New! Manage Section – New Location, Look and Feel

• All administrative functions are now located in one place to make completion of

manage tasks quicker and more efficient.

• Click on the "Manage" button on the main dashboard to access this new

Manage section. Here you can manage Residents, Samples, Groups, MDS

Uploads, Facilities, and Users according to your access privileges.

• To return to the Main Dashboard, click on the abaqis logo or the Main

Dashboard selection from the menu.


New! Meeting Minutes

• Users with QA Admin or Corporate Admin permissions have the ability to

create/view Meeting Minutes by clicking on the Minutes icon from the

dashboard.

• User can select Meeting Minute Type including QA/QAPI Meeting, Morning

Meeting, or Other.

• Minutes can be designated as View and Edit or Read-Only for other QA

Admin/Corporate Admin Users.

New! Resource documents for the CMS Emergency Preparedness Tags (E-Tags) have

been added to the Final Rule tab of the Help and Training Resources Section of abaqis.

New! Family Interview for Completion by Mail

• This new form is available under the Forms tab of the Help and Training Resources

section.

Improved! Investigations Section

• An additional Save button has been added to the top of each F-Tag question to

limit the need to scroll to the bottom of the page to Save content.

• Improvements to Investigations Reporting

o New Deficiencies by F-Tag Report groups all deficient F-Tag responses

within the selected snapshot(s) into the specific F-Tag citation.

o Deficient F-Tags Report has been renamed Deficiencies by Care Area

Report and groups all deficient F-Tag responses within the selected

snapshot(s) into Care Areas.

o Multiple Snapshot Selection: All Investigation Reports include capability

for viewing results of multiple, combined Snapshots. This does not include

Ad Hoc Investigations.


Improved! Facility Assessment Sufficiency Analysis Enhancement

• Additional selection options have been included in the Sufficiency Analysis

section:

o Sufficient

o Insufficient

o In Progress

o Not Applicable – used only if your resident population does not require the

service.

Improved! PAC Assessment – auto generation of Reviewer name

• When a user opens a partially completed PAC Assessment, the Reviewer's name

will be auto-populated as long as the Reviewer field is currently blank.

Improved! Calendar Date Picker Enhancement

• Calendar date pickers now have the capability to scroll not only by month but

also by year.

Other! IE 10 and below are no longer supported. A modern browser is required.

New! Snapshot Types

• Upon snapshot creation, user is now able to select a snapshot type in order to

provide improved guidance for the Investigation process.

• (3) Snapshot Types:

o QAPI Investigations Snapshot: prioritizes care areas based on completed

QAPI Assessments. All Investigations and Facility Tasks are available as is

the ability to manually add residents.

o Purposeful Investigations Snapshot: focuses only on user-initiated

Investigations and does not include Facility Tasks. Targets only desired

care areas with manually added residents.

o Facility Tasks Snapshot: focuses only on Facility Tasks and does not include

prioritized or resident level Investigations.

New! Enhanced PAC Module include (2) levels of PAC Tracking and Reporting

• Basic PAC:


o Uses MDS data to gather basic information for hospital readmission and

community discharge reporting.

o No assessments required, however, optional PAC Assessments can be

completed in order to provide additional tracking measures for Entry

Hospital and Payor.

• Advanced PAC:

o Uses MDS data to gather basic information for hospital readmission and

community discharge reporting.

o No assessments required, however, optional PAC Assessments can be

completed in order to provide additional tracking measures for Entry

Hospital, Payor, ER Transfers, HRRP diagnosis, elective hospitalizations,

advance directives, and discharge details.

Improved! Custom Sample Size for Random QAPI Samples

• Custom sample sizes can now be created for Census and Admission Samples

when generating Random QAPI Samples.

Improved! New 2018 Basic & Advanced QAPI Accreditation Standards

• In order to continue to align the QAPI Accreditation Program with current

regulations, Basic and Advanced Accreditation Standards have been updated.

These standards are in effect as of January 1, 2018.

Improved! Facility Assessment: Rehabilitative Services Frequency Column

• Frequency column is now referencing Average Number of Days vs % of

Admissions/Stays.

Improved! Facility Assessment Updates and Enhancements

• The MDS Resident Profile has been updated to include MDS data for residents

with an MDS assessment that falls within the specified time period. This

enhancement will assist in improved representation of long-stay residents.


• The Facility Assessment look back period is one year from when the Facility

Assessment is created. (For Example, if a Facility Assessment is created on

12/14/17, the look back period would be 12/14/17 - 12/15/16.)

• Potentially High Risk Treatments (section B.1. within the Acuity Section of the

Resident Population Profile) has been revised as follows:

o Instead of selecting a frequency of potentially high risk treatments,

facilities will now select the number of stays over the past year that

required potentially high-risk treatments.

o Selection options include: None, 1-5, and More than 6.

• New Facility Assessment Resource Documents have been created to assist in

answering frequently asked questions. These documents include:

o Admission/Stays Summary Guide

o Frequency Guide

o Sufficiency Guide

Fixed! A subset of Facility Tasks/Investigations were missing the corresponding CE

Pathway link within the Investigation

This has been fixed.

• Facility Tasks have been separated out from Resident Level Investigations and

can now be completed within a snapshot or outside of a snapshot as an Ad Hoc

Facility Task.

• This Ad Hoc Facility Task capability will be an account-level subscription.

New! Investigations Reporting

Investigations will now have a printable summary View page of all Investigation

responses.

New! QAPI Questions have been added to a subset of Care Area Investigations in order

to highlight new guidance related to CMS Final Rule

This QAPI Audit can be utilized as a preliminary investigation prior to completion of the

more in-depth CE Pathway.

Updates and Enhancements for QAPI Tools:

Improved! Facility Assessment

• Printable pdf has been reformatted for improved organization and structure.

• Sufficiency Analysis sections: "Insufficient" response selections have been

highlighted for improved visibility.

Improved! QAPI Plan read-only option for text areas

Corporate Admin users have the ability to designate sections of the Corporate QAPI

Plan Template as read-only and, thus, made un-editable when generating a facility

level QAPI Plan.

New! Facility Assessment

● Regulation Requirement: Nursing facilities will conduct, document, and annually

review a facility-wide assessment, which includes both their resident population

and the resources the facility needs to care for their residents (§483.70(e)).

● The purpose of the assessment is to determine what resources are necessary to

care for residents competently during both day-to-day operations and

emergencies.

● Use this assessment to make decisions about your direct care staff needs, as well

as your capabilities to provide services to the residents in your facility.

New! MDS Resident Profile Report

The MDS Resident Profile report can be found under the QAPI section of the Reports

page. This new report allows customers to review characteristics of their facility’s

resident population over the past year without creating a Facility Assessment.

New! Facility Assessment Help Doc

A facility Assessment Help Doc has been created to provide Users with a step-by-step

guide to generating and completing a Facility Assessment.

Improved! PIP Enhancements

● Team Section: The question regarding PIP team meeting frequency has been

relocated to the end of the Team Section in order to avoid being overlooked as

was previously the case.

● Measure Section: Users will now record a Baseline Measurement within the same

step as Adding a New Measure. This will help to increase efficiency of this

process and prevent a user from navigating to a new section within the PIP

without recording this measurement.

Improved! Date Filters have been added to Readmitted Residents and ER Transfer

Reports

Timeframe for these reports previously defaulted to prior 100 days. A filter option is now

available to select start and end dates to adjust report to desired date range.

Improved! Updated CMS Documents pertaining to the Final Rule can now be found in

the Help and Training Resources section

● The CMS Survey and Cert Memo (6-3-17) provides a summary of upcoming

changes to include Revised Interpretive Guidance, Revised F-tags, as well as

provides additional training resources.

● The State Operations Manual - Advance Appendix PP including Phase 2

[Effective November 28, 2017] can now be found under the Final Rule Tab of the

Help and Training Resources section.

Fixed! PIP Issue

There was an issue related to the team member dropdown not working when

attempting to add a PIP team member. This has been fixed.

Other! Facility Name field has been disabled within the Facility Edit section

There have been instances where users were inadvertently editing Facility Name when

attempting to edit Facility Assignments. This field has now been disabled.

New downloadable QAPI Immediate Action Plan is now available to assist customers in

performing QAPI functions.

Improvements!

● All abaqis forms have been updated to include Form Name and Page Number

for better organization and form management.


● Guidance to Surveyor forms have been updated to include the most recent

revisions as per the State Operations Manual Appendix PP issued 3-8-17.

Fixed!

● QAPI and PIP Document Upload Issue

○ Document Upload button was causing an error when double clicked. This

has been fixed.

● IE 11 PIP Issue

○ In IE 11, user was unable to navigate to a different section after adding an

intervention to a PIP. This has been fixed.

New! Supporting Document Upload Feature

Users are now able to upload supporting documents to both the facility and corporate

level QAPI Plans and PIPs. All documents can be downloaded.

Improved! QAPI Plan Enhancements

● “Workable” Corporate QAPI Plans - Corporate Admin Users can create

Corporate QAPI Plans and edit/modify until document is completed as desired.

Upon completion, the Corporate Template can be Activated and made

available for facility use in creating their individualized facility-level QAPI Plan.

**Note: Facilities are not able to create a facility level QAPI plan until a

Corporate QAPI Plan template has been created and activated.

● Expanded QAPI Plan printing capabilities - Corporate Admin Users are able to

print both Active and Inactive Corporate QAPI Plans.

Other Improvements

● Readmission Rate Report now includes a 7 day Readmission Rate Option

● Enhanced messaging when creating a new snapshot indicates repercussions of

that action (ie. current snapshot is archived and uneditable; accreditation

status may be impacted)


● PAC csv downloads (including Readmitted Residents, ER Transfers, and

Community Discharges) will now include resident DOB for improved resident

identification

● Market Analysis Map includes an updated Nursing Home icon indicating “My

Facilities” while also retaining CMS Overall Star Rating

● Accounts that have SSO enabled will now display an SSO ID column on Users list

page

● Discontinued QCLIs (from July 2015 QIS Update) have been removed from

reports

● Updated minimum requirements for Internet Explorer to IE10 for all new abaqis

features and enhancements

Fixed!

● “Unsaved Changes” error message was displayed when attempting to navigate

away from the Intervention Section of PIP. This has been fixed.

● Printing the csv for the Corporate Customer Satisfaction Report as well as the

Stage 2 Snapshot Progress Reports did not reflect selected filter options. This

has been fixed.

● Facilities link from the User page was not working. This has been fixed.

● Notifications from unassigned facilities were still showing up in user’s Notifications

Report. This has been fixed.

Improved! New and Improved abaqis Help section

The abaqis Help section has been updated and reorganized for better access to

desired abaqis resources. In addition, this new section will include updated Help

Documents as well as new Implementation Documents and Forms. To access the Help

section, the abaqis Help link on the dashboard has been renamed “Help and Training

Resources”.


Improved! abaqis Stage 2 Guidance and regulatory language has been updated to

reflect the CMS Final Rule

In addition, access to the CMS Final Rule document will be available under the new

Final Rule tab in the Help and Training Resources section of abaqis.

Fixed! Market Analysis Map

Market Analysis Map information was not refreshing on ipad when user was attempting

to change locations. This has now been fixed.

Fixed! QAPI Self Assessment

Printing the QAPI Self Assessment to PDF was cutting off comments. This has now been

fixed.

New! QAPI Plan Builder

QAPI Plan Builder is a new feature in abaqis that will assist facilities in preparing for

upcoming CMS regulations as they pertain to the QAPI process. Specifically, this feature

will help facilities to generate a formal, documented QAPI plan that can be presented

to surveyors upon request.

● Corporate Admin User will be required to create a QAPI Plan template before a

facility is able to add a QAPI Plan.

● Templates will be provided both at the Corporate and Facility level to guide

users in providing relevant information and documentation that is necessary for

completion of the QAPI Plan. The Plan Builder includes example content that

can be edited or replaced to guide you in developing a plan that is unique to

your organization.

● The completed QAPI Plan can be reviewed, saved, and printed upon request.

New! QAPI Plan Help Doc

Corporate Admin and QA Admin Users will find this step-by-step guide a helpful

resource in generating a QAPI Plan.


Improved! Market Analysis Map Improvements

Market Analysis maps will now load faster and will automatically locate on the center

from which you launch the map in abaqis.
