Established in 2000, Hazelwood Care Home’s comprehensive facilities allow us to

provide general nursing care for older people on Primrose Unit on the ground fl oor, as well

as dementia care with nursing needs onPoppy Unit on the fi rst fl oor.

It’s our aim to make residents feel like Hazelwood is their home through a warm,

welcoming and friendly environment. We want residents to feel settled from

day one, no matter what their individual needs are by providing a life-long service.

Hazelwood Care Home caters for everything you would expect and a little bit more with

50 tastefully decorated bedrooms with en-suite, a beauty salon, cinema room, dining and private

dining areas as well as a sensory room and private gardens – you’ll be hard pushed not to fi nd

something to do any day of the week.

Hazelwood o� ers a comfortable and safe environment for elderly individuals, who can enjoy the benefi ts of group living. We pride ourselves in respecting privacy and choice, while promoting independence at all times.

The well-being of our residents isour top priority. Our passionate team

pride themselves in providing the highest quality of care and all of our

sta� are fully qualifi ed and pro-active in their approach. We o� er exemplary

standards of Nursing, Residential, Dementia, Respite and

Palliative Care.

Each resident is assigned a Primary Care Nurse, who will discuss and

assess with them their needs, as well as develop a Person Centred Plan and monitor and review their care. All our

sta� are chosen for their maturity, warmth and caring attitude.

O U R C A R E

Our in-house Chef and kitchen sta� will cater for all dietary requirements, providing a full menu choice for breakfast, lunch and tea.

Residents can also enjoy a meal together with relatives and friends if they wish.

There’s always lots to do at Hazelwoodwe have Activities Co-ordinators who

encourage residents to take part in group activities, such as exercise, bingo, quizzes

and listening to music.

Our Activities Co-ordinators also spend time one-to-one with residents concentrating on person-centred interests. We enjoy regular

visits from outside entertainers including pianists and singers.
