safety, permanency, and well-being, but it is necessary now to start moving forward with
the plan for the Prestons to adopt the children.
Worker reminded Ms. Dobson of full-disclosure discussions and of concurrent plan.
Worker explained this is why attending drug and alcohol treatment and therapy is
encouraged. This will help her obtain ways to handle problems. These activities could
help support her in meeting her children’s needs for safety, timely permanency, and well-
being. And parenting education and regular visitation are important to help support her
in being better able to meet her children’s needs.
Ms. Dobson said, “Yes, I understand this, I know what the drugs did to my life and the
kids. I know I need help, but I need more time.”
The worker stated that the children have been in care over a year and she has not been

able to make changes so she can meet the children’s needs for safety, timely permanency,
and well-being. The worker told Ms. Dobson that the case has been recently reviewed by
the DSS attorney and the agency has decided to seek a TPR. Worker explained what this
meant for Ms. Dobson and the possible outcomes of the TPR hearing. Worker stressed to
Ms. Dobson that the children need to achieve permanency and, if it can’t be with her, then
there needs to be a plan for an alternate permanency resource. Ms. Dobson said she
doesn’t need another plan or family because she will get the kids back.
The worker asked Ms. Dobson if they could talk more concretely about her thoughts and
feelings about the Prestons adopting the children. Ms. Dobson said, “Absolutely not!
You’re not taking my kids from me!” and quickly left the office.
