• Service coordination for those transitioning and transitioned from nursing facilities• Linking resources to independent living for those transitioning and transitioned

from nursing facilities• Support and counseling for those transitioning and transitioned from nursing facilities• Educational opportunities for people with disabilities • Crisis intervention • Community outreach to prevent isolation• Conflict resolution • Advocacy and mentoring • Information and referral to community and health/wellness resources
