Principles of Incision and Flap Design in Oral Cavity Many oral and maxillofacial surgical procedures require incisions. Some basic principles are important when performing tissue incisions. The first one is the use of a proper size sharp blade. Most incisions in the oral cavity are done with a size 15 blade. The second one is that long, continuous strokes are preferable to short, interrupted ones. And the third principle is that the surgeon should avoid cutting vital structures. Concerning the site, it is desirable to incise through attached gingiva and on the top of healthy bone. Properly positioned incisions allow the wound margins to be sutured over intact, healthy bone, thereby providing support for the healing wound.
