REGISTRATION FORM

The Prosthodontics Update 2021 event is organised by the Queensland Dental GroupTM ACN 139051362, ABN 32139051362. Participants may use this course to meet CPD requirements as set by the Dental Board of Australia. CPD Verification has also been received from the New Zealand Dental Association (NZDA). For Australian and New Zealand dental practi-tioners, the maximum hours of CPD credit attainable for attending Prosthodontics Update 2021 is 15 hours. The Queensland Dental GroupTM guarantees advertised courses are compliant with the current Dental Board of Australia guidelines on continuing professional development. NZDA verifies, that based on the infor-mation provided to NZDA, these activities meet the Dental Council policy require-ments for CPD. NZDA CPD verification does not imply promotion or endorsement of the contents of any course. Participants need to use professional judgment to assess the validity and usefulness of techniques, materials or therapeutic products to their own practice. Registra-tion for Prosthodontics Update 2021 includes morning refreshments, lunch & afternoon tea. The Early Bird registration rate is availa-ble for the Prosthodontics Update 2021 for registrations received and paid in full by the 31st of March 2021. No sponsorship has been sought for the event to allow all presenters to provide their own unbiased views, opinions and preferences. Registration for this event does not in-clude insurance of any kind and it is strongly recommended that participants arrange their own travel and other insurances. The organisers reserve the right to make changes from time to time and will, if appropriate, make such information available on the website. PROSTHODONTICS UPDATE 2021 THE PRESENTERS

Dr Michael Mandikos is a registered specialist in Prosthodontics. Michael received his Bache-

lor of Dental Science Degree with Honours, from the University of Queensland and in 1998 he completed a three-year residency program at the State University of New York at Buffalo (USA), graduating with a Certificate in Prosthodontics and Masters Degree in Biomaterials. His research was in composite resin materials and he has published several papers in Australian and interna-tional journals on clinical and dental materials topics. Michael was admitted as a Fellow of The Royal Australasian College of Dental Surgeons and aside from his private practice, Michael is a Visiting Specialist Prosthodontist to the University of Queensland Dental School and formerly to the Royal Australian Air Force. He is a Reviewer for several dental journals, and a product evaluator for several dental companies. Michael has presented continuing education programs at Dental meetings throughout Australia, Southeast Asia and the USA.

Dr Christopher Ho is a Specialist Prosthodontist who received his Bachelor of Dental Surgery

with First Class Honours at the University of Sydney. He completed postgraduate studies in the Graduate Diploma in Clinical Dentistry in Oral Implants at the University of Sydney, a Masters of Clinical Dentistry in Prosthodontics with Distinction from the University of London and Doctor-ate in Clinical Dentistry in Prosthodontics from the University of Sydney. He is Fellow of the Pierre Fauchard Academy, Special Field Member of the Royal Australasian College of Dental Surgeons in Prosthodontics and a Fellow of the American College of Dentists. Dr Ho lectures extensively on aesthetic and implant dentistry both nationally and internationally. He contrib-utes to the postgraduate Oral Implant Diploma and Prosthodontic programs at the University of Sydney. He is a Faculty member of the Global Institute for Dental Education and is Visiting Lecturer at Kings College London, United Kingdom. He is author of numerous publications, as well as editor of the Wiley-Blackwell textbook “Practical Procedures in Aesthetic Dentistry” and the upcoming “Practical Procedures in Implant Dentistry”.

Dr Justin Boulton regularly lectures nationally on a range of aesthetic dentistry and Prostho-

dontic topics. Presently, Justin teaches part-time in the postgraduate Prosthodontic Masters Degree at the University of Queensland Dental School and maintains a private prosthodontic practice in Brisbane. He completed an undergraduate degree in 1986 and postgraduate degree in 1992 at the University of Queensland. He is a fellow of the Royal Australasian College of Dental Surgeons and the International College of Dentists.

Dr David Roberts is a past President of the Queensland branch of the Australian Prosthodon-

tic Society, the Australian Osseointergration Society and the current president of the Bennett Movement Dental Study Club. David completed his Bachelor qualification in 1991, graduating with first class honors and four academic awards in his final year. Prior to this he had accepted a commission from the Royal Australian Navy and on graduation took up his position as a Dental officer and served at numerous locations in Australia and with the United Nations in Rwanda. David commenced his full time Masters study in 1996 and graduated in 1998. After completing his specialist training he served his remaining time with the Navy in Canberra and Brisbane. David has been an examiner and lecturer at both undergraduate and postgraduate level at the University of Queensland Dental School and the Griffith University Dental School and he has lectured extensively on Prosthodontics and Dental Implants throughout Australia and interna-tionally. Since 2000, he has conducted a Specialist Prosthodontic practice in South Brisbane and Camp Hill and he has a special interest in full arch implant supported restorations and CADCAM integration in Prosthodontics.

Professor Florian Mack has completed three research degrees by thesis in 1993, 2000 and

2007. His research interests include bio-compatibility of dental materials, tongue dysfunction and laser welding. Florian has published over fifty papers and is a reviewer for various interna-tional dental journals. He is a member of many professional associations and has lectured nationally and internationally. Dr Florian Mack received his dental degree in 1992 from the University of Berlin in Germany. He then worked at University of Berlin from 1993 to 1996 and also part time private practice from 1992 to 1994. In 1996 Professor Mack accepted the position of Assistant Professor at the University of Greifswald in Germany. In 2000 he completed his specialty degree in Prosthodontics in Greifswald. Professor Mack and his family moved to the Gold Coast in 2006 where he accepted the head position of Professor in Prosthodontics and Comprehensive Adult Dental Care at Griffith University. Since mid-2016 he has been working as a full time private practitioner in his specialty of Prosthodontics in Brisbane, Gold Coast and Toowoomba.

Dr Sid Kothari is a registered specialist prosthodontist. After graduating, Sid spent a year in

private practice in North Queensland and three and a half years in public and private practice in Canberra. He pursued a Master’s degree in Advanced Dentistry from the University of Sydney in 2015 - a course that involved both clinical work and a research component, for which he under-took three systematic reviews. Equipped with the skills to read professional dental literature critically and apply that knowledge in clinical practice, Sid enrolled at the University of Otago to study Prosthodontics in 2017 on a prestigious Doctoral Scholarship, graduating with a Doctorate in Clinical Dentistry as a Prosthodontist in 2019. He has published four research articles in various journals which include the Journal of Dentistry and also attained Membership of the Royal Australasian College of Dental Surgeons. Upon graduating, Sid practiced as a specialist Prosthodontist in New Zealand before joining Brisbane Prosthodontics in 2021. Sid enjoys teaching all areas of removable, fixed and implant prosthodontics and has taught in the under-graduate program at the University of Otago and has been invited to speak at a number of CPD events in New Zealand and Australia.
