9th Annual MHEC Member/Supplier Golf Tournament & Networking Event

This 18 Hole Tournament (scramble format) will be held on Friday, May 18, 2018

12:00 am Raffle Drawing (winners will be posted) 2:00 pm Luncheon & Awards (upon completion of play)

Established in 1999, Blackstone National Golf Club is designed for golfers of all abilities with multiple tees, giving you a choice of how you wish to play. This beautiful Rees Jones designed golf course creates an environment for the game of golf that is aesthetically pleasing with its natural , classic course that blends with its surroundings.

Raffle prizes will be drawn prior to lunch and awards will be given following the luncheon. Please consider bringing an item to
represent your company for the raffle. (Hats, umbrellas, sweatshirts, golf shirts, t-shirts, etc.)
Promote your company! Donate company logoed prizes such as golf balls, tees, etc. to be included in our player swag bags. All gifts are greatly appreciated (175 units required). Please contact [email protected] to arrange for your donation.

If you are an individual player provide just your name and email address leaving all other areas blank. If you are a team sponsor, and are sponsoring more than two teams, please provide additional player names and email address on separate sheet of paper. Note: If you are a team sponsor and would like to invite a specific member player (2 per team sponsorship), please follow the instructions below, otherwise the MHEC will make the team selection.

Team Request (OPTIONAL): If you have a member player or players (2 per team sponsorship) with whom you would like to be paired, please indicate their name and email address above. IMPORTANT (read carefully): You must have contacted the member you desire to be paired with, invited them to play, and the member(s) must register indicating their desire to be paired with you. Team requests must be received by March 15th, 2018. After March 15th, teams and placement will be determined by the MHEC, no exceptions will be possible. PLEASE NOTE: Only the first 50 teams are guaranteed to golf with members . Please register as soon as possible. To be sure that you receive all of the benefits associated with your level of sponsorship you must complete and return this form no later than March 15th, 2018. Please fax your registration to (413) 577-2202. For credit card security please do not email your registration. You must be awarded on an active MHEC contract to participate.
ADDITIONAL INFORMATION (please review): The MHEC makes no guarantees for team selection. Counts must be provided to the Golf Club in advance, therefore no refunds will be possible after May 1, 2018. This form serves as your invoice.
