This leaflet tells you about thefree NHS Health Check.

A free NHS Health Check will help youknow more about your health and howto keep fit and well.

It could stop you getting heart disease,diabetes or kidney disease or having astroke.

This leaflet is an easy read version of alonger leaflet about NHS Health Checks.

You may like to have someone to supportyou when you look at this leaflet.

Who should have a healthcheck?

If you are between 40 and 74 you will beasked to come for a health check.

Other people can ask for a health checkif they are worried about their health.

If you already have heart or kidney diseaseor diabetes or have had a stroke then youwill not be asked to come fora health check.

Health checks happen every 5 years.

Why do I need a healthcheck?

Anyone can get heart disease or diabetesor kidney disease or have a stroke.

As you get older you are more at risk ofgetting these illnesses.

There are some things that can makeyou more likely to get these illnesses.These are:

l being fat

l not keeping fit

l not eating a good diet

l smoking

l drinking too much alcohol

l having high blood pressure

l having too much fat in your blood,called high cholesterol.

You can have these things if you are aman or a woman and they may cause:

l a stroke

l a heart attack

l kidney disease

l diabetes

l certain types of dementia.

Having a health check can help stop yougetting these illnesses and keep you fit andwell.

Even if you feel fit and well it is a good ideato have a health check. We can catchsome illnesses early on before you knowyou have them.

Some illnesses run in families. It is a goodidea to have a health check as you may bemore at risk of getting some things.

What happens at a healthcheck?

The health check takes 20 to 30 minutes.

We will ask you some questions about yourhealth and illnesses you have had.

We will check how tall you are, howheavy you are and some other questionsabout you.

We will take your blood pressure.

We will take some blood to check yourcholesterol.

What happens after the healthcheck?

After the health check we will tell you whatwe found out.

We will talk to you about your risk of gettingany illnesses.

We will talk to you about how you can keepfit and well. And how we can help you keepfit and well.

You may have to come back another dayfor this.

If we found you have high blood pressurewe will check your kidneys are ok by takingsome blood.

We may have to take some blood to checkfor diabetes.

We may give you medicine to stay fit andwell. But this is something your nurse ordoctor can discuss with you.

If you smoke we can help you tryand stop.
