EMPLOYER CONTRIBUTION PLAN

To provide the best benefits at the best value,Abacus has developed a program where youincrease your group’s health benefits deductiblesand co-pays and free up dollars to help pay forthe Abacus Medical Support Insurance Planavailable through Kansas City Life InsuranceCompany. It provides more first-dollar benefits ata lesser cost – a win for both you and youremployees.

Or, you may choose to leave your current benefitplans in place and still provide the Abacus Medical Support Insurance Plan as an addedbenefit for your employees.

VOLUNTARY BENEFITS PLAN

As the cost of health insurance continues toescalate, you may think your only alternative tohigher premiums is shifting some of the addedcosts to your employees – through increaseddeductibles and plan co-pays. The AbacusMedical Support Insurance Plan providesemployees with the option to purchase avoluntary, supplemental plan that may helpreduce the out-of-pocket hospital deductibleswith a First Day in Hospital benefit.

No one plans to get sick or injured –but your employees can be prepared if ithappens to them.

Your employees and eligible dependents are presently covered under your Group Medical Insurance Plan, withbenefits that are mandated through the Patient Protection and Affordable Care Act (PPACA). Your coverage hasdeductibles and co-pays that may require employees to pay as much as $6,350 out of pocket for an individual and$12,750 for a family.

You can’t stop medical costs from rising. However, you can help your employees manage the costs they will incurthrough deductibles and co-pays by offering the Abacus Medical Support Insurance Plan.

Protect their savings with a benefit that can be used to cover deductibles and co-pays.

HIGH OPTION

First Hospital Confinement *Pays the benefit amount for an Insured’s first hospital confinement as listed per option. Benefits forthe rider will be limited to the First Hospital Confinement each Calendar Year for each insured,(this includes one continuous Hospital Confinement or several Hospital Confinements for the sameor a related cause which are separated by less than 60 days from date of discharge). This benefitis not a cumulative benefit and will not exceed the maximum amount listed:

Low Option: Day 1, $750 Day 2, $1,500 Day 3, $2,250 Day 4, $3,000High Option: Day 1, $1,000 Day 2, $2,000 Day 3, $3,000 Day 4, $4,000

SurgicalPays the benefit listed in the Surgical Schedule for surgery performed by a physician if due toa covered injury or sickness, based on the tiers. (If more than one surgical procedure isperformed at the same time, only one benefit – the largest – will be paid.)

Anesthesia(Pays 25% of the Surgical Benefit for Anesthesia administered by a Physician in connectionwith such surgical procedure.)

Emergency AccidentPays a specified benefit amount for emergency care due to a covered injury that is rendered by aphysician in a hospital emergency room or physician’s office within 72 hours of the injury. Paymentwill be made for up to four such emergency care treatments in a calendar year per insuredcategory. (Four for employee, four for spouse, and a total of four for all children.)

Daily Hospital ConfinementPays a daily benefit for hospitalization (resident bed patient) due to a covered injury or sickness upto 180 days of confinement. Benefit begins on the first day of confinement and continue whilehospitalized until the benefit period expires.

SURGICAL RIDERSSchedule of Operations

If a physician performs surgery due to Injury or Sickness while the Rider is in force for the Insured, we will pay a Surgical Benefit. If an operation is not listed in this Schedule, we will find acomparable procedure, similar in severity and complexity, and pay that amount. If two or more surgical procedures are performed simultaneously through the same or different incisions,only one benefit -- the largest -- will be paid. (Coverage is for In Hospital or Out of Hospital). Maximum benefit is $2,500.
