A MESSAGE FROM THE CEO OF ACHIEVEHello Everyone!
As summer comes to a close, we here at ACHIEVE find ourselves gearing up for a very busy autumn and winter season.
Let me share with you some of the initiatives we are currently working on.
As some of you know, OPWDD is developing a People First Care Coordination System. The driving factor for this is the federal government’s requirement for “conflict free care management” which means that a person cannot receive care management from the same agency that provides services. As such, OPWDD and the Department of Health are seeking applications from entities who wish to establish Coordinated Care Organizations which will combine OPWDD services with health and wellness services, behavioral health and medication management, require development of a single person-centered Life Plan, focus on measuring and monitoring performance, and will establish the pathway to Managed Care for the individuals we support. It is anticipated applications will be due in November, and the Coordinated Care Organizations-Health Home will begin operations next summer. Existing MSCs will be offered the opportunity to transition to CCOs after development. ACHIEVE will be working with our staff as we navigate this transition in the next few months.
ACHIEVE is also partnering with 2 + 4 Construction on the development of an affordable housing complex in Owego, New York. 10 apartments will be made available to individuals with intellectual and disabilities, and we are excited to be able to offer these independent living opportunities to the individuals we support. We hope that the project will begin early 2018.
ACHIEVE is also now a partner with HealthLink, New York, a Rural Health Information Organization (RHIO). We believe this partnership will help us create better communication with health care providers in our community, to better support the overall health of the individuals we support.
I want to thank everyone who supported our most recent fund raising event – Savor the Summer. A wonderful evening of fine wine, craft beer, gourmet food and wonderful entertainment was enjoyed by all, capped by fireworks at the close of the evening. So many individuals and businesses came together to support this event, and we thank you and convey our appreciation for your generosity.
Lastly, we continue our transition of our employment centers from “Production to Instruction”. While we no longer will be operating our manufacturing operations, we will continue to provide a continuum of services that will support skill enhancement of the individuals we support to help prepare them for future vocational/employment opportunities. Planning meetings are ongoing to support individuals through this transition.
As we bid good-bye to the remaining days of summer, know that we at ACHIEVE will continue working for and with the individuals and families we support to enhance the quality of their lives with rewarding opportunities.
Thank you all again for your support!
Warm regards, Mary Jo

NYSARC Grant AnnouncementACHIEVE Broome-Chenango-Tioga was awarded $105,500 in grants from the Trustee Management Board of NYSARC Trust Services to support ACHIEVE’s guardianship and recreation programs.
NYSARC Trust Services administers Supplemental Needs Trusts (SNT) that can make dramatic improvements to the lives of peoples with disabilities while enabling individuals to maintain eligibility for public benefits programs, such as Medicaid and SSI. For 2017, NYSARC Trust Services is proud to fulfill its commitment to enhancing the lives of people with disabilities by awarding a total of $2,793,500 in grants to NYSARC Chapters statewide.
This year, ACHIEVE received $75,500 to provide guardianship services for people with intellectual and developmental disabilities who would not otherwise have a legal guardian or advocate. Supported by Chapter staff and community volunteers, ACHIEVE’s Guardianship program currently provides guardianship services to 15 people and is ready to assist 4 more as a Standby or Alternate Standby guardian. The guardianship program provides information on Guardianship and Supported Decision Making to
individuals, families and the broader community. This includes information on Trusts, Wills, and Health Care Proxy.
NYSARC Trust Services also awarded ACHIEVE $30,000 to develop new recreational opportunities to enrich the lives of the more than 2,200 members it serves. ACHIEVE plans to use the recreational grant funds to expand community based inclusive recreation opportunities for participants. With finances being the largest barrier for folks enrolled in this program, grant funds will significantly lower participants’ out of pocket expenses. The program would also like to acquire adaptive recreation equipment, which will offer additional meaningful experiences.
NYSARC Trust Services: NYSARC Trust Services has administered supplemental needs trusts since 1972, helping more than 16,000 people with disabilities protect their eligibility for government benefits and improve their quality of life. Our trust programs give people with disabilities the opportunity to remain in their communities with greater comfort and independence while providing peace of mind to their family and loved ones.

ACHIEVE Savor the Summer Chili
ACHIEVE Day Hab participants Cliff Conger and Ed Tripp spent the week leading up to ACHIEVE’s 8th Annual Savor the Summer preparing All American Achieve Chili to serve to all of the guests in attendance. Speech Therapist John Adams and Food Service Manager Donna Harchar were on hand to help out as Chef Ed and Chef Cliff created their wonderful chili. Chef Ed and Chef Cliff were joined by both John and Jill Adams the night of the event as they served the 400 guests in attendance the delicious chili they had spent the week cooking.
What is Supported Decision Making?Supported Decision Making or SDM is a way to formalize the way that all of us make decisions. If we are thinking about buying a new vehicle or taking a new job there is much to consider when making decisions. We may do some research, make a list of benefits and drawbacks, ask friends and family members whose opinion we trust. A supported decision making agreement or SDMA for the individuals we support lets them remain as the Decision Maker and helps them to identify who those trusted partners are. The agreement identifies who the supporters will be; it identifies what areas of decision making they will help with, and what kind of information they will provide. For example an individual may identify their mother as a resource for financial decisions and they will help with banking and paying bills and they may identify an Aunt as a supporter for relationship issues and they will provide education and resources for birth control. SDM is a way to empower and support the individuals we serve to make decisions on their own. As always if you have questions about guardianship or supported decision making feel free to call or email me.
