On April 18, 1988, while Nine Mile Point Unit 1 (NMP1) was in cold shutdownfor refueling, it was discovered that several portions of the ASME SectionXI Class 1 Pressure Boundary had not been hydrostatically tested in 1986.ASME Section XI requires a hydrostatic test of the pressure boundary forClass 1, 2, and 3 piping and equipment at or near the end of each InserviceInspection (ISI) Interval. The first ten year ZSI Interval was to becompleted in 1986. Failure to complete testing in accordance with ASMESection XI constitutes a violation of plant Technical Specification 3.2.6.

The root causes of this event have been determined to be managementineffectiveness in implementing the ISI Program Plan, and inadequate reviewof the hydrostatic test procedures. Contributing factors weremisinterpretation of ASME Section XI requirements, an overly aggressiveschedule, inadequate control of measuring and testing equipment, andproviding insufficient resources for tracking ASME Section XI requirements.

Corrective actions include: conducting a review of the first ten year ISIInterval hydrostatic test procedures and test perfor'mance data to assurethat ASME Section XI requirements were met; completing first ten year ISIInterval hydrostatic test; submitting Relief Requests where required;completing ASME Section XI training, and issuing a Lessons LearnedTransmittal to address root causes and contributing factors.

DESCRIPTION OF EVENT

On April 18, 1988, during the development of a procedure for ASMESection XI Class 1 System Leakage Testing, it was noted that a portionof the ASME Section XI Class 1 boundary was not incorporated into thehydrostatic test performed for the first ten year Inservice Inspection(ISI) Interval. 'ine Mile Point 1 (NMP1) was in a cold shutdowncondition for a refueling outage with the core off-loaded when thisevent was discovered.

ASME Section XI paragraph IWB-5210 requires a hydrostatic test of thepressure retaining boundary for all ASME Section XI Class 1 piping andequipment at or near the end of each ISI Interval. NMP1 was tocomplete the first ten year ISI Interval in 1986. Failure to performhydrostatic testing in accordance with ASME Section XI constitutes aviolation of plant Technical Specification (T.S.) Section 3.2.6.

Hydrostatic Test Procedure Nl-ISI-HYD-01, "Reactor Pressure Vessel andAll ASME Class I Systems and Reactor Vessel Instrumentation Class 2HFRevision 0, was being utilized to perform hydrostatic testing inaccordance with ASME Section XI, paragraph IWB-5210. The intent ofthis procedure was to incorporate all ASME Section XI Class 1 systemswithin the test boundary. However, the valve line-up described in theprocedure required that the manual blocking valves upstream of theElectromatic Relief Valves (EMRVs) be closed. This resulted inexcluding from the test boundary, one weld, the manual blocking valve,and associated pressure retaining components for each of the EMRVs.

There are six EMRVs located on the two main steam lines (three on eachline) within the primary containment. Each EMRV bolts to a flangewhich is welded to the manual blocking valve. The manual blockingvalves are connected to the main steam lines via welded attachments.

This condition was discovered by Nuclear Energy Services, Inc. (NES)'nApril 18, 1988. NES was contracted by Niagara Mohawk Power

Corporation to support implementation of both the first ten year ISIInterval program and the current ISI Interval program. Procedure Nl-ISI-HYD-01, Revision 0, was written in May of 1986 during the refuelingoutage by E.N. Consulting, Inc., who was subcontracted by NES. Theprocedure was based upon a Reactor Pressure Vessel Hydrostatic TestProcedure written by Stone & Webster Engineering Corporation (NMP1Construction Architect/ Engineer) in January 1968, which also requiredthe manual blocking valves to be closed. Hydrostatic Test ProcedureNl-ISI-HYD-01, Revision 0, was reviewed and approved on May 25, 1986,iq, accordance with Site Administrative Procedures, and performed onJune 3 and 4, 1986.

Revision 0 of the hydrostatic test procedure, written by .Stone andWebster Engineering Corporation in January 1968, is the originalconstruction test procedure. Revision 1 of this procedure, datedOctober 1968 and performed in November 1968, opened the manual blockingvalves and hydrostatically tested the EMRVs as part of ASME Section XIClass 1 at a pressure of 1800 pounds per square inch gauge.

Additionally, a review of the first ten year ISI Interval (1986)Hydrostatic Test. procedures and test, performance data was conducted toprovide assurance that the ASME Section XI requirements were met. Thereview identified discrepancies in 23 hydrostatic test procedures (i.e.various systems whose pipe and/or equipment should have beenpressurized and ASME Section XI, VT-2 examined but were not, orsections of pipe and/or equipment which were tested at lower pressurethan that required by ASME Section XI). Requests for relief from theASME Section XI requirements had not been submitted to the Commissionfor any of the above discrepancies.

CAUSE OF EVENT

The root cause of this event was originally determined to be personnelerror. However, as a result of the additional 23 test discrepanciesidentified, the root cause was re-evaluated and has been determined tobe:

A. Management ineffectiveness in implementing the ISI Program Planand;

B. Inadequate review of procedures used in hydrostatic testing.Other factors which contributed to the pressure testing errors are:

Misinterpretation of ASME Section XI requirementswhich'eadto the omission of portions of piping and equipment

from the hydrostatic test procedures;

Lack of NMP1 Controlled ASME Section XI Boundary Diagrams;

An overly aggressive schedule;I

Inadequate control of measuring and testing equipment and;

Providing insufficient resources for tracking ASME SectionXI requirements.

The misinterpretation of ASME Section XI requirements by both theauthor of the hydrostatic test procedure (N1-ISI-HYD-01) and procedurereview personnel was determined to be a result of an ineffective and

inadeuqately trained staff knowledgeable of ASME code requirements.The hydrostatic test procedures received the level of review prescribedby Site Administrative Procedures, however, the procedure reviewersfailed to recognize the impact of procedurally excluding the relevantpiping or equipment from the test boundary. Relief Requests to excludethe relevant piping or equipment from hydrostatic testing were neithersubmitted nor granted.

ANALYSIS OF EVENT

An Occurrence Report (OR No. 88-368) was initiated on April 22, 1988,and was preliminarily determined to be not reportable. During secondlevel review by the Station Technical Staff, the determination ofreportability was questioned. After further review by Technical Staff,Operations, Engineering and Maintenance, the event was determined tobe reportable on April 29, 1988, in accordance with 10 CFR 50.36(c)(2),"Limiting Conditions for Operation". This event was also consideredreportable in accordance with 10 CFR 50.73(a)(2)(i)(B), "Any operationor condition prohibited by the plant's Technical Specifications."At the time OR 88-368 was determined to be reportable, NMP1 stationmanagement directed Site Engineering ISI to perform a comprehensivereview of the first ten year ISI Interval hydrostatic test requirementsand test performance data. As a result of this review, additionalOccurrence Reports were generated. There were no adverse safetyconsequences associated with these events. The failure to perform thehydrostatic tests, in and by themselves, did not result in any failureof the piping and, therefore, did not affect the health and safety ofplant personnel or the general public.There were, however, potential adverse safety consequences associatedwith these events. Hydrostatic tests provide information on theintegrity of the piping and equipment to contain design pressures.

The'ailureto perform these tests increased the uncertainty of knowledgeas to the condition of the piping and equipment. In the worst case,the failure to perform the hydrostatic tests could have resulted in notidentifying an impending failure and then having this failure occur;however, leakage monitoring systems should detect any significantincreases in leakage.

CORRECTIVE ACTION

A. The Root Cause of management ineffectiveness is a generic issuewhich has been addressed in the NMP1 Restart Action Plan (RAP)and Nuclear Improvement Plan (NIP). B. A Lessons Learned Transmittal has been issued to departmentsinvolved in the production and review of ISI related HydrostaticTest procedures. It addresses the importance of selecting orassigning test procedure reviewers with technical expertisecommensurate with the testing being performed and providingadequate time for review. It also addresses the importance ofmaintaining accurate communication of the technical requirementsof performing hydrostatic testing in accordance with ASMESection XI.

The Planning & Scheduling Department. was reorganized; an Outage,Manager .was appointed as well as an increase in staff.A Site Measure and Test Equipment (M&TE) Department has beenorganized including new procedures for control of M&TE, and SiteEngineering Department personnel have been trained on M&TEprocedures.

Procedure N1-88-7.7 (Restart requirements; Inservice PressureTesting Program) was developed to track and close out the ASMESection XI Pressure Test requirements for the first ten yearISI Xnterval. A Pressure Test Program Plan will be developedwhich will enhance the ISI Program. The second ten year ISIXnterval Program Plan is currently undergoing an extensivereview using personnel experienced in the area of ASME SectionXI. This review will ensure compliance to ASME Section XI andUnit 1 T.S. for the future.

The portions of plant systems which were excluded from the ASME SectionXI Class 1, 2 and 3 hydrostatic tests or that were tested at the wrongpressure in the 1986 outage will be tested as necessary prior tostartup from the current refueling outage. Relief requests have beensubmitted and granted for portion of systems that cannot be tested toASME Section XI requirements. Hydrostatic test procedures have beengenerated which will test the portions of the systems which wereomitted from the first ten year ISX Interval. Certain systems havebeen reclassified as non-safety related, or have been excluded from theISI Program based on Regulatory Guide 1.26; no pressure test isrequired for these systems.

A. Previous similar events:

There have been three previous events involving deficienciesidentified in the first ten year ISI Interval program. Details ofthese events are contained in LER 87-27, LER 88-01, and LER 88-01-S1. However, the conditions identified in those events did notinvolve deficiencies related to the hydrostatic testing program.Consequently, corrective actions from these previous events would nothave identified the conditions described in this LER.

Identification of procedures referred to in this LER:

The 23 discrepancies found with the hydrostatic test procedures arecategorized as follows: 6 discrepancies required ASME ReliefRequests to be written, submitted and approved by the NRC; theremaining 17 discrepancies required original procedures to berewritten. Presently, 11 of these 17 test procedures have beensatisfactorily completed with no conditions adverse to quality found.The remaining 6 tests will be completed prior to plant startup.The status and identification of the 23 discrepancies is provided inTable 1. Table 2 provides a summary of the 6 issued Relief Requests.
