What is Sutter SeniorCare?To help seniors 55 years and older live safely in their homes, Sutter SeniorCare is a health plan that offers:

• A care team that works together and knows your needs.

• A comprehensive services network, including doctors, hospitals, prescription drugs, rehabilitation, dental and vision.

• A PACE center, designed and built for seniors, where you can socialize with other seniors, enjoy activities, and meet with your care team.

• Transportation and home care services.

How Much Does Sutter SeniorCare Cost?If you qualify for Medi-Cal, either alone or in combination with Medicare, your costs may be fully covered. And if you don’t know if you’re eligible for Medi-Cal, we can help you figure it out.

Participants must receive all needed health care services through the PACE plan network, except in the case of emergency services. Participants may be fully and personally liable for the cost of out-of-network services or services without prior approval.
