VTE (Venous Thrombo Embolism) Prevention Team Quality and Patient Safety Institute (Clinical Transformation)CEO Team Award for Quality

Hospitals everywhere struggle with post-operative blood clots. The incidence of venous thrombo-embolism, or VTE, has become an important quality metric in and outside of the organization. To meet this challenge, the Clinical Transformation Institute formed a VTE Prevention Team. It included staff physicians, nursing, pharmacy and the Clinical Systems Office.

The team worked as a unit to develop a data-based solution to identify at-risk patients and provide appropriate interventions. They enhanced prevention, increased accountability, visibility and collaboration among VTE stakeholders.

As a result of their efforts, there were 95 fewer cases of VTE in 2014 – a full 22% reduction. The work of this outstanding team will continue to save lives and prevent suffering in the years ahead.– Nominated by Kristin Mergler

Weston Transplant/OR Team Cleveland Clinic Florida

CEO Team Award for Teamwork Only three years ago, there was no transplant program at Cleveland Clinic Florida. Many patients who needed heart, kidney or liver transplants had to travel outside of South Florida for their care. Once these programs were approved, the Weston Transplant/OR Team hit the ground running. From 16 cases in 2011, they ramped up to 58 in 2014 – with excellent outcomes. The team is on track to perform over 150 or more transplants in 2015. – Nominated by Jaculin Puntel

Cleveland Clinic Abu Dhabi Highlights

Caregivers as far away as Abu Dhabi were also recognized at the Caregiver Celebrations Banquet. Two outstanding teams from Abu Dhabi received awards – The Facilities Management Team, which worked diligently to prepare the facilities for the first patient and the Mass On-Boarding Team which successfully on-boarded 1,500 caregivers over a twelve month period. Dana Dweik, from Clinical Operations in Abu Dhabi received an individual award. Her story is featured below.

Dana Dweik, Clinical Operations

Dana Dweik is an amazing caregiver at Cleveland Clinic Abu Dhabi, (CCAD) and has been instrumental in setting up the Eye Institute (the first service to go live.) She played a critical role in training for the activation initiative and she has an infectious enthusiasm which propels the physicians to action. She has a high level of understand-ing to identify any issues or shortcomings beforehand which helps to avoid delays. She is encouraging, positive, optimistic, detail-oriented and kind. She embodies the spirit of CCAD and truly exemplifies all the values of CCAD as its best.

Patient Recognition Award Highlights Adam Patsko, Radiation TherapistTaussig Cancer InstituteIndividual Award for CompassionThe Patient Recognition program focuses on patient feedback and gives patients, family members and visitors the ability to recognize their caregivers for providing world class care

during their visit. All patient submissions were reviewed and Adam Patsko was selected for this year’s award.

Adam Patsko is a dedicated radiation therapist who cares about his patients.Patients have been so impressed and appreciative of Adam’s compassion that they often take the time to send comments via the Patient Recognition comment card boxes. One such patient, Mr. Albert Belsole shared these words about Adam’s care:“I have completed 35 of 40 radiation treatments for prostate cancer at your main facility. I wanted to recognize one of your employees who has such a caring and compassionate attitude. Adam Patsko has always made feel comfortable with my treatments. I always felt he was very careful when apply-

ing the radiation and he always has a friendly smile. He makes you feel like you’re in the best place for the best care.”

Adam is a role model for all of us and is an example of our “Patients First Mission” in action. He puts patient at ease and gives them confi-dence in Cleveland Clinic. – Nominated by patient, Mr. Albert Belsole

Gladys Gibson Award Highlights Heidi Raynor, Medical Secretary, Heart And Vascular InstituteIndividual Award for Compassion

The Gladys Gibson Award honors Cleveland Clinic caregivers who display extraordinary courage, energy and devotion to the needs of others, are heroic in their performance and exemplify service and self-sacrifice in the tradition of Gladys Gibson, whose bravery and devotion to duty in the disaster of May 15, 1929 can never be forgotten.

Heidi Raynor exemplifies the spirit of Gladys Gibson. She approaches her job with intensity each and every day and is a strong advocate for patients.

On June 18, Heidi received a call from a patient who was in the acute process of having a stroke and could not speak intelligibly. She saw the phone number and recognized it because of the frequent contact with this very sick man. She immediately called EMS to go to his house to assist in the Philadelphia area.

After initiating an EMS dispatch, she found the address listed in our records was not current. Heidi took the time to track down relatives of this patient, who relayed his current address. She again contacted the local EMS and redirected them. EMS broke into the patient’s house and found him in dire straits. He was transported to the hospital and treated for an acute stroke.

Heidi’s resourcefulness, intelligence and compassion saved this patient’s life. Her quick-thinking and follow-through, in the spirit of Gladys Gibson, helped in getting the patient the medical attention he desperately needed. Heidi was a hero that day and is deserving of the 2015 Gladys Gibson Award.

Individual and Team Caregiver Award Highlights

Dean Sauer, Patient Service Representative, Respiratory InstituteIndividual Award for Service Dean Sauer, Patient Service Representative for the Respiratory Institute was calm in a crisis. On January 5th, there was a power outage in the Crile building. In the true spirit of “patients first,” Dean not only escorted a number of patients down the stairs from the 9th floor, but he also ran back upstairs to retrieve a patient’s medical records they had left there. On one of his trips up the stairs, he was stopped where a patient had coded in the stairwell. Dean carried the patient out of the stairwell to an open area where the response team could care for him, in a true hero’s fashion. – Nominated by Ann Parker

Ebola Teams Have Lasting Impact for Cleveland ClinicEbola Caregiver Team, Respiratory InstituteTeam Award for ServiceThe Ebola Caregiver Team from the Respiratory Institute selflessly volunteered to lead the department in preparing for the worst outbreak of the Ebola virus ever seen. While not knowing what Ebola would mean for the United States, this brave team knew that if Ebola patients showed up in Cleveland, they needed to be ready. Realizing all the risks in caring for Ebola patients, this team showed leadership and were willing to make the sacrifices for patients. They underwent weeks of training and countless simulations, often times enduring sixteen hour days. These individuals truly support the Cleveland Clinic’s mission of providing world class care. -Nominated by Ann Parker
