A Review of Plant Species Used to TreatConjunctivitis

Conjunctivitis is a major occular external infection in tropical countries. Although not a very serious dis-ease, it gives much discomfort and sometimes leads to partial blindness or blindness due to corneal invol-vement. Conjunctivitis has been known to occur in epidemic proportions in different parts of the world,mainly African and Asian regions. The indigenous cure of conjunctivitis using herbal products has beenpopular for centuries. This paper is an attempt to compile information on plants mentioned as a cure forconjunctivitis during ethnobotanical surveys between 1933 and 2000. Plants mentioned as a cure in var-ious systems of medicine, namely Siddha and Ayurveda, are also included. Copyright � 2002 John Wiley& Sons, Ltd.

INTRODUCTION
Conjunctivitis is an inflammation of the conjunctiva(mucous membrane covering the anterior portion of theeye ball, reflected upon the lids and extending to theirfree edges). Types include allergic, primary irritant orvernal conjunctivitis. Conjunctivitis can also be asso-ciated with Reiter’s syndrome, a form of arthritis. Inconjunctivitis, the eyes become red and the eyelids swelland stick together while sleeping. Common symptoms ofconjunctivitis are rapid onset of redness, burningsensation, itching, grittiness, photophobia, increasedlachrymation, swelling of the conjunctiva and lids, andvisual disturbances (Nayak et al., 1987: Leino et al.,1992). Conjunctivitis occasionally causes serious neuro-logical complications leading to partial blindness, orblindness may be due to corneal involvement (Para-meswamy et al., 1986; Rao and Rodrigues, 1983;Srinivas, 1986; Sharma et al., 1999).
This disease is of great public health importance as it isextremely contagious affecting the eyes in frequentepidemic and pandemic forms. Acute haemorrhagicconjunctivitis (AHC) figures prominently amongophthalmic clinics. Epidemics of AHC have beenreported in India in 1971, 1975, 1979, 1981 and 1984(Ponnuraj and Mukundan, 1989). Almost a million casescould have occurred in India within a few months(Banerjee, 1996).
Conjunctivitis can be due to bacterial, fungal or viralinfection or due to allergy. The most common pathogenin bacterial conjunctivitis is Staphylococcus aureus
(Anonymous, 1966), Chlamydia trachomatis, an obligateintracellular parasite, is a causative agent of primaryconjunctivitis (Madhavan et al., 1996). It is commonlycaused by adenoviruses and to some extent Enterovirus770 (EV 70) and Coxsackie virus A 24 variant(Madhavan, 1999; Taylor et al., 1982). Seasonal allergicconjunctivitis (SAC) is a type I conjunctival allergicresponse to air-borne allergens in hypersensitive indivi-duals (Leino et al., 1992). Chronic non-infectiousconjunctivitis may be caused by any irritant factors suchas foreign bodies, atmospheric contaminants, in-growingeyelashes or even prolonged use of eye drops (VanBijsterveld, 1984).
Sodium cromoglycate (SCG) is an effective treatmentfor allergic conjunctivitis (Ruggieri and Scorcia, 1987).Recently newer antiallergic drugs such as nedocromil,olopatadine and pemirolast have entered the marketplaceas a therapy for allergic conjunctivitis (Yanni et al.,1997). Corticosteroids are the most effective availablepharmacological agents for the suppression of conjuncti-val inflammation but evidence indicates that steroids mayenhance microbial replication and may therefore producea potentially deleterious effect on the disease process(Leibowitz et al., 1976; Nayak et al., 1987). Use ofantibiotics for conjunctivitis can sometimes be injuriousto the occular tissues and may produce resistant bacteriain the long term (Srinivas, 1986). Therefore there is aneed for decongestants with good efficacy, with longerduration of action and minimum side effects.
Ayurveda (the science of life), the Indian system ofmedicine is composed of two words, Ayu (Life) and Veda(Science). It utilizes plant, animal and mineral productsfor preserving and promoting health, and to cure disease.It is a complete science of life and not merely a treatise onmedicines or treatment of disease. All diseases, inAyurveda, are supposed to be the result of imbalanceamong three humors, i.e. vata (air), pitta (bile) and cough(phlegm). The practice of use of herbal drugs in the

treatment of eye ailments dates to the days of Rigveda,Bhrigutantra, Asvini Kumaras, Charak Samhita andSushruta Samhita and is well documented in Ayurvedicliterature (Das et al., 1995). The Ayurveda systempractised in South India is termed as the “Siddha” systemof medicine. It relies more on mineral drugs and has aunique way of processing herbs and minerals. It issupposed to have originated from Lord Shiva. Thepractitioners of this system were supposed to be Sidhs(Saints).
The WHO has also recognized the importance ofherbal cures and has been active in creating guidelinesand standards of botanical medicine (Patwardhan, 2000).It is likely that many of the traditional herbal eyeremedies are harmless and a few may be beneficial. In aphased programme in Zimbabwe, it was decided tocollaborate with the country’s traditional healers topromote research into the value of herbal medicinalremedies used in the treatment of eye diseases (Chana etal., 1994). Stress should be laid on the standardization ofophthalmic herbal drugs so as to lessen toxic side effectsand to ensure efficacy and safety. Indiscriminate use andblind faith in folk medicines may play an important rolein complicating ocular diseases (Taylor et al., 1982).
Herbal eye preparations might contain active com-pounds with beneficial effects, but little work has beendone to isolate and identify these agents (Foster andJohnson, 1994). Herbal remedies for occular diseaseshave not, however, been fully studied on the part ofscientists (Ghosh et al., 1985), and the use of herbal drugsin conjunctivitis has not been reviewed previously. Thus,the present paper is an effort to compile the literaturepertaining to the use of plants in the treatment ofconjunctivitis. Most of the data have been obtained fromthe publication. “The wealth of India” and from“Medicinal and Aromatic plants Abstracts”.
PLANTS AS REMEDIES FOR CONJUNCTIVITS
Numerous ethnopharmacological surveys have beenconducted regarding plant products for various ailments.Table 1 shows a list of plants recorded worldwide to treatconjunctivitis. Table 2 comprises plants used in theSiddha system of medicine against conjunctivitis,whereas Table 3 indicates those further tested clinicallyon the basis of their mention in the Ayurvedic system ofmedicine.
DISCUSSION
Table 1 shows 169 different plant species belonging to 66families used as a cure for conjunctivitis duringethnomedicinal surveys conducted throughout the world.Of these families, Asteraceae has the maximum numberof plant species (17), followed by Euphorbiaceae (11)and then Mimosaceae (8). Therapies of acute conjuncti-vitis include agents effective against Gram-positive andGram-negative bacteria (Mitra et al., 2000). Some plantsused as a cure for conjunctivitis have shown potentantimicrobial activity against different strains of bacteria,fungi and virus. Many plants used as a cure forconjunctivitis possess antiinflammatory activity. This
trend is more common to plants belonging to the familiesAsteraceae and Euphorbiaceae. Some examples ofanticonjunctivitis plants with antimicrobial or antiin-flammatory activity are given below.
In the family Asteraceae, the weed Ageratum con-yzoides contains flavones, polyoxygenated flavones,chromenes and essential oils. Its leaves and stem showedthe presence of stigmasterol and �-sitosterol (Dubey etal., 1989; Riaz et al., 1995; Wandjii et al., 1996). Theessential oil from the leaves of Ageratum conyzoidesshowed a significant antiinflammatory activity (Abeha etal., 1996) and antifungal activity against Penicilliumitalicum (Dixit et al., 1995). The leaves of Blumeabalsamifera have been shown to contain flavonoids andquercetin ethers (Rastogi and Mehrotra, 1993). Theextracts of Calendula officinalis contain phenolic acids,sterols, carotenoids, sugars, flavonoids, triterpenic alco-hols, �-sitosterol glucoside, stigmasterol glucoside,isofucosterol glucoside, taraxsterol, �-taraxsterol andtheir esters, calenduloside A, B, E, F, G and H.Calenduloside B showed antiulcer, antiinflammatoryand neurotropic activities (Rastogi and Mehrotra,1991). It was confirmed in additional studies that theseextracts contain compounds with antimicrobial activityagainst Staphylococcus aureus (Gora et al., 1980;Kasprzyk and Wojciechowski, 1967; Kasprzyk andPyrek, 1968; Wojciechowski et al., 1972; Swiatek andGora, 1978; Dumenil et al., 1980). Russian investigatorsfound that sterile preparations of Calendula extractsalleviated the signs of chronic conjunctivitis and otherocular inflammatory conditions in rats; the extracts alsohad a systemic antiinflammatory effect (Olin, 1992). Thefresh leaf juice of Calendula officinalis showed analgesicand antiinflammatory activities in rats (Hore et al., 1994).The faradiol monoester of a purified triterpenoid fractionof the flowers has been shown to play an important role inantiinflammatory activity (Della Loggia et al., 1991). Theflowers of Carthamus tinctorius possess luteolin glyco-sides, chalcones, sitosterol alkaloids, fatty acids and theplant has shown antiinflammatory activity (Murty et al.,1962: Seshadri and Thakur, 1960: Rastogi and Mehrotra,1993; Verma et al., 1997). The aerial parts of Cichoriumintybus contain flavonoid glycosides, chlorogenic, neo-chlorogenic, 3-feruloyl-and 3-p-coumaroylquininic, di-caffeoyltartaric acid and sesquiterpene lactones. Theinflorescence contains cichoriin, esculin, 6,7-dihydroxy-coumarin, umbelliferone and scopoletin (Rastogi andMehrotra, 1991; de Kraker et al., 1999). From the aerialparts of Eclipta alba �-amyrin, wedelolactone, luteolin-7-O-glucoside, ecliptasaponin D have been isolated,whereas from the roots thiophene derivatives have beenisolated (Rastogi and Mehrotra, 1993: Zhang et al.,1997). The seeds of Eclipta alba were found to be activeagainst Pseudomonas cichorii, Bacillus subtilis, Salmo-nella typhimurium and Escherichia coli (Kumar et al.,1997). The plant also showed antifungal activity againstAspergillus flavus, A. niger, Fusarium solani, F.oxysporum, Colletotrichum capsici, Alternaria alternata,Gymnascella dankaliansis, Sclerotium oryzae and My-rothecium verrucaria (Thyagarajan and Krishnaswamy,1999). Fresh juice along with the methanol extract ofEmilia sonchifolia (Shylesh and Padikkala, 1999) andcaffeoylquinic acids from the whole plant of Vernoniacinerea (Abeysekera et al., 1999) showed antiinflamma-tory activity. Six pyrethrins, pyrethin I, cinerin I,jasmolin I, pyrethin II, cinerin II, jasmolin II, were also

isolated from Vernonia cinerea (Sarin, 1995). The plantSphaeranthus indicus has been reported to contain abitter alkaloid sphaeranthine. The inflorescence containsglucoside, �-sitosterol, stigmasterol, n-triacontanol, n-pentacosane and �-sitosterol (Anonymous, 1998). Thebenzene extract of Sphaeranthus indicus exhibitedantibacterial activity against Escherichia coli, Proteusvulgaris, Salmonella typhii, Bacillus subtilis and Staphy-lococcus aureus (Mahajan et al., 1999). Tridax procum-bens contains sterols, tannins, sugars and flavones. Theflowers contain luteolin and glucoluteolin (Kasture andWadodkar, 1971; Yadava and Saurabh, 1998), whereasthe alcohol extract of the plant contains fumaric acid(Gaitonde and Airan, 1961). The aqueous extract of theleaves showed antiinflammatory activity (Margaret et al.,1998).
The aerial parts of Xanthium strumarium containalkaloids, flavonoids, saponins, coumarins, triterpenoids,xanthatin, 8-epi-xanthatin, 8-epi-tomentosin, xantanolideand xanthane-type sesquiterpenoids (Ahn et al., 1995;Khalid et al., 1998; Mahmoud, 1998), whereas theaqueous acetone extract contained thiazine and phenoliccompounds (Ma et al., 1998). The aerial parts showedantimicrobial activity against Proteus vulgaris, Staphy-lococcus aureus, S. faecalis, Salmonella typhy para B, S.typhimurium, citrobacter freundii, Escherichia coli,Bacillus subtilis, Candida albicans and C. pseudotropi-calis, which was attributed to the presence of thesesquiterpene xanthonol (Jawad et al., 1988; Khalid etal., 1998).
In the Euphorbiaceae family, leaf extracts of Emblicaofficinalis displayed antiinflammatory activity in differ-ent studies (Summanen et al., 1993). From Euphorbiahirta leucocyanidol, quercitol, camphol, quercetin andquercitol derivatives containing rhamnose and chloro-phenolic acid were isolated (Rastogi and Mehrotra,1991). The methanol extract of flowers displayedantibacterial activity against Shigella flexneri and S.dysenteriae (Vijaya et al., 1995). The leaves of Jatrophacurcas contain stigmasterol, �-sitosterol, triterpenealcohol and flavonoidal glycosides (Khafagy et al.,1977) while its latex was found to contain a novel cyclicoctapeptide curcacycline A (Van den Berg et al., 1995).Phyllanthus fraternus contains norsecurinine (alkaloid),phyllanthin, hypophyllanthin and flavonoids (Chauhan etal., 1977), whereas the leaves contain flavonoids,terpenoids and lignans (Rastogi and Mehrotra, 1991). Itshowed antibacterial activity against Escherichia coli andStaphylococcus epidermidis (Thomes et al., 1999) andthe alcohol extracts of the leaves and roots were activeagainst Micrococcus pyrogenes and Escherichia coli(Ahmed, 1998). The seeds of Ricinus communis containlipids, phosphotides and non saponifiable matter (Rastogiand Mehrotra, 1991).
In the family Mimosaceae, Acacia catechu has beenshown to contain kaempferol, dihydro kaempferol,taxifolin, isorhamnetin, (�)-afzelchin, quercetin, (�)-epicatechin and a dimeric procyanidin (AC) (Deshpandeand Patil, 1981). The bark of Acacia nilotica containspolyphenolic phlobaphenes consisting mainly of catecholand pyrogallol, flavones, tannins and glycosides while theflowers have shown stearic acid, kaempferol-3-glucoside,isoquercitrin, leucocyanidin and phenolic compounds(Bhargava et al., 1998; Rastogi and Mehrotra, 1991;Saleem et al., 1998). Its antifungal activity againstFusarium oxysporum has been attributed to the
presence of polyphenolic substances (Bhargava et al.,1998).
The Papaveraceous plant Argemone mexicana isrecorded in 11 different studies from various locationsindicating some important constituent in the latex isresponsible for biological activity. A 50% ethanol extractand the flavonoid, isorhamnetin, from the latex of theplant was found to be antifungal against Candidaalbicans and antibacterial in nature against Escherichiacoli, Staphylococcus albus and Streptococcus faecalis(Khanna et al., 1980). The alcohol extract of the entireplant of A. mexicana was found to possess antiviral andantibacterial properties. The aqueous extracts of flowersand leaves possess weak antibiotic properties, whereasthat of the bark possesses antiviral property. The alcoholextract of fresh leaves and roots inhibits the growth ofMicrococcus pyrogenes var. aureus (Anonymous, 1985).Chemically it contains the alkaloids berberine andprotopine and is used as an antibiotic in allergicconjunctivitis (Sahoo, 1995). Based on its propertiesand utility in the Ayurvedic system of medicine it isconsidered equivalent to sulpha drugs or penicillin(Sharma et al., 2000) although no studies were founddirectly comparing it with these drugs. Whole plant of A.mexicana showed alkaloids (�)-cheilanthifoline, proto-pine and norsanguinarine (Tripathi et al., 1999). Thechemical constituents of the stem and leaves areprotopine nitrate, berberine nitrate, ceryl alcohol, �-sitosterol, succinic acid, citric acid, tartaric acid, malicacid, arginine, leucine, alanine, tyrosine, glycine, alanine,valine, phenyl alanine, glutamic acid, glucose, fructose,potassium nitrate, calcium phosphate and calciumsulphate (Mishra et al., 1961). The latex contains thealkaloids berberine and protopine. It has also shown thepresence of L-glutamine, hydroxyproline, threonine, �-alanine and methionine as free amino acids (Santra andSaoji, 1971).
In addition to the Ayurvedic drugs mentioned in Table3, which have been clinically tested, some other drugscommonly employed for treating conjunctivitis are asmentioned below (Anonymous, 1978).
1. Intake of 100 mL of milk boiled in the decoction of‘Triphala’ or 12–24 g ghee boiled in the decoction of‘Triphala’, twice a day (a combination of equal parts offruit rinds of Terminalia bellerica Roxb., T. chebulaRetz. (Combretaceae) and Emblica officinalis Gaertn.(Euphorbiaceae).
2. 12–24 g of ghee boiled in the decoction of ‘Dasamula’twice a day. Dasamula is a combination of 10 roots inequal parts of the following plants: aegle Haliumarmelos Correa. (Rutaceae), Desmodium gangeticumDC. (Papilionaceae), Gmelina arborea L. (Verbena-ceae), Oroxylum indicum Vent. (Bignoniaceae),Premna integrifolia L. (Verbenaceae), Solanum indi-cum L. (Solanaceae), Solanum xanthocarpum Schrad.& Wendl. (Solanaceae), Stereospermum suaveolensDC. (Bignoniaceae), Tribulus terrestris L. (Zygophyl-laceae) and Uraria picta Desv. (Papilionaceae).
3. 100–250 mL of milk boiled with 12–24 g of the root ofAsparagus racemosus Willd. (Liliaceae) and 7 mL ofrhizome juice of Zingiber officinale Rosc. (Zingibera-ceae) is taken twice a day.
4. The eye is fomented with cotton dipped in warmgingelly oil [oil from the seeds of Sesamum indicum L.(Pedaliaceae)].

CONCLUSION
Conjunctivitis is a highly infectious disease and canspread in an epidemic form. Corticosteroids bring a quickresponse, but the prolonged use of steroids, precipitates
serious complications like secondary glaucoma, postpollen cataract and proliferation of bacteria and viruses.Thus there is a need to look for sources of drugs, whichare newer, well tolerated and non toxic. More than 169plants worldwide are claimed as remedies for conjuncti-vitis based on ethnomedicinal usage. Even though there isenough literature available regarding numerous simpleherbal remedies in traditional medicine very little by wayof systematic clinical aspects of these plants has beeninvestigated. There is a good possibility of finding a newdrug from one of these traditionally used plants,especially the latex of Argemone mexicana since thisplant is claimed as a cure for conjunctivitis over a widegeographical area as studied by different authors.
