ABOUT THE SERVICE: It shall be the responsibility of the physician who attended the deceased or the clinic where the person died to prepare the proper Death Certificate and shall order its registration in the Office of the City Civil Registrar.

ABOUT THE SERVICE: The Civil status of a person must be recorded in the Civil Registry. This is to ensure that important rights can be exercised and that the person can receive protection and care from the law. Such contract will have civil effects on both.

For marriage with license, submission of certificate of marriage should be

within fifteen (15) days following the solemnization of marriage. For marriage that is exempted from the license requirement, the prescribed period is thirty (30) days at the city/municipality where the marriage was solemnized.

NOTE: Affidavit of Art 34 is a joint affidavit of the contracting parties that they had been living together as husband and wife for more than five years without the benefit of any lawful wedlock.

If solemnized outside the church, letter request from the couple with the approval of the solemnizing officer and duly notarized by the administering officer.

ABOUT THE SERVICE: A Foundling is a deserted or abandoned infant or a child found by another person whose parent/s, guardian/s, or relatives is/are unknown, or a child admitted in an orphanage or charitable or similar institution with unknown birth and parentage background.

Reglementary Period and place of Registration Finders/charitable institutions within thirty (30) days shall make the registration of foundlings at the Office of the Civil Registrar in the City/Municipality, where the child was found. Any report made after the thirty (30) day period shall be considered late and the concerned party shall be required to submit a sworn statement indicating the circumstances that caused the late reporting to the Civil Registrar.

Certification of the Barangay Captain or Police Blotter regarding the report made by the finder.

f. Issuance of Marriage License

ABOUT THE SERVICE: When applying for a marriage license, each of the contracting, parties shall file separately a sworn application for such license with the proper local Civil Registrar of the place where either one (1) or both of the contracting parties reside.

DELAYED REGISTRATION OF DEATH

ABOUT THE SERVICE: The Office of the City Civil Registrar accepts delayed registration of death.

WHO MAY AVAIL OF THE SERVICE: Nearest of Kin, hospital clinic Administrator, attendant-death, or authorized

representative of the family.

REQUIREMENTS:a) Four (4) copies of certificate of Death which must be accomplished correctly and completelyb) Affidavit for delayed registration which shall be executed by hospital/clinic administrator if the person died in the hospital/clinic or similar institution, or if the person died elsewhere, by the attendant at death.In default of the hospital/clinic administrator or attendant at death, the affidavit shall be executed by any of the nearest relative of the deceased, or by any person having legal charge of the deceased when he was still alive;c) The affidavit referred to in (b) shall state among other things, the name of deceased, the facts of his death, the facts of his death, the date and place of burial or cremation, and the circumstances why the death was not reported for registration within thirty (30) days after death;

j. Registration of Legal Instruments

ABOUT THE SERVICE: This sworn statement in the form of an affidavit which affects the civil status of person. These acts are executed without an order from the court. As a general rule, all legal instruments shall be registered in the Civil Registry of the place where they were executed except the following places: Affidavit of Reappearance – where the parties to the subsequent marriage are

residing. Marriage Settlement – where the marriage was recorded Admission of Paternity, Acknowledgement, Legitimations, Voluntary

m. Issuance of Endorsement of Civil Registry Record to the Office of Civil Registrar – General (OCRG)

ABOUT THE SERVICE: Generally, all City / Municipal Civil Registrars submit Civil Registry documents to the Office of the Civil Registrar – General (OCRG) through their respective National Statistic Office – NSO Provincial Office every 1st

ten (10) days of each months, copy of entries made during the preceding month for filing. There are times or instances where the National Statistic Office cannot issue requested SECPA copies (security papers) because records are not available and requested copy cannot be located in their archives or requested documents are still being processed by the Provincial office and has not been forwarded yet to NSO –Manila. Endorsement is also made if COLB is without a given name or 1st name. To facilitate the issuance of requested documents, the Civil Registrar is requested to submit a letter of endorsement to NSO – Manila thru NSO – Bacolod.
