6th Grade Science Camp Packet Instructions &Detailed Information YMCA Camp Orkila: October 1-4, 2019 1. Camp permission forms are due by 3pm on Friday, September 13th. The main district permission forms are located online at FinalForms; the behavior contract is included with this paper packet. Any form that is mentioned below but not included in this packet or on FinalForms is available on the Chinook website or from the main office. *NOTE: We are aware of the conflict with Rosh Hashanah; if this affects your family, please contact us at ChinookCamp(a~bsd405.org to discuss options.* 2. If your child takes any medication (over-the-counter or prescription), and we do not already have a Medication Authorization Form on file at Chinook, you and your health care provider must complete and sign adistrict-provided "Medication Authorization Form". You may hand-deliver, mail or fax the form to your health care provider. The health care provider may fax the form to Chinook at 425-456-6304 or return it to you to submit. The form must be submitted to Chinook's nurse by Friday, September 13th, and the actual medication is due by Wednesday, September 18th. It is imperative that the nurse is allowed time to review the forms and medications and prepare instructions for the Chinook staff. Students may not self - medicate while at camp (some exceptions apply; ask for details). 3. Complete the Release of Liability form online at IlttpS:~~blt.ly/2m5xr99 This link will also be sent via email. A paper copy of the form will be provided upon request. * *Your student will not be allowed to participate in any camp activities until this form is complete. 4. If you have any dietary concerns, review the Dietary Restrictions Form on the Chinook website. If your student has any restrictions, please email the required information to hwinchell~~seattleymca.org by September 13t h. Camp Orkila is completely nut-free! If your student has no restrictions, you do not need to do anything. Please contact us at ChinookCamp~C~bsd4o5.org if you have any further questions. 5. Review the "What to Bring Checklist". Please read it carefully, including the "Do Not Bring" section. If you have difficulty acquiring any of the items, please let Mrs. Erickson, Mrs. Horsfall or your student's counselor know as soon as possible so we can arrange extras. Activities will take place rain or shine. Please pack for warm days and cold evenings. Dressing in layers is best -don't forget a rain jacket! 6. Camp 4rkila Packing Checklist A few words of advice:

✓ Activities happen rain or shine so be sure to pack appropriately! ✓ If you are missing something'on the required list, Falk to Mrs. Horsfall, Mrs. Erickson or your counselor. ✓ Bring old clothes. New clothes will come home looking very old and/or dirty! ✓ You may put at most ONE suitcase or duffel bag and ONE bag with your sleeping bag or bedroll on the

luggage truck, and carry ONE small daypack/lunch onto the bus. ✓ MARK YOUR NAME ON EVERYTHING!

REQUIRED ITEMS ❑ Sack lunch for the first day (can be ordered through the front office by 9/20). DO NOT PACK YOUR LUNCH with

your luggage. Carry it in your hand or your daypack onto the bus on Tuesday. Note: You will need to discard all food waste/leftovers and containers prior to entering your cabin; please pack accordingly!

Bedding: ❑ Warm sleeping bag or 3-blanket bedroll — in a bay marked with student's name (sturdy black trash bag is okay,

but pack a second for the return trip as they are often not reusable) ❑ Pillow —pack this either in your suitcase/duffel bag or with your sleeping bag/blankets

Clothing: D Pajamas ❑ Sturdy walking shoes or tennis shoes ❑ Daily changes of socks and underwear ❑ Shirts (both heavy and light weight —layering is important) ❑ Warm sweaters and/or sweatshirt ❑ Long pants (at least 2 pairs) ❑ Raincoat with hood or hat *make sure it is waterproof, not just a windbreaker ❑ Waterproof boots (or an extra pair of sturdy shoes so others can dry) O Warm jacket D Warm hat and gloves/mittens (yes, you may wear hats at camp) ❑ Bathing Suit for shower house (not for swimming) —this is a must!

Toiletries: ❑ Toothbrush and toothpaste ❑ Soap and shampoo/conditioner ❑ Bath towel, hand towel, washcloth ❑ Deodorant ❑ Comb or brush ❑ Sunscreen lotion, chapstick/lip balm

Camp/Class Equipment: O Reusable water bottle for hiking O Daypack —your school backpack or string bag is fine (bring with you on the bus/ferry on Tuesday and Friday) ❑ Flashlight or headlamp and extra batteries (make sure it is labeled with your name) ❑ Sharpened pencils or pens (you will be given a journal to use for notetaking in your classes)

Camp Payment Information —Checks payable to Chinook Middle School You may also pay online with a credit card (instructions below).

The per-student cost of camp including expenses necessary to cover transportation, supplies, and teacher and high school counselor camp fees is $350. This fee has been automatically applied to your student's account.

Please choose the payment plan that best fits your financial situation and/or lunch status. Scholarship support is automatically allocated based on federal free/reduced-price lunch qualification via a Fee Waiver. If

you have applied for Fee Waiver along with Free &Reduced Lunch program, DO NOT pay for camp until you receive confirmation of Free &Reduced Lunch status.

Please contact Abigail Horsfall or Katy Erickson, Camp Coordinators, at ChinookCamp~a bsd405.org or your student's counselor if you have any other financial concerns. Additional scholarships are available upon request, regardless of

free/reduced-price lunch qualification, and extended payment plans are also available with prior arrangement.

Include your STUDENT'S full name on the memo line of the check.

1) Full payment: one payment of $325

2) Regular payment plan: two payments of $162.50 each

o The first payment of $162:50 is due on or before Friday, September 20. o The second payment of $1.62.50 is due on or before Friday, October 18.

3) Suggested payment for students who receive reduced-rice lunch: X100

o The first payment of $50 is due on or before Friday, September 20. o The second payment of $5 0 is due on or before Friday, October 18.

4) Suggested payment for students who receive free lunch: $50

Hit the little round button:

Pay Fees

Username: enter your student's school ID number

Password: enter your student's last name

Example: User Name: 1234567

Password: Jones (case sensitive)

Click on your child's name.

The fee has been assigned to your student's account automatically and shows at the top of the page. Click View

Enter the camp payment amount that is appropriate for your family and/or that has been arranged with Camp

Coordinators or the Office Manager. Select the small Pay box if you have changed the amount. Hit the blue Pay Selected Fines/Fees to put in Checkout.

For online donations, you may request a receipt for tax purposes from the Chinook front office.

BEHAVIOR CONTRACT Sixth Grade Outdoor Environmental Education Camp

Chinook Middle School

I agree to be respectful, responsible, and safe during Sixth Grade OEE Camp. I agree to the following while at Camp Orkila, during the trip to/from Camp Orkila, and during all camp-related activities:

1. I will be responsible and keep camp litter free. 2. I will respect and preserve natural or man-made surroundings. 3. I will treat all living things with respect. I will not collect or damage live plants,

animals, or sea life in any way. 4. I will leave rocks, sticks and other natural materials where I found them. 5. I will keep myself, my surroundings, and my possessions clean and orderly. 6. I will respect the property, belongings, and rights of those around me. 7. I will treat others the way I like to be treated. I know the high school counselors and

Orkila staff are considered guest teachers, and I will be respectful and treat them according to Chinook Middle School R.O.C.K.S guidelines.

8. I will be responsible and leave all electronic equipment and devices at home, including, but not limited to: cell phones, MP3 players/iPods, iPads/tablets, video games, etc.

9. I will be safe and responsible and refrain from eating or storing food in my cabin. 10. I will be safe and only go to my cabin when a counselor is present. 11. I will remember that camp and camp preparation activities are an extension of the

regular school curriculum and that the basic behavior criteria found in the Chinook Student Handbook still apply while at camp.

12. I understand that inappropriate behavior will not be tolerated. Anyone choosing to break any Camp or Chinook rules or guidelines may lose the privilege of being at camp. If this happens, I understand my parent/guardian will be notified and may be required to pick me up at Camp Orkila.
