• Hypothesis: sitravatinib can be administered in combination with nivolumab at an effective dose and augment nivolumab responses in patients with metastatic clear cell RCC who progressed on anti-angiogenic therapy.

OBJECTIVES/ENDPOINTS• PRIMARY

• Dose-finding based on safety (no DLT within 12 weeks) and efficacy (no PD within 6 weeks)

• SECONDARY• Efficacy endpoints including ORR (by two consecutive

measurements), PFS, OS• Quality of life & psychosocial measures: FACT, SF-12, CES-D

(depression), history of depression, FMCS• EXPLORATORY

• Circulating PD-L1, immune cell populations and cytokines

• Tumor cell PD-L1 expression, tumor infiltrating immune cell populations & gene expression signatures

• Tumor gene alterations in circulation & in tumor tissue

KEY ELIGIBILITY CRITERIA• Similar eligibility criteria to those used for

CheckMate 025• Histologically-confirmed clear cell RCC • Disease progression after 1-2 prior anti-angiogenic

therapies (prior cytokine therapy allowed)

• Measurable disease, as per RECIST Version 1.1

• KPS ≥70

STATISTICS• Dose-finding is performed using the sequentially

adaptive phase I-II late-onset EffTox (Lo-EffTox) trade-off-based design of Jin et al. J Am Stat Assoc2014;109:525-36.

Tumors Included:Metastatic RCC with a

Clear cell component and PD after VEGF tx

Four sitravatinib dose levels: 60, 80, 120 and 150 mg/day

1st cohort: 80 mg PO daily

2 weeks to achieve steady state

Day 15:Begin nivolumab 240 mg q2 wks (after week 12: can switch to nivolumab

480 mg q4 wks)

Peripheral blood and optional tissue sample for correlative studies

Toxicities

• Toxicities requiring dose-reduction of sitravatinib have been noted in 21/40 pts (52.5%).

• 15/21 (71.4%) of these DLTs occurred within 12 weeks and were thus captured by the late-onset EffTox design.

• All three patients on the 150 mg dose of sitravatinib experienced DLT within 12 weeks.

Conclusions

• In this single-arm phase I/II study of sitravatinib + nivolumab using similar enrollment criteria as Checkmate 025, we observed higher ORR and longer PFS than historically reported with single-agent nivolumab or single-agent cabozantinib.

• The combination has an acceptable toxicity profile with manageable AEs.

• Sitravatinib 120 mg was the most frequently selected dose by the EffTox algorithm and can be used as the RP2D.

• Analyses of patient-reported outcomes and tissue & blood correlative studies are ongoing.

• Sitravatinib 120 mg + nivolumab is now being explored in other tumor types.
