Momyer, CIH. Mr. Mammarelli, a navy veteran, received his Master’s Degree from the University of Pittsburgh in Public Health. He was well known and highly respected for his integrity and love for the industrial hygiene and safety profession, especially in the steel and metals industry. Mr. Quinn, President and Chief Executive Officer, is a Certified Industrial Hygienist (CIH) in comprehensive practice. He brings to the firm more than 30 years of experience in the fields of industrial hygiene, safety, product safety, environmental control, and radiation protection on a nationwide and international basis. Mr. Quinn has extensive working experience and knowledge in all aspects of the environmental health and safety fields. Mr. Quinn has over twenty years of experience developing and managing a successful health, safety, and environmental consulting practice. He developed his professional skills as a practicing industrial hygienist with eight years in the steel industry and three years in the chemical industry before starting, AM H&S’s predecessor firm, Astorino Branch Environmental, Inc. in 1990 as its president. Mr. Momyer, Executive Vice President and Chief Operating Officer, is a Certified Industrial Hygienist (CIH). He specializes in health and safety management and brings more than 20 years to the firm. Mr. Momyer’s specific knowledge of the steel and metals industries, his extensive exposure to varied manufacturing processes, and hands on safety and health expertise provides a vital resource to our company and our clients. She also has experience in regulatory and environmental compliance, asbestos, hazard/risk evaluations and hazardous waste operations. In addition, Ms. Cummings holds an M.S. Degree from Duquesne University in Environmental Science and Management, and is an OSHA-Authorized Trainer for General Industry. Mr. Jason Flory, CIH is the Director, Midwest Regional Operations and brings to the firm over ten years of health, safety, and radiation protection experience, including: industrial hygiene, air, water and noise assessments; radiation dose assessments, surveys, and leak testing; coordinating and evaluating safety/health consultants and contractors; and involvement with engineering projects as they relate to OSHA regulatory guidelines. In addition, Mr. Flory brings practical and effective knowledge of occupational safety and health programs and program management, as well as education and training, from his ten-year career as a United States Air Force officer, retiring from service with a rank of Major.

Mr. Paul N. Pantages, CSP, Manager-Safety Services, brings to AM H&S more than 35 years of experience in the field of safety management, loss prevention, hazardous materials response/handling, emergency response, fire prevention and construction safety. In addition to his health and safety experience, Mr. Pantages is a Pennsylvania State Fire and Rescue Instructor and Hazardous Materials Instructor.

Mr. Tyrone Snyder, Manager-Safety and Health Services, brings over 10 year experience in the field of occupational health and safety along with experience in monitoring for occupational exposure to potential airborne contaminants, noise and heat. He has conducted industrial hygiene hazard assessments and confined space evaluations, developed industrial hygiene sampling plans/strategies and development and implementation of employee training programs.

Our Health and Safety Associates are required to have a BS in safety sciences or related topic from a four-year college or university; or one to five years’ related experience and/or training; or equivalent combination of education and experience and/or successful completion of an industrial hygiene and safety internship. In particular, our industrial hygienists are preferred to have a BS in industrial hygiene or a basic science. Regardless of academic background, our industrial hygienists receive extensive indoctrination training upon being hired in the discipline of industrial hygiene in that each is required to complete an on-line basic certificate program from the Ohio University and an on-line course from the Ohio Bureau of Workers Compensation; complete the AM H&S fundamentals of industrial hygiene course/standard operating procedures review; and receive mentoring from our certified industrial hygienists. Health and Safety Associates also complete either the OSHA ten-hour training course in General Industry or Construction as well as client-specific or industry-specific training. In addition, our occupational nurses have a nursing degree and/or are registered nurses. A training matrix is maintained for all our professionals in order to ensure not only their development but to ensure that the most experienced, and qualified professional is assigned to the appropriate client project team.

AM H&S is committed to providing continual and appropriate professional development training for all of our team members in order to bring this knowledge to our clients. Furthermore, our professionals are encouraged and required to be active in professional organizations such as the American Industrial Hygiene Association (AIHA) and American Society of Safety Engineers (ASSE) as well as professional industry specific trade associations. Our professionals are also required to receive training in the fundamentals of project management and contractual liability.

In addition to our full-time staff, AM H&S has a cadre of experienced part-time, semi-retired employees or consultants, with a significant amount of experience as safety professionals, supervisors, managers, and/or as union safety coordinators or maintenance professionals. QUALIFICATIONS AND CLIENT PROJECT APPROACHAM H&S will guarantee to provide you with the best health and safety professionals for your project. A proposed project team will be selected according to relevant experience and technical capability to meet or exceed your project requirements. The unique AM H&S Client Project Delivery Approach involves the assignment of a specific Project Team Manager/Leader for every individual project. This professional is responsible for the completion of the project as well as working with the Client Leader and Client Principal-in-Charge/Owner to staff the project team with the most appropriate, experienced professionals to accomplish the client’s project needs and goals. A specific Client Leader is assigned to every AM H&S client. The Client Leader is the main or central contact for the client for all professional work or projects being performed, scheduled or potentially being scheduled for the client, regardless of the specialty being performed; i.e. safety, industrial hygiene, environmental, radiation safety, toxicology, engineering, ergonomics, risk management, etc. In this fashion, the client need only contact one person to obtain updates or schedule additional work. The Client Leader may be also function as a Project Leader for a specific project for the same client. Furthermore, one of the owners or principals of AM H&S is assigned to each of our clients as a Principal-in-Charge such that the client always has a direct line of contact with an owner. The Client Leader and Project Team Leaders will continuously update the Principal-in-Charge as to the status of projects being performed for the client. In addition, the Principal-in-Charge shall assure client service and deliverables meet client expectations as well as ensure that deliverables are appropriately reviewed for quality assurance and quality control. Furthermore, the Client Leader, Project Team Leaders, and Principal-in-Charge will work closely with the client on business development with the client. Our assigned professionals are provided lap top personal computers and cellular communication phones\devices along with information technology support and software to assist in rapid communication, client contact and completion of deliverables.

In addition to the project specific hard copy and electronic deliverables, all industrial hygiene and analytical data is entered into our computerized tracking system that allows for long term data storage/recordkeeping and statistical treatment. Thus, we can arranged for our clients to access their data, which is stored on the AM H&S industrial hygiene and analytical computerized tracking system, through the AM H&S web site.

BERNARD J. QUINN, CIH, MSPresident / Chief Executive Officer

With more than 30 years in the fields of industrial hygiene, safety, product safety, environmental control, and radiation protection at both the plant and corporate levels, Mr. Quinn, as President and Chief Executive Officer of AM H&S, is responsible for the overall management and direction of the firm as well as taking the lead on sales and marketing efforts. In addition he is responsible for the development, coordination, and performance of a wide range of field industrial hygiene and environmental surveys, as well as safety and health audits and inspections. He also creates and conducts educational presentations and seminars on industrial hygiene health hazards and provides expert witnessing on a nationwide and international basis. Mr. Quinn is personally involved in all health and safety projects, especially in quality assurance aspects. He is an active member of the Steel Manufacturers Association, the Association of Iron and Steel Technology, American Industrial Hygiene Association, American Society of Safety Engineers, and the Western Pennsylvania Engineers Society. He was a Board member of the former Western Pennsylvania Safety Council. He is a Board Certified Industrial Hygienist in Comprehensive Practice. In addition, Mr. Quinn has extensive experience with Phase I and Phase II Environmental Site Assessments, environmental health and safety due diligence/feasibility studies, along with comprehensive indoor air quality investigation.

During his career, Mr. Quinn has been a Senior Industrial Hygienist, Coordinator of Radiation Protection, Environmental Health Engineer in industry as well as a senior principal, founder and President of two successful consulting safety and health manager, firms. He has developed and managed extensive asbestos abatement programs; written, developed, implemented, and managed a corporate radiation protection program for the use of industrial isotope and x-ray devices; and coordinated the transfer and disposal of more than 500 radioactive sources. He has also been the plant-level safety and health manager for a stainless steel finishing facility. Prior to AM H&S, Mr. Quinn served as senior principal, founder and President of the Environmental Division of Astorino, where he became involved in all environmental projects especially in quality assurance aspects. He oversaw all business development for the Environmental Division. As a senior principal, Mr. Quinn was involved in corporate aspects affecting the architectural and engineering firm as well as the environmental division. Mr. Quinn has extensive working experience in all aspects at the plant and corporate level, as well as a thorough knowledge of the steel, metals, manufacturing, utilities, construction, and other related industries.

York International Corporation (now Johnson Controls, Inc.)York, PAPart of the corporate health and safety team with the goals of providing a safe and healthy work environment for 20,000 employees globally; helping to reduce losses; improving regulatory compliance; and assisting with the overall quality process being implemented by York management.

Services that AM H&S is providing includes audits and assessments, employee training, program development, industrial hygiene exposure surveys and data management. In addition, AM H&S assists with health and safety strategic planning, regulatory reviews and interpretations and provides routine support for field health and safety functions.

Army Corps of EngineersNeville Island, PADeveloped a health and safety plan for the removal of asbestos containing materials from a sunken barge and boat. Project included water testing upstream and downstream for oils and PCBs that may have been released by the vessels. Working as subcontractors, AM H&S served as project manager, coordinating the entire remediation effort.

Housing Authority City of Pittsburgh Pittsburgh, PAPerformed identification, testing, analyses, X-Ray Fluorescence (XRF), abatement specifications, product evaluation, inspections, oversight, observations, project management, bid reviews, and risk assessments at over 10,000 units for the Housing Authority of the City of Pittsburgh (HACP).Activities in all abatement procedures from project initiation through final clearance were monitored. ArcelorMittal USA (formerly ISG, LTV)Cleveland, OH, Columbus, OH, Monessen, PA, Warren, OH, Indiana Harbor, IN, Riverdale, IL, and Weirton, WVAs part of a master service agreement, AM H&S provides a number of environmental health and safety services to ArcelorMittal. AM H&S conducts asbestos-containing material (ACM) surveys and reviews lead-based paint (LBP), drinking water quality, radon, IAQ, training, and IH surveys.

Superior ValveWashington, PATo evaluate employee exposures to airborne metals, acids, or dusts during specific machinery operations, AM H&S conducted an industrial hygiene survey for possible airborne contaminants. AM H&S monitored airborne phosphoric acid and metal exposure during soldering operations, and airborne oil mist and metal exposure during machining operations.

Routinely provided a plant wide industrial hygiene air contaminant and noise survey for Wheeling-Pittsburgh Steel.

J & L Structural, Inc. - Safety TrainingAliquippa, PATo ensure compliance with Occupational Safety and Health Administration (OSHA) regulations, AM H&S provided safety training to over 300 supervisors and hourly employees at J & L Structural, Inc.’s Aliquippa and Ambridge Plants. Training was held in 15 module sessions and covered topics such as forklift operation, overhead crane safety, noise protection, hazard communications, and accident prevention.

Koppers Inc. Pittsburgh, PAAM H&S performs Industrial Hygiene studies to evaluate respiratory and personal protective equipment (PPE) requirements for various tasks at Koppers twenty plus facilities. The surveys consist of long and short-term exposure assessments throughout the facility. The findings are compared to the established permissible limits, and recommendations were presented and integrated into Job Safety Analysis (JSA).

Mr. Momyer is the Vice President and Chief Operating Officer for AM H&S. He has over 18 years of experience emphasizing in health and safety management. ATI Flat Rolled ProductsCorporate WidePrincipal in charge of providing a full scope of industrial hygiene services for ATI Flat Rolled Products, a large stainless steel manufacturer. U. S. Ecology Oak Ridge, TNAM H&S provided U.S. Ecology with an eight-hour PCB training course on hazards and regulations, covering aspects such as hazard training and disposal.

In addition, we reviewed a U.S. Ecology health and safety plan and revised it to be project-specific for the State of New York. For this project, AM H&S will also serve as site project manager for health and safety.

Mini-Mill Steel Manufacturer Southern USAM H&S performed an industrial hygiene and safety audit of eight steel facilities, including operations such as electric-arc steel making, casting, hot rolling, scrap processing and shipping.

Ormet CorporationHannibal, OHAM H&S performed a mock-OSHA inspection at Ormet Corporation, a leading supplier of aluminum products. A team of five AM H&S specialists spent four 12-hour days on site, and identified items of non-compliance, which were presented to Ormet in a written report.

Mr. Paul Pantages, CSP, Manager-Safety Services for AM H&S, is responsible for the performance of all safety services, as well as the continuing development of these services. Mr. Pantages has more than 30 years in the field of health & safety management, and has experience in the areas of safety program development and implementation, OSHA compliance, NFPA and fire protection, energy control, confined space operations, workers compensation and industrial hygiene. Mr. Snyder has over ten years of experience in the health and safety profession. In his present capacity, he is responsible for supervising, directing, coordinating, and at times performing professional industrial hygiene and safety services for general industry, commercial and construction clients and projects. Tyrone returns to AM Health and Safety, Inc. from Allegheny Ludlum where he served for four years as a Senior Safety and Health Administrator. Mr. Snyder’s last assignment while at Allegheny Ludlum, involved the responsibility for the safety and health of over 100 employees on a 66 million dollar melt shop construction project. One of the most important components of any contract service project is contractor oversight. When contract services are well thought out and executed, the project can be completed on time and within budget; however safety is a key element in any contract process. Accidents can cause both costly and significant delays to a project. AM H&S provides contractor oversight services to assure that all contractor work performed is in accordance with acceptable work practices. These practices are identified by the client as well as by all federal, state, and local regulatory requirements. The need for owner involvement directly or using a firm, such as AM H&S to track the contractors performing work at the client site, is a true financial liability concern.

The magnitude and extent of third party construction safety and health observation varies according to the owner’s needs and requirements. Below is an outline of various tasks associated with third party construction safety and health observation services. Prequalification of ContractorsAM Health and Safety can provide professional support to help the owner identify contractors who are compatible with their operating safety principles by using a selection process which evaluates performance criteria: EMR’s, injury rates, programs, and competency. Injury and Illness Record ReviewTo assure that programs provided by the contractor are adequate, AM H&S review the contractor’s Occupation Safety and Health Administration (OSHA) injury and illness records. In the review process, AM H&S looks for trends in injury/illness types to determine if there are pre-existing conditions that may lead to increased costs to the client. For example, if there are a number of repetitive motion injuries and task being performed is repetitive in nature, AM H&S will advise the client of the potential issue and work to minimize the risk potential.

Field Audits and Inspections Site visits are conducted to assure compliance with appropriate rules and regulations. These site visits can be random and/or scheduled. A list of deficiencies are identified and provided to the contractor and the client for corrective action. Copies of all reports and actions by the contractor are tracked by AM H&S until corrected.

Program Development To assure compliance with appropriate federal, state or local requirements, AM H&S works with the client to review and/or develop programs and procedures that may be necessary. An example of these services may be the development of an excavation program for an underground tank removal, a unique confined space entry procedure, or lead paint removal procedure.

Training and Education As directed by the client, AM H&S can provide training and educational programs. Programs covering topics such as general safety requirements, biological hazards, and hazard communication can be provided to all contractors and their on-site employees.

Incident Investigation Support AM H&S provides incident investigation services ranging from conducting independent or team investigative services, root cause analysis, report coordination and corrective action follow up for all or designated incident types. These services can be provided under the Attorney/Client Privilege if necessary.

Exposure Monitoring The nature of a specific task may warrant exposure monitoring of the contractor, background monitoring of the contractor, or background monitoring in the general work area to assure that the contractor and client employees are not being exposed to unnecessary harmful health hazards.

Additional Support/ Staff Augmentation Depending on the clients staffing needs AM Health and Safety can provide a safety and health professional on site to coordinate the contractor safety program.

Training in CPR/First Aid and AED Depending on the clients needs AM Health and Safety can provide training to meet your medical requirements with a selection of CPR, CPR with First Aid, or CPR with First Aid and AED. AM H&S clients can select any of the services outlined above that are appropriate for the task being performed by the contractor. AUDITS/INSPECTIONS

Audits and inspections provide a record of the company’s performance, demonstrate that rules and/or policies have been established and enforced, and confirm that professionals train employees. The goal of a company’s health and safety program is protecting employee health and safety, complying with federal, state, and/or local programs and controlling direct and indirect costs (such as workers compensation).

AM H&S’s professional staff possesses extensive knowledge of the health and safety regulations. Our professionals have skills in investigating, collecting the appropriate information, and interviewing personnel. AM H&S audit or inspection leaders possess certifications as safety professionals and/or industrial hygienist.

Health and Safety Audits (Program or Regulatory Compliance) A health and safety audit provides a record of overall condition of facility and management systems/programs as well as the strength and weaknesses of these programs. Many times audits are conducted under attorney/client privilege, either for the company’s in-house legal department or outside counsel. A health and safety audit allows companies to examine their policies, practices, and actions regarding employee health and safety. Health and Safety audits also evaluate the financial impact of both direct and indirect costs to the company in issues such as workers compensation costs.

Our health and safety professional team can provide a simulated OSHA inspection of the client’s facility, which can include an opening conference with management and labor. INDUSTRIAL HYGIENE SERVICES

The basis of industrial hygiene is to protect worker health and in turn reduce the liability to employers through recognition, evaluation, and control of exposures to workplace hazards. AM H&S industrial hygienists have the technical skills and experience required to solve the environmental concerns that may arise in today’s workplace. Our industrial hygiene team members are trained professionals with extensive experience in the recognition, evaluation, and control of workplace hazards.

All industrial hygiene projects are performed under the direct supervision of professionals Certified by the American Board of Industrial Hygiene (CIH) and/or the Board of Certified Safety Professionals (CSP). All team members are required to be actively involved in the American Industrial Hygiene Association and to remain current with all-applicable regulations, codes, standards, and guidelines as well as pursuing their certifications.

In order to provide the most cost-effective and innovative solutions, AM H&S industrial hygienists become part of their client’s team by attempting to understand the client’s processes and work environment.

Process Review & Preliminary IH AssessmentThe industrial hygiene process begins with the discussion and review of the manufacturing process; understanding the process flow; identifying the hazardous components of raw materials, products, and by-products or wastes; identifying worker interface with the contaminants; determining the nature or extent of complaints; and defining a sampling strategy to determine the exposures.

EvaluationThe method or methodologies for properly collecting and analyzing the contaminants are determined by the IH and CIH in conjunction with the laboratory and current industry/government standards. Depending on the nature of the process and contaminant in question, sampling may include full-shift, short-term, task-specific, area, and/or grab or instantaneous sampling.

Reporting and Recommending ControlsAM H&S provides detailed, quality reports to clients. The reports include an executive summary, background information, criteria or regulatory references, descriptions of tasks performed, conclusions or findings, and recommendations designed to control the environmental factors. Daily monitoring sheets with field notes, actual laboratory analytical results, relevant MSDS, CADD quality drawings depicting sample locations as well as process layout, posting tables, summary tables, and other pertinent data are part of the written industrial hygiene reports. AM H&S can provide the experience of design engineers to assist in the development of practical, workable local exhaust ventilation system design or noise control design. AM H&S works closely with the client to incorporate the most practical method or combination of methods (engineering, administrative and/or personal protection) to reduce exposures.

IH Staff Augmentation and IH AnalysesFor clients having industrial hygiene departments or managers, AM H&S can provide our experienced industrial hygienists to supplement a client’s needs. An example may be providing a professional to only collect exposure samples at the direction of the client.

In addition, due to the quantity of samples collected by AM H&S, the laboratory fees we obtain from our 3rd Party, American Industrial Hygiene Association (AIHA) accredited labs are low. An integral part of a hazard communication program is writing, reviewing and approving Materials Safety Data Sheets (MSDS). RADIOLOGICAL SAFETY SERVICES

A quality radiation safety program is determined by how well an organization can take existing regulations and apply those requirements to the specific needs of their organization. Throughout this process, a licensee may have a written radiation safety program designed to promote the safe, effective use of radioactive material in their normal day-to-day requirements. These activities require enormous attention by facility radiation safety officers to ensure exposures to their employees are as low as reasonably achievable (ALARA) and to ensure emergencies involving radioactivity are controlled.

AM H&S provides professional health physics support services designed to assist licensees in ensuring compliance with federal, state and local laws. The addition of the AM H&S team to a company’s radiation safety department increases productivity, training, and safety of personnel, as well as increased credibility with management, employees, and organized work forces.

AM H&S offers a wide variety of radiation safety services that are formulated to meet the needs of all industrial, academic, and healthcare facilities. AM H&S’s wide range of services are designed to maximize the efficiency of use by allowing our customers to select any individual task or a combination of tasks to be performed.

Safety Program ReviewWe review the customer’s radiation safety program, policies, and procedures to assure compliance with appropriate rules and regulations.

Field Audits and InspectionsRandom and scheduled site visits are conducted to assure compliance with appropriate rules and regulations. A list of deficiencies are identified and provided to the customer for corrective action. Copies of all reports and actions by the customer are tracked by AM H&S until corrected.

Program DevelopmentAM H&S works with the client to review and/or develop programs and procedures, which may be necessary to assure compliance with appropriate federal, state or local requirements. An example of these services may be the development of a facility’s radiation protection manual, or portions thereof, such as emergency procedures, lockout/tagout procedures of radiation producing devices and waste management procedures.

Sealed Source Leak Test/SurveyPeriodic leak test of sealed sources are required of all licensees of sources utilized at customer facilities. AM H&S will conduct wipe tests of sources for analysis at independent laboratories, and will perform radiation surveys of sealed source radiation producing devices in accordance with manufacturers’ specifications and current regulations.

Training and EducationAM H&S provides training and educational programs as directed by the client to maintain compliance with all initial and refresher training requirements.

INDOOR AIR QUALITY

Indoor Air Quality (IAQ) problems can arise from a number of sources such as contaminated air being pulled into the building; emissions from office equipment such as copiers or blue print machines; improperly maintained HVAC systems; human activities such as smoking, housekeeping activities, and maintenance activities; and emissions from building components and furnishings. The use of IAQ services in a building will protect against owner liability; reduce employee sickness and absenteeism; and improve overall building maintenance and production.

AM H&S has trained teams to perform IAQ investigations with a combination of multi-disciplined services and the use of state-of-the-art equipment to ensure that indoor air quality problems will be evaluated and solved in a comprehensive, efficient, and cost-effective manner.

To identify and eliminate existing and/or potential air quality problems, the IAQ team at AM H&S reviews all records and mechanical drawings relevant to indoor air quality; interviews employees; inspects the facility; conducts long- and short-term air sampling for airborne contaminants such as dust, relevant volatile organic compounds, formaldehyde, and bioaerosols; examines and evaluates building HVAC systems; and performs basic IAQ testing for comfort levels. Basic IAQ testing consists of the collection of carbon monoxide (CO), carbon dioxide (CO2), relative humidity, and temperature utilizing a hand-held direct reading instrument to log results.

Upon completion of these steps, the team compiles and analyzes lab data and building information, creating a report of administrative and engineering recommendations to promote safe and healthy indoor air quality. In addition, the IAQ team recommendations meet guidelines for interpreting ventilation data established by the American Society of Heating, Refrigerating, and Air Conditioning Engineers (ASHRAE) Standard 62-1989 “Ventilation for Acceptable Indoor Air Quality.”

In order to efficiently investigate IAQ problems, the causes must be understood. The National Institute of Occupational Safety and Health (NIOSH) performed a study of 500 federal buildings. Of all the known causes of IAQ problems, 52 percent were inadequate ventilation, 16 percent were pollution sources inside the building, and 10 percent were sources outside the building. For this reason, airborne contaminant sampling may not always be the primary step. Due to the complexity of indoor air quality and the diversity in each case, the following IAQ investigation strategies are utilized as guidelines that are modified to each specific building. Preliminary IAQ InvestigationA Preliminary IAQ Investigation may involve: Discussions with property/building management; Evaluation of concern; Walkthrough investigation of building and surrounding areas by an industrial hygienist; Discussions with HVAC maintenance—drawings, review of maintenance schedule, preventative maintenance program; Preliminary observation/review of the HVAC system; Direct readings of carbon monoxide, carbon dioxide, relative humidity, and temperature for a general review of the indoor comfort levels, ventilation system efficiency, and possible causes of building occupant

health complaints; Determining potential indoor and outdoor sources of indoor air pollution; Obtaining relevant material safety data sheets (MSDS); and Presenting report of findings with Phase I IAQ survey strategy, if necessary.

Phase II IAQ SurveyIn addition to the services of a Phase I IAQ Survey, a Phase II IAQ Survey may involve: A definitive review of HVAC system and drawings by a mechanical engineer; Follow-up occupant questionnaires with detailed occupant interviews; CADD quality drawings of HVAC systems and other indoor pollutant sources as necessary to illustrate indoor

source pollution; An industrial hygienist on site for three to four full shifts to observe work practices of people working inside

and in the vicinity of the building; A report with findings and recommendations; A client meeting with management to discuss report; and An IAQ survey follow-up meeting with building occupants and building management to explain the purpose

for the investigation, findings, and proposed solutions.

DATA MANAGEMENT

The most integral part of evaluating the effectiveness of an employee health and safety program is reporting and recording procedure. Methods vary from manual filing systems to sophisticated computer systems. Without adequate records, system compliance with local, state, and federal regulations is difficult, if not impossible, to maintain. However, with the proper tools, a data management system is efficient, compliant, and cost effective.

As part of a full line of services, AM H&S offers data management services including Materials Safety Data Sheets (MSDS), exposure assessments, chemical inventories, and other services aimed at tracking programs and activities in the work environment. ASBESTOS MANAGEMENT

The presence of asbestos poses health, financial, and legal concerns for building owners. Building owners must develop pragmatic, effective asbestos management programs to insure public safety, avoid reduction of property, and prevent costly lawsuits and medical judgments. By providing careful inspection documentation, safe abatement procedures, and final clearance, the timely use of asbestos management services can prevent or limit asbestos-related litigation by employees or tenants, as well as regulatory fines by the Occupational Safety and Health Administration (OSHA), the Environmental Protection Agency (EPA), or local agencies. These programs include operations/maintenance procedures, and abatement alternatives.

AM H&S provides a full-service asbestos management program, including a review of the building's background, a collection of bulk samples of suspected asbestos-containing building material (ACBM) and air samples, and a documented report of findings and recommendations. If abatement is necessary or mandated, AM H&S will develop and design specifications (including requirements for personal protective equipment, area containment and hazardous waste containment) outlining the steps that must be followed to ensure safe abatement of all asbestos. In addition, training for maintenance and custodial personnel is provided to insure compliance of asbestos-related regulations. AM H&S works closely with the client to ensure the most appropriate abatement alternative is recommended.

Job Bidding and Contract AdministrationJob bidding and contract administration services include sending requests for proposals and reviewing bids.

Removal Observation and MonitoringAM H&S will observe contractors to ensure that abatement procedures are carried out safely and effectively in accord with the contract document.

Final Site InspectionUpon completion, AM H&S will conduct a final site inspection to ensure that the asbestos containing materials are properly abated.

LEAD-BASED PAINT MANAGEMENT

The Occupational Safety and Health Administration (OSHA) has developed criteria to protect construction employees from the potential exposure of lead-based paint, dust, and/or fumes during renovation activities. AM H&S’s lead-based paint management services include testing, abatement design, risk assessment, and encapsulant product development, as well as assisting the owner in preparing request for proposals (RFPs) for consultants and abatement contractors to provide lead-based paint design, testing, and abatement. In addition, public housing and military housing are required to comply with not only OSHA but also the strict Housing and Urban Development (HUD) lead regulations.

ENVIRONMENTAL SERVICES

The Environmental Response, Compensation, and Liability Act (CERCLA) is an act which imposes a legal duty on any current or past owner to clean up a contaminated site, whether or not that owner was responsible for the contamination. Reinforced by the Pennsylvania Special Water Pollution Regulation, prospective property buyers must undertake “due diligence” to determine potential environmental hazards on the proposed site. As a result, potential buyers need reliable and thorough comprehensive environmental site assessments to protect themselves against liabilities for possible pre-existing hazardous materials.

For reliable investigations of possible contamination, and for official documentation of property conditions and remediation services, AM H&S offers Phase I, Modified Phase I, Phase II and Phase III Environmental Site Assessments (ESAs). Phase I and Phase II ESAs notify potential buyers of property contamination problems before purchase, insuring that the buyer will avoid liability and/or costs associated with cleanups. Phase III services aid the property owner in cost-effective remediation efforts.

Phase I Environmental Site AssessmentPhase I Environmental Site Assessments are inquiries into past uses and present conditions, which may have created environmental problems for which the current owner may be liable. A typical Phase I ESA involves: Reviews of maps and aerial photographs to determine areas of possible environmental concern; Reviews of title history, available regulatory compliance records and site development drawings or records

identify potential or existing problems; Investigations of the historical uses of adjacent properties to reveal their potential or existing impact on the

site; Interviews with individuals and agencies familiar with the site to identify possible problems not revealed in

documents; and Visual site surveys to reveal such possible environment degradation indicators as stressed vegetation, leachate

seeps, ground staining, or filled areas.

After examining information gathered from document reviews, interviews, and visual surveys, AM H&S submits a final report to the client. If applicable, the report may include recommendations for further investigation.

Modified Phase I Environmental Site AssessmentModified Phase I ESAs include all Phase I processes along with isolated testing for such environmental contaminants as asbestos, lead-based paint, and soil-based heavy materials or hydrocarbons.

Phase II Environmental Site AssessmentBecause the goal of Phase II ESAs is actual contaminant qualification, Phase II Environmental Site Assessments/Impact Studies include more intrusive testing. Phase II services include: Itemization of hazardous materials, including type, quantity, and storage method; Evaluation of site waste management materials; Identification of polychlorinated biphenyl (PCB) or possible PCB equipment; Assessment of underground storage tanks and evaluation of tank conditions and management practices; Inventory and assessment of possible asbestos-containing materials, including sampling and laboratory

analysis; and Sampling of soil and water for suspected contaminants.

A completed Phase II Site Investigation will supply vital information on site contamination, specific contaminated areas, and alternative remediation recommendations.

Phase III Environmental Site Remediation DesignAM H&S’s Phase III Site Remediation Services will be needed if a Phase II Assessment indicates the presence of contaminants that require remediation: asbestos, lead-based paint, underground storage tanks (USTs), PCBs, soil/groundwater contamination, and hazardous waste containers. Phase III services include: Review of owner’s plans for site contamination remediation; Creation of remediation contract documents, including AM H&S-prepared working drawings and job

specifications; and Oversight of all remediation efforts.

With all AM H&S site assessment team members certified in asbestos and lead bulk sampling, and because of their thorough training and extensive experience, AM H&S can accurately document all relevant property conditions in accordance with all necessary federal, state, and local hazardous materials laws. Quality Assurance/Quality Control (QA/QC) Procedures

Rigorous Quality Assurance/Quality Control (QA/QC) procedures are in plant at AM Health and Safety, Inc. (AM H&S), to ensure the integrity and compliance of procedures used during inspection and air monitoring of industrial hygiene projects including asbestos abatement projects. The QA/QC protocol established by AM H&S followed in two main areas:

1. Laboratory QA/QC2. Field Sampling & Inspection and Report QA/QC.

Laboratory QA/QCThe Laboratory QA/QC portion centers around the procedures developed and implemented by the contractual 3 rd

party laboratories hired by AM H&S to perform appropriate analysis.

AM H&S utilizes third-party laboratories that are successful participants in the NIOSH Proficiency Analytical Testing (PAT) Program sponsored by the American Industrial Hygiene Association, fully accredited by the National Institute of Standards (NIST) under the National Voluntary Laboratory Accreditation Program (NVLAP), and fully accredited by AIHA for asbestos, metals, organic vapors, environmental lead or other appropriate qualifications.

The qualification information including summaries of their respective QA/QC programs can be provided upon request. The three main industrial hygiene laboratories utilized by AM H&S are: Analytics Labs, RJ Lee Group or Analytics. In addition, the two main laboratories utilized by AM H&S for microbiological analyses are Aerobiology and U.S. Micro Solutions.

In addition to the laboratories in our QA/QC program, the Certified Industrial Hygienist (CIH) from AM H&S will review the laboratory and field blank results on a routine basis in order to identify any unusual trends. These unusual trends will be immediately brought to the attention of the Laboratory Manager.

Field Sampling for further investigation & Inspection QA/QCAM H&S has an organizational system that facilitates the consistent production of reliable sampling and project oversight inspection. Individuals are assigned responsibilities within their area of technical and managerial expertise. The duties and responsibilities of the CIH and associates of this project are defined.

Standard Operating Procedures have been developed for the critical areas associated with IH field surveys and asbestos abatement project management. These are calibration procedures, project over-sight inspection guidelines, and Associate Booklet. AM H&S’s industrial hygienist review the OSHA field manual for conducting industrial hygiene sampling along with the AM H&S IH sampling guidelines prior to the start of any survey.

To ensure the integrity of samples taken, a strict chain-of-custody record is maintained on each sample. At the time of sampling, a chain-of-custody form is completed by the associates and accompanied by a group of samples. The chain-of-custody form serves as a record of the samples’ progress from the point of collection to the laboratory, where the sample is analyzed. The form also includes a unique Bulk Sample Identification Number, and requires the signature of the Health & Safety Associate, the laboratory staff member who accepts receipt of the sample, and any other individual who handles the samples. The chain-of-custody from remains attached to the group of samples to be analyzed. When the samples are not in anyone’s direct possession, they are kept in a secure location.

On arrival at the laboratory, the laboratory staff member who accepts receipt of the samples logs them in and verifies receipt on the chain-of-custody form. After sample analysis is complete, the chain-of-custody form is kept on file at the laboratory with a notation of the final disposition of each sample. It is the policy of AM H&S to maintain all air and bulk samples for a period of 30 to 45 days from the date of analysis.

Specific QA/QC supervisory responsibilities of the project leader will be to review all field monitoring and inspection forms of the associates on a weekly basis. The project leader will observe the health and safety associates performing field calibration of air sampling equipment and will accompany the health and safety associate inside containment of work area during inspection at least weekly.

In turn, the CIH will review observations of the project leader on a weekly or project basis. At least once a quarter, the CIH will conduct a field inspection of the work site to review the associate’s data and procedures. Furthermore, the CIH will review air sample results daily as well as review the laboratory and field blank results as previously stated. All reports of the project leader will be reviewed and approved by the CIH prior to issuance.

The Project Team Leader reviews all deliverables and a final, detailed QA/QC review is performed by the appropriate CIH or CSP or CPE and one of the Principals/Owners prior to release to the client.
