• PEO = Professional Employer Organization is an organization that provides a cost-effective approach to the administration of human resources (aka “benefits”).

• Physician Solutions, LP = The entity that IC providers will join via a simple contract and become Partners/Owners in order to obtain benefit options. Physician Solutions, LP is a client of LBMC Employment Partners.

• LBMC Employment Partners = A division of LBMC (Lattimore, Black, Morgan & Cain) to provide HR outsourcing services. One of their service offerings is the PEO.

Overview of Enrollment Process for Physician Solutions

• Step 1: The main contact with Physician Solutions will email you the following items that must be completed/signed and returned to us before proceeding:

• Partnership agreement for Physician Solutions, LP

• Banking Authorization form for the account you would like us to deduct funds from

• An initial capital contribution check for $2,000 (must be a physical check mailed)

Overview of Enrollment Process for Physician Solutions

• Step 2: Complete online forms for our HR compliance, including:

• Application• Direct Deposit Form (if you want remaining funds returned to the same account,

please just write “Same account” on the form and SIGN this)• Partner Information Form• I-9• * If you have already sent a voided check in Step 1 and are using the same

account, you do not have to include another check in this step.

• Step 3: Complete online benefits enrollment including entering dependents and electing or waiving insurance options. You will receive a confirmation email when this step is complete.

• * Insurance cards will be mailed after this step is complete, but if you have not received them and are in need of a temporary card, please contact Kathy Waggoner at [email protected] to request this card.

• The entire enrollment process should not take you more than 30 minutes – 1 hour to complete. Email [email protected] for next steps in the enrollment process.

ICs joins Physician Solutions, LP through simple contract and initial capital contribution of $2,000.

ICs completes ACH authorization for additional $2,500 per month draft to occur(drafts will be the 1st business day of the month). IC enrolls for benefits online and

completes the remaining forms.

The money deducted will flow from the Physician Solutions’ account to LBMC EmploymentPartners where insurance premiums and administrative fees will be paid and a direct deposit

will be returned back to the IC on the 15th and last business day of each month.

ICs will work continuously with the LBMC Employment Partners HR/Benefits Manager toassist with any benefit questions.

Benefit selections will continue through the remainder of the calendar year*, and ICs willrenew or change benefits for the following calendar year at annual open enrollment.

*unless proof of qualifying event such as birth, marriage, divorce, job loss/change, etc.

The Process – Understanding How it WorksExample A – Dr. John Smith

Dr. Smith joins Physician Solutions, LP through a simple legal contract

When initially joining Physician Solutions, he writes a check for $2,000 from his personal bank account – this is his initial capital contribution.

He needs to cover his wife and 2 children so he chooses Option 2 – family coverage for health, as well as dental and vision insurance.

On the first day of each month, an ACH draft of $2,500 is made.

Option 2: Family Deductible of $6,600Funds are received and the following deductions occur:

Health Insurance (Family – Option 2) $1,766.70+ Dental Insurance (Family) $99.87+ Vision Insurance (Family) $22.08+ Monthly Administrative Fee $95.00**

Total Cost $1,983.65

Each month thereafter, payments will be made on the 1st of each month through ACH draft

$2,500.00 capital contribution less benefits cost of $1,983.65 = $516.35 returned**Balance will be returned in 2 equal installments of $258.17 via direct deposit on the 15th and last business day of each month

** Administrative/Management fee includes PEO fees, life insurance premium for $30k guarantee issue coverage, workers’ compensation insurance, management fee for Physician Solutions, LP, Life Assistance Program, and Employee Benefits Advocacy Services

Facts You Need to Know & Remember

• Changes can only be made with proof of a Qualifying Event and within 30 days of the event (i.e., marriage, divorce, new baby, loss of other coverage).

• If you want to cancel coverage, you must let us know by the 15th of that month. There are no mid-month cancellations. If notified timely, all plans will be cancelled at the end of the month.

• This is a qualified HDHP; therefore, you cannot be covered as a dependent on someone else’s PPO plan.

• When electing Individual + One or Family Coverage, you are responsible for the Family deductible. Individual deductible doesn’t apply if you have Individual + One or Family Coverage.

• If you elect Medical coverage and want to take advantage of the Health Savings Account with HealthEquity, please elect a deduction amount when completing your online enrollment

• Remember to use In-Network Providers to receive maximum benefit.

Facts You Need to Know & Remember

• Preventive care is covered at 100% in-network.• Once you meet the deductible in a plan year, the plan co-insurance

coverage takes effect (Preventive care does not apply to deductible).• Your out-of-pocket maximum equals your deductible plus your

coinsurance amount.• Step Therapy: For specified drugs, BCBS may require that members first

try the generic or a lower cost brand drug before the requested drug is dispensed.

• Pre-authorization requirements: Certain procedures and drugs may require pre-authorization.

• Due to state insurance laws, IC’s residing in Nevada or Hawaii are not eligible for Physician Solutions.

The IRS/US Treasury Department has released the 2019 HSA Contribution Limits as follows:

Health Savings Account (HSA) Information

• The funds in your Health Savings Account are yours – you own them. Your tax free dollars can earn interest.

• After your balance reaches $2,000, you will have the option to invest funds.

• Your contributions can be made in a lump sum payment or at your discretion throughout the year.

• You can choose to use your funds to offset your deductible, or you can continue to let your balance grow.

• Your year-end balance remains and rolls over to the next year. Unused funds continue to earn interest.

• All funds are yours and balances can be saved to use during retirement for qualified medical expenses.
