2 Ways to Make a Claim
1. Online Claims• Claims under $2,500• Scanned receipts required
2. Paper Claims• Claims over $2,500must be mailed• Claims under $2,500 may be mailed if you prefer

A complete claim form includes:
‐ Your name, policy number and date of birth
‐ Reimbursement information (who do we pay)
‐ Original / scanned invoices from the healthcare provider
‐ Receipts showing proof of payment
‐ Your signature (or your guardian’s) and the date

With the introduction of our new reloadable international insurance Virtual Prepaid Card, students will benefit from: A reduced claim‐to‐payment timeline, and an innovative offering for a better experience.
• When your claim is approved, a virtual card will be created ,loaded and delivered to you via email within minutes.
• Once your virtual card has been activated online if you choose, a custom physical card it will be mailed to you.
• If you choose to receive a physical card, once the physical card has been activated, funds from the virtual card will be available on the physical card.
• Virtual cards can be used for online transactions. Physical cards can be used for both instore and online transactions wherever Mastercard is accepted worldwide.
