What does all this mean?
1- Patients with 30< GFR < 45 can now receive full dose contrast MRIs. Previously half dose or no contrast
2- Patients with 30< GFR < 45 who require a CT, now can get a C+ MRI instead of C- CT. Huge diagnostic advantage. Examples:
CTAs (hepatic arteries, PE studies) Abscesses, septic workups… Acute abdomen, Acute chest syndrome…
How can I take advantage? Keep in mind to request MRI C+ for all
patients unless GFR < 30 Greatly simplifies ordering
Keep in mind to ask for MRI C+ instead of the “limited” CT non-contrast if 30< GFR < 45 .
Liberally call the radiologists for advice until the practice is well established.
